RE: APPROVE PURCHASE OF SERVICES AGREEMENT FOR LET'S GET MOVIN' AND
GROVIN' IN 2010 PROJECT AND AUTHORIZE CHAIR TO SIGN -CITY OF GREELEY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Purchase of Services Agreement for the
Let's Get Movin'and Grovin' in 2010 Project between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the City of Greeley, on behalf of the Greeley Senior
Activity Center, commencing upon full execution, and ending December 31,2010,with further terms
and conditions being as stated in said agreement, and
WHEREAS,after review,the Board deems it advisable to approve said agreement, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Purchase of Services Agreement for the Let's Get Movin' and Grovin' in
2010 Project between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on
Aging, and the City of Greeley, on behalf of the Greeley Senior Activity Center, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 24th day of March, A.D., 2010.

COLORADO RE: Purchase of Services Agreement between the Weld County
Department of Human Services' Area Agency on Aging
and the City of Greeley's Greeley Senior Activity Center
Enclosed for Board approval is the Purchase of Service Agreement between the Weld County Department of
Human Services' Area Agency on Aging and the City of Greeley's Greeley Senior Activity Center. This was
presented at the Board's January 6,2010,Work Session.
This grant is to purchase outreach activities and is in response to needs data which shows that a high
percentage of Weld seniors are lonely and isolated from community life. The Greeley Senior Activity Center
has agreed to provide a program called Let's Get Movin' and Grovin' in 2010 that is designed to reach out to
isolated and lonely seniors in their communities. The major provisions of this Agreement are as follows:

PURCHASE OF SERVICES AGREEMENT

THIS AGREEMENT, made and entered into thisc24' day of March , 2010, by and between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Division of Social Services' Area Agency on Aging, hereinafter referred to as
"AAA" and the City of Greeley, Colorado on behalf of the Greeley Senior Activity Center hereinafter referred
to as "Greeley".
WHEREAS, AAA develops and administers the Area Plan for a comprehensive and coordinated system
of services, and
WHEREAS, Greeley maintains a senior center providing recreational, nutritional, and educational
opportunities for senior citizens; and
WHEREAS, Greeley proposes to implement a"Let's Get Movin' and Grovin' in 2010"program through
the Greeley Senior Activity Center to reach out to area seniors.
NOW THEREFORE, it is mutually agreed:
1. The AAA hereby agrees to award grant monies to Greeley in the sum of$500.00 (Older Coloradoan
Fund). Greeley understands that the grant of$500.00 may be used only to pay for the cost of the "Let's Get
Movin' and Grovin' in 2010" outreach program.
2. Greeley agrees to implement a"Lets Get Movin' and Grovin' in 2010" outreach program. As part of
this program, Greeley agrees to send flyers to low income isolated seniors inviting them to apply for an
Exercise Scholarship that would cover the cost or a portion of the cost for the Senior Shape-up and other
exercises classes. This class promotes physical fitness and positive social interaction. Greeley agrees that
transportation will be provided by the volunteers for seniors who can not provide or obtain access to
transportation services.
3. Greeley agrees to document the results of this outreach effort monthly. Greeley agrees to provide
such documentation and all other required paperwork to the AAA by the 10th of each month.
4. Term and Termination. This Agreement shall be effective from the date of execution of this
agreement and shall continue through December 31, 2010
5. Entire Agreement. This Agreement contains the entire understanding between the parties hereto
relating to the subject matter contained herein.

THE CITY OF GREELEY, COLORADO
RESOLUTION 15 , 2010
A RESOLUTION AUTHORIZING THE MAYOR TO ENTER INTO AN AGREEMENT
BETWEEN THE CITY OF GREELEY AND WELD COUNTY TO AUTHORIZE A GRANT
FROM THE COUNTY'S AREA AGENCY ON AGING TO THE CITY OF GREELEY.
WHEREAS,the City of Greeley("City")and Weld County desire to enter into an Agreement
to obtain a grant from the Area Agency on Aging to support a program at the Greeley Senior Activity
Center entitled "Let's get Movin' and Groovin' in 2010; and
WHEREAS,the City and County are each willing to cooperate in this matter under the terms
and conditions contained in Exhibit "A"; and
WHEREAS, it is in the best interest of the citizens of the City of Greeley to enter into this
agreement with the County.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
GREELEY, COLORADO:
1. The City Council hereby authorizes the Mayor to enter into the agreement attached hereto
and incorporated herein as Exhibit "A".
2. This Resolution shall become effective immediately upon its passage.
PASSED AND ADOPTED, SIGNED AND APPROVED THIS 16th day of
February , 2010.
