RE: APPROVE SEVEN PURCHASE OF SERVICES AGREEMENTS FOR OUTREACH
ACTIVITIES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with eight Purchase of Services Agreements for
Outreach Activities between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Agency, and the various Senior Centers, as follows, commencing December 1, 2009,
and ending June 30, 2010, with further terms and conditions being as stated in said agreements,
and
Fort Lupton Community Center Hill N Park Senior Center
Johnstown Senior Center Mead Senior Center
Pierce Senior Center Platteville Community Center
Tri-Town Senior Center
WHEREAS,after review,the Board deems it advisable to approve said agreements, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the eight Purchase of Services Agreements for Outreach Activities between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services,Area Agency on Agency, and the various
Senior Centers as listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreements.

RE: Purchase of Services Agreements between the Weld County
Department of Human Services and Seven (7) Service
Providers for the Loneliness Grants
Enclosed for Board approval are Purchase of Services Agreements between the Weld County
Department of Human Services and seven (7) service providers. These Agreements were
presented at the Board's January 6, 2010, Work Session.
The Service Providers are:
1. Fort Lupton Community Center
2. Hill N Park Senior Citizens
3. Johnstown Senior Center
4. Mead Senior Center
5. Pierce Senior Center
6. Platteville Community Center
7. Tri-Town Senior Center
The major provisions of these Agreements are as follows:
1. These Agreements are the Loneliness Agreements in the amount of$500.00 each.
2. These grants help the centers provide a variety of programs that are designed to reach out to
low-income isolated and lonely seniors in their communities through special events,programs,
and scholarships.
3. The term of these Agreements is a one time only award and the terms are valid through
June 30, 2010, and are funded by the Older Coloradoan Fund.

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 1st day of December, 2009 by and between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County,on behalf of the Weld County Division of
Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Fort Lupton Comntunity Center
hereinafter referred to as "Fort Lupton." ?r r„

WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services,and
WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Fort Lupton" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Fort Lupton" proposes to implement the"Social Enhancement"program in order to connect with area
seniors;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to"Fort Lupton" in the sum of$500.00(Older Coloradoan Fund).
2. "Fort Lupton"agrees to implement the"Social Enhancement"program. The program will focus on connecting with
community seniors through special events, programs and scholarships for low income and isolated seniors. Transportation
will be provided to seniors who can not access events on their own.
3. "Ft Lupton"agrees to document the results of the outreach efforts monthly and complete all required paperwork by
the 10th of each month. "Ft. Lupton"understands that the grant of$500.00 may be used only to pay for the cost of
the outreach program.
4. "Fort Lupton" understands this is a one time only award and the terms of the agreement are valid through June 30,
2010.

THIS AGREEMENT, made and entered into this 1st day of December, 2009 by and between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Services'
Area Agency on Aging,hereinafter referred to as"AAA"and the Hill N Park Senior Citizens hereinafter referred to as"Hill N Park"
WITNESSETH:
WHEREAS,"AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services, and
WHEREAS,"AAA"considers senior centers a vital link to seniors who are lonely and isolated,and
WHEREAS,"Hill N Park"is a program providing recreational,nutritional,and educational opportunities for senior citizens, and
WHEREAS, "Hill N Park"proposes to implement and maintain a loneliness outreach program to reach out to seniors in the Hill N
Park area to involve them in the social activities at the Hill N Park Senior Center;
NOW THEREFORE,it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to"Hill N Park" in the sum of$500.00(Older Coloradoan Fund).
2. "Hill N Park"agrees to implement and maintain an outreach project that would involve contacting seniors in the Hill N Park
community and encouraging involvement at the center. Phone outreach,information flyers,guest meal passes will be a few of
the techniques used to connect with isolated community seniors.
3. "Hill N Park"agrees to document the results of this outreach effort monthly and complete all required paperwork by the 10Th of
the month."Hill N Park"understands that the grant of$500.00 may be used to pay for the cost of the outreach program and to
advance the overall"Hill N Park"senior citizens program.
4. "Hill N Park"understands this is a one time only award and the terms of this agreement are valid through June 30,2010.

2. "Johnstown"agrees to implement and maintain the"Senior Outreach Project"that would involve the
maintenance of a volunteer committee that will continue to reach out to isolated or homebound seniors via
monthly mailing of flyers/calendars/newsletters and phone and one on one outreach. Special programs such as
ice cream socials and other social outreach events will be implemented and/or maintained. Local media
options will be used to advertise events and provide service information.
3. "Johnstown"agrees to document the results of this outreach effort monthly and complete all required
paperwork by the 10'"of the month. "Johnstown"understands that the grant of$500.00 may be used to pay
for the cost of the outreach program and to advance the overall"Johnstown"senior program.
4. "Johnstown"understands this is a one time only award and the terms of this agreement are valid through June
30,2010.

WITNESSETH:
WHEREAS,"AAA"develops and administers the Area Plan for a comprehensive and coordinated system of
services,and
WHEREAS,"AAA"considers senior centers a vital link to seniors who are lonely and isolated,and
WHEREAS, "Mead" is a program providing recreational,nutritional,and educational opportunities for senior
citizens, and
WHEREAS,"Mead"proposes to implement and maintain the"You are not Forgotten"program to reach out to
seniors in the Mead and Carr areas to involve them in the social activities at the Mead Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to the"Mead" in the sum of$500.00(Older Coloradoan
Fund).
2. "Mead"agrees to maintain the"You are not Forgotten"program by recruiting volunteers who will contact
homebound seniors and new seniors to the area,to increase phone calls and mailings,develop special events
and to help seniors with transportation,when possible.
3. "Mead"agrees to document the results of this outreach effort monthly'and complete all required paperwork
by the 10' of each month. "Mead"understands that the grant of$500.00 may be used only to pay for the cost
of the outreach program
4. "Mead"understands this is a one time only incentive award and the terms of this agreement are valid through
June 30,2010.

WHEREAS, "Pierce" is a program providing recreational, nutritional, and educational opportunities for senior citizens,
and
WHEREAS, "Pierce"proposes to implement the"Old Time Jam" Project to reach out to isolated seniors in their
community to involve them in the social activities at the Pierce Senior Center;
NOW THEREFORE, it is mutually agreed:
I. The"AAA"hereby agrees to award grant monies to"Pierce" in the sum of$500.00 (Older Coloradoan Funds).
2. "Pierce"agrees to develop and maintain the"Old Time Jam"project. This project will use a both community
volunteers and senior center volunteers to encourage broader senior participation at the senior center. Outreach
efforts will include advertizing events, phone contacts,flowers,cards and special projects.
3. "Pierce"agrees to document the results of this outreach effort monthly and complete all required paperwork by the
I0°i of each month. "Pierce" understands that the grant of$500.00 may be used only to pay for the cost of the
outreach program.
4. "Pierce" understands this is a one time only award and the terms of this agreement are valid through June 30, 2010.

WHEREAS, "Platteville" is a community center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Platteville " proposes to maintain the "Friends Project"to reach out to area isolated seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to"Platteville" in the sum of$500.00 (Older Coloradoan Fund).
2. "Platteville" agrees to maintain the"Friends Project"that will include volunteer outreach efforts that will
include identifying seniors in the community who are at risk of loneliness and isolation and encourage
participation in senior activities.
3. "Platteville"agrees to document the results of this outreach effort monthly and complete all required paperwork by the
10t of each month. "Platteville" understands that the grant of$500.00 may be used only to pay for the cost of the
outreach program.
4. "Platteville" understands this is a one time only award and the terms of the agreement are valid through
June 30, 2010.

WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Tri-Town" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Tri-Town"proposes to implement a"The Connection"project to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to"Tri-Town" in the sum of$500.00(Older Coloradoan Fund).
2. "Tri-Town"agrees to implement and maintain the"The Connection"project. The project will use a volunteer
committee that will reach out to homebound and isolated seniors and encourage senior center involvement and/or
provide home visits to homebound seniors and volunteer transportation services. Volunteers will also send cards,
letter, brochures and flyers to community seniors.
3. "Tri-Town"agrees to document the results efforts monthly and complete all required paperwork by the 10th of the
month. "Tri-Town"understands that the $500.00 may be used to pay for the cost of the outreach program and to
advance the Tri-Town Senior Program.
4. "Tri-Town"understands this is a one time only award and the terms of this agreement are valid through June 30,
2010.
