RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT AND
AUTHORIZE CHAIR TO SIGN - MEALS ON WHEELS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment to the Purchase of Services
Agreement between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on
Aging, and Meals on Wheels, commencing March 1, 2010, and ending June 30, 2010, with further
terms and conditions being as stated in said amendment, and
WHEREAS, after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Amendment to the Purchase of Services Agreement between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services,Area Agency on Aging, and Meals on Wheels be,and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 23rd day of June, A.D., 2010, nunc pro tunc March 1, 2010.

1 PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS,AMENDMENT made and entered into this 1st day of March,2010 by and between the Weld County
Department of Human Services' Area Agency on Aging,hereinafter referred to as "AAA" and Meals on Wheels,
hereinafter referred to as "MOW"
WITNESSETH:
WHEREAS, the "AAA"desires to continue to purchase adult day services from"MOW",and
WHEREAS,the"AAA" and"MOW"are desirous of amending the original agreement of July 1, 2009
NOW THEREFORE, it is mutually agreed:
I. The"AAA" will increase the current contract from$72,547,(ARRA,Older American Act and Older Coloradoan
funds)to$102,547,reimbursing"MOW"$4.00 per meal served."MOW"agrees to provide an additional 7 500
units of services for 80 seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from March 1,2010 through June 30,2010. This Amendment will be
terminated immediately in the event of loss of funding.
IN WITNESS WHEREOF,the parties hereto have executed this Amendment on the day first written above.

COLORADO RE: Amendment to the Purchase of Service Agreement between
the Weld County Department of Human Services' Area
Agency on Aging and Meals on Wheels
Enclosed for Board approval is an Amendment to the Purchase of Service Agreement between
the Weld County Weld County Department of Human Services' Area Agency on Aging and
Meals on Wheels. This amendment was presented at the Board's June 21, 2010,Work Session.
The original contract for services was $72,547.00 and this amendment increases the service
agreement to $102,547.00. The increase in the contract will result in an additional 7,500 meals
for 90 frail and homebound Weld County seniors. This amendment is effective from March 1,
2010 through June 30, 2010.
