Home My WebLink About20102597.tiff RESOLUTION RE: APPROVE FIVE (5) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with five (5) Amendments to Child Protection Agreements for Services with Various Providers between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the following providers, commencing June 1, 2010, and ending May 31, 2011, with further terms and conditions being as stated in said amendments, and 1. Savio House (Community-Based Services) 2. W. Troy Hause (Mediation Services) 3. Lutheran Family Services of Colorado (Life Skills) 4. Ackerman and Associates, P.C. (Mediation) 5. Transitions Psychology Group, LLC (Mental Health Services) WHEREAS,after review, the Board deems it advisable to approve said amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the five (5)Amendments to Child Protection Agreements for Services with Various Providers between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the above listed providers be, and hereby are, approved. These amended Agreements were presented at the Board's October 25, 2010, Work Session. These Amendments are necessary to stay within the Core Allocation. The major provisions of these amended Agreements are as follows: No. Provider/Term Program Area/Funding Rates I Savio House, Inc. Home Based Services Increase of$20,000.00. (Amendment 1) (Community Based *Decreasing Lutheran Family Services— Child Services Life Skills June 1, 2010—May 31, 2011 Protection) (Visitation) contract by $20,000.00 to add to this Core Agreement. New Max. = $40,000.00 2 W. Troy Hause— Attorney Mediation Increase of$2,500.00. at Law Decreasing Ackerman& (Amendment 1) Core Associates Mediation contract by $2,500.00 to add to this June 1, 2010—May 31, 2011 Agreement. New Max. =$60,000.00 One of the Agreements(Savio-HB) is being funded through a substantial decrease to the Lutheran LS (Visitation) contract. Service Utilization has found the LFS Visitation contract is not being utilized as much as anticipated. Furthermore, it is not desirable to decrease other Home Based contracts any further at this point as many of them are specific to coaching and mentoring services which are expected to be used heavily in attempts to maintain placements throughout the year. It is preferable at this time, and makes more sense from a service utilization perspective, to move funds from a contract that is not expected to be fully utilized, which in this case is the Lutheran LS (Visitation) contract. WITNESSETH WHEREAS,the parties desire to modify their current Agreement for Community Based Services —Child Protection(PY-10-11-CORE-0182)in order to increase the maximum reimbursement under this contract;and WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and WHEREAS,required approval,clearance,and coordination have been accomplished from and with appropriate agencies. NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follow: 1. Exhibit B,Payment Schedule,Item 1,is hereby revised to include the following: The Department agrees to reimburse CONTRACTOR in consideration for the work and services performed from Core Services funding,not to exceed$40,000.00.
