RE: APPROVE SCOPE OF WORK FOR 2010 PREVENTIVE HEALTH AND HEALTH
SERVICES BLOCK GRANT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with the Scope of Work for the 2010 Preventive
Health and Health Services Block Grant from the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, to the Colorado Department of Public Health and Environment,
commencing April 1, 2010, and ending December 31, 2010,with further terms and conditions being
as stated in said Scope of Work, and
WHEREAS, after review, the Board deems it advisable to approve said Scope of Work, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Scope of Work for the 2010 Preventive Health and Health Services Block
Grant from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, to the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said Scope of Work.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 22nd day of March, A.D., 2010.

SUBJECT: Preventive Health and Health Services
Block Grant Contract
Enclosed for Board review and approval is the Scope of Work Contract for Preventive Health
and Health Services Block Grant funding between the Colorado Department of Public Health
and Environment and the Weld County Department of Public Health and Environment. The time
period for the grant is April 1, 2010 through December 31, 2010. The total amount of the
contract is $55,000.
The Preventive Health Block grant funding will provide partial support to improve access to
healthy foods in a disparate neighborhood in East Greeley/Weld County. The residents of this
area do not have access to a variety of affordable fresh foods that constitute a healthy diet on a
year around basis.
The project will be collaboration with the residents of the East Greeley/Weld County area, three
small corner store food retailers, two elementary schools and local government to improve access
to healthy foods available in the neighborhood while increasing consumer demand for these
products through education and marketing.
The funding will support existing FTE, a .5 registered dietitian/program coordinator, and a .25
OT III.

Grant Award: $55,000
Grant period: April 1,2010—December 31,2010
Scope of Work:
The project will focus on improving access to healthy foods in a low income disparate
neighborhood in East Greeley/Weld County. The residents of this area do not have access to
affordable fresh foods that constitute a healthy diet on a year around basis. We propose to work
with the residents, three small food retailers, two elementary schools and local government to
improve access and increase consumer demand for more fresh fruits and vegetables, multi grain
products and dairy products that are part of a healthy diet.
Outcomes/Goals:
Improve healthy food access and consumption among East Greeley/Weld County area residents by
implementing one or more strategies recommended by CDC in its July 2009 Publication Recommended
Community Strategies and Measurements to Prevent Obesity in the United States.
Services to be performed -
1. Organize an East Greeley Fresh Food Task Force with the East Greeley residents, City of
Greeley, Weld County government, School District Six and local food retailers to identify
partners interested in making healthy foods move available and affordable in East
Greeley by May 15, 2010.
2. By June 30, 2010, conduct purposeful interviewing and surveying of at least 500
residents of East Greeley about their food intake, food access issues, awareness and
interest in local food production and interest in nutrition education programs.
3. By June 30, 2010, conduct a survey of three current food retailers in East Greeley about
access to affordable healthy food supplies and barriers to selling more fresh healthy
foods, what marketing strategies may increase sales and consumption of healthy foods
and what incentives are needed to increase healthy food supplies in their food stores.
4. By September 30,2010, determine which strategies will be most effective to improve
access to healthier foods in this environment based on the information gathered during
the survey process. These may include more local production through expanded local
gardens, farmers market, working with retail suppliers and schools to combine orders and
lower prices, decrease access to unhealthy foods and beverages, purchasing equipment
such as refrigeration units to provide storage for healthy foods, and marketing new
healthy food choices to increase demand.
5. Begin implementation of at least two of the identified strategies to improve healthy food
access and demand in East Greeley with the help of the East Greeley Fresh Food Task
Force by December 31,2010.
6. By December 31,2010, process evaluation activities will document steps and major
milestones toward achieving our goal. Non-probability surveys and qualitative data will
document movement toward intermediate and longer-term outcomes. The initial survey
will estimate baseline data for several health status indicators for adult residents.

Contractor Reporting Requirements:
The Contractor will be required to submit periodic progress reports by the following dates:
July 31, 2010 (for the work period of April 1, 2010 through June 30, 2010)
October 31, 2010 (for the work period of July 1, 2010 through September 30, 2010)
January 31, 2011 (for the work period of October 1, 2010 through December 31, 2010)
Compensation under this Purchase Order:
The contractor will submit invoices for reimbursement on a quarterly basis. Payment during the
initial, and any renewal or extension, term of this Purchase Order shall be conditioned upon
affirmation by the State that all services were rendered by the Contractor in accordance with the
terms of this Purchase Order. Invoices for reimbursement will be submitted by:
July 31,2010 (for the work period of April 1, 2010 through June 30, 2010)
October 31, 2010 (for the work period of July 1, 2010 through September 30, 2010)
January 31, 2011 (for the work period of October 1, 2010 through December 31, 2010)
