TO HEALTHY FOOD AND PHYSICAL ACTIVITY OPPORTUNITIES IN WELD COUNTY
AND AUTHORIZE CHAIR TO SIGN - COLORADO HEALTH FOUNDATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Grant Application for Various Projects to
Increase Access to Healthy Food and Physical Activity Opportunities in Weld County from the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Public Health and Environment, to the Colorado Health
Foundation, commencing January 1, 2011, and ending December 31, 2012, with further terms and
conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Grant Application for Various Projects to Increase Access to Healthy
Food and Physical Activity Opportunities in Weld County from the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Public Health and Environment,to the Colorado Health Foundation be,and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said application.

Re: Colorado Health Foundation Project Funding Request
Enclosed for Board review and approval is the application for funding to the Colorado Health Foundation by the
Weld County Department of Public Health and Environment to improve access to healthy foods and physical
activity in Weld County. The application is for a two year period,January 1, 2011 through December 31, 2012.
The total amount of the request is$394,228.00.
The project includes strategies to work with East Greeley and the town of Milliken to improve access to healthy
food, exploring the possibility of year around farmers' markets in these neighborhoods and expanding the
community gardens program. Other strategies are focused on improving the infrastructure for biking and
walking trails and working with schools to improve access to healthier foods through the Farm to School
program.
The grant will fund existing 1.25 FTE, including the .5 project coordinator, .5 bilingual health educator, and.25
OT Ill, staff currently funded by LiveWell Weld County. Corporate support includes in kind salaries for the
Division Director and Health Data Analyst,office space,meeting space,and marketing materials.
This private funding will allow us to build on the positive healthy eating, active living changes that have
occurred as a result of the Steps to a Healthier Weld and LiveWell programs in Weld County.
We recommend your approval.

Colorado Health Foundation Grant Application
Proposed Food Access&Physical Activity Objectives&Strategies
Objective 1: By December 31, 2012, improve healthy food access for at least 10,000 underserved
Weld County residents.
Strategy 1:Continue the food retailer mini-grant program started in East Greeley to incentivize one food
retailer located in the Town of Milliken to increase their capacity to carry a wider variety of healthy food
options.
The East Greeley Food Access Task Force,started in April 2010, partnered with two food retailers (18th
Street Discount Grocery&Carniceria Lozano) in East Greely(Pop. 7000+) to expand their inventory of
healthy foods with the purchase of new equipment, better signage, education and marketing.
This model will be used to improve food access in the Town of Milliken (Pop. 6,200+). Milliken was identified
as a food desert during the 2010 Northern Colorado Regional Food System Assessment project because it
does not have a grocery store.We will work with Carniceria Y Restaurante Munoz,the only established food
retailer in downtown Milliken.
We have identified key individuals to serve on a Milliken Food Access Task Force.We will survey residents
about consumer preferences, determine barriers to expanding fresh food options and implement one or
more evidence based strategies to improve access to healthy foods in Milliken.(1)
Strategy 2:Expand an existing county-wide community and school garden mini-grant program to implement
and/or enhance up to 50 gardens that will serve up to 6,000 underserved residents.
LiveWell micro-finances 18 gardens in 7 municipalities (Ault, Eaton, Frederick, Greeley, Kersey, Nunn,
Windsor) and has a reach of nearly 1,500 residents. In 2011 &2012,we will expand by working with 4
school districts, Boy's and Girl's Clubs, Clay Center, Catholic Charities,senior centers, faith groups, and
resident groups.At least 6,000 lower income residents will benefit from this strategy(2).
Strategy 3:Support the development of two year-round farmers'markets in food desert areas to provide more
consistent access to fresh healthy foods while supporting local agriculture.
The Milliken Year-Round Farmers'Market and East Greeley Grow House Project will be implemented during
2011 and 2012 using the GrowHause as a model.We will work with the municipalities, task force members
and a consultant to develop a business plan, identify funding sources and provide some financial incentives
to bring these markets into operation.
A consumer survey conducted in East Greeley identified community gardens and farmers' markets as a way
to improve access to healthy foods in that neighborhood.Through the Regional Food System Assessment,we
have identified new sources of locally grown healthy foods to supply these new markets.
Members of the East Greeley Task Force, including a local grower and food retailer,are committed to
establishing a full service market or"GrowHaus"in that neighborhood. The Milliken town administrator
strongly supports this strategy and suggested the historic grain elevator located on Main Street be
considered for this use.
Objective 2: By December 31, 2012,improve access to physical activity opportunities for at least 75%
of Weld County residents.
Strategy 1:Continue to enhance infrastructure supporting bicycling and walking and other recreation
opportunities with a focus on influencing public support by:

a. We will evaluate and build Weld County citizen and municipality support for sustainable funding
mechanisms for the creation, improvement, and/or maintenance of recreational opportunities including
trails.
The Weld Trails Coordination Committee (WTCC) will research and evaluate county-wide citizen and key
leader support for increasing the connectivity of the county's 215 built and 590 planned recreational
trails. Steps will include up to five citizen/public meetings, individual interviews, collection of existing
administrative and qualitative data, issue and solution analysis, and partner,message,and materials
development.This will result in one or more proposals for key leaders and citizens to develop local
sustainable funding mechanisms for trails and other recreational opportunities (1).
b. We will create a regional walking and biking guide to increase the use of trail networks for physical
activity in Northern Colorado.
The WTCC will partner with the Northern Colorado Cultural Tourism Alliance (NCCTA) to create a
comprehensive guide to promote regional trail network use for physical activity,recreation,and
economic vitality.WTCC completed a county-wide trail inventory map in 2010. Building on this GIS data,
we will add tourism destinations identified by NCCTA to create the guide.The long term impact will be
expanded access to physical activity opportunities,increased regional economic growth, and support for
future trail creation and maintenance (1).
Strategy 2: Work with the Highway 85 Coalition and Weld County Department of Public Works to develop
active living language for the Highway 85 Corridor Regional Comprehensive Plan.
The Coalition, formed in 2009, consists of nine municipal partners with an estimated county population of
over 131,000,Weld County Government, seven economic development groups, and fourteen other agencies.
The purpose of the Coalition is to develop a regionally coordinated sustainable plan to create regional
standards of service in areas of: workforce development, economic revenue sharing, and strategies to meet
this region's air,water, energy,public outreach and education, and quality of life needs.
We will assist with the public outreach and education and quality of life portion of the Regional
Comprehensive Plan. We will assess the health-related quality of life in Gilcrest, LaSalle, and Platteville (Pop.
5,800) by gathering existing data (GIS maps and population-based health survey data), conducting scoping
activities (tours and key informant interviews), providing recommendations and technical assistance to help
prioritize, implement, and sustain programming and policy that improves their communities' health-related
quality of life.
Objective 3: By December 31, 2012,improve access to healthy foods and physical activity
opportunities for at least 6,000 Weld County students enrolled in three Weld County school districts.
Strategy 1:Conduct a school wellness assessment in 3 high need school districts and assist them with at least
one recommended improvement around nutrition or physical activity.
We will develop an assessment tool for rural school districts using best-practice resources.We will work
with Johnstown/Milliken (Pop. 3,136), Kersey(Pop. 1,122),and Gilcrest/ LaSalle/Platteville (Pop. 1,784)
where we have established relationships with their nutrition departments and school leadership.
Results will drive research-based programming solutions (1). Programming may include building capacity
for physical activity programs or healthy home cooked food by assisting them with equipment purchases for
their cafeterias or playgrounds, offering or arranging trainings for food service staff or supporting their
involvement in the USDAs Farm to School program.

Section 2 - Organizational information
The mission of the Weld County Department of Public Health and Environment (WCDPHE) is to
preserve,promote and protect the health and environment of the residents of Weld County in
partnership with the communities we serve.Together, we are working to make Weld County a
healthy place to live, learn,work and play.
WCDPHE was established in 1938,the second organized health department in Colorado. The health
department has 5 divisions to serve the public: Administration, Public Health Nursing Division,
Environmental Health, Emergency Preparedness and Health Communication, Education and
Planning. The department offers a full range of public health services in Weld County. For the
purposes of this grant application,we will focus on the Health Communication, Education and
Planning Division (HCEP).
The HCEP Division works to improve the health of Weld County residents through community-
based education programs for a diverse population of 250,000, public support and capacity
building for the built environment and food access, health data collection, strategic planning and
social media campaigns.
We have a strong history of successful programming, most recently with the Steps to a Healthier
Weld/US five year grant and LiveWell Colorado funding, promoting active living and healthy eating.
According to the BRFSS data, intake of fruits and vegetables has increased and physical activity is
significantly higher among adults in Weld County since programming began in 2003. We also have
long standing programs for cancer screening, tobacco education, injury prevention and healthy
relationships for teens and parents.
The project team worked together on the Steps to a Healthier Weld and LiveWell Weld County
grants. Bobbie Puckett, RD, is the LiveWell Weld County coordinator; Pam Smith has experience
with environmental land use planning and coordinated the active community environment(ACE)
projects; Ofelia Orozco, a bilingual/bicultural health educator and Promotora,works with our
Latino communities; and Marjorie Hanson, a graphic artist and administrative technician,
produces the educational materials for the department,.The Health Data analyst provides
evaluation support for department grants and manages a variety of data projects. The Division
Director is an RD with 11 years experience managing the HCEP team and grants.
The members of the North Colorado Health Alliance are key partners representing the community
health center,human services,the local hospital, United Way, behavioral health,higher education,
local businesses and municipalities, such as Milliken and Greeley. In addition,we partner with
many other entities such as the Boys and Girls Clubs, Catholic Charities, CDI Head Start, individual
schools in Weld County,worksite wellness programs in local businesses, and individual community
members.
WCDPHE is a department of Weld County government. The Board of Weld County Commissioners
is made up of five elected officials that serve as the policy makers for Weld County government.The
Board of County Commissioners and department heads recently completed the 2010-2014
Strategic Plan for Weld County that is available on line at www.co.weld.co.us .
The goal of the health department's strategic plan is to protect and improve the health of residents
and visitors to Weld County by focusing on health outcomes that are based on health data and
community input.The plan outlines strategies that address four goals and thirteen objectives
endorsed by the County Commissioners and Board of Health. It provides general guidance for the
health department without impinging on its need to remain flexible to changing and emerging
health priorities.
Section 3 -Financial Information
The WCDPHE receives approximately one third of its funds from the county, one third from state
funds and one third from fees for services.The Health Department budget for 2010 is over$8
million dollars.The annual report for 2009 is available at www.co.weld.co.us .
Support for this project comes from county funding, a Preventive Health and Health Services grant
and our community partners. Weld County will fund 25%of the ACE TA contractor, .15 FTE HCEP
division director, .075 FTE health data analyst, mileage, meeting expenses and incentives for
participation as well as provide office space and equipment for the staff The project
coordinator/RD is partially funded (.5FTE) by the PHHS grant for 2011. She will manage this both
projects as she has done as the LiveWell Weld County coordinator.
Our partners in improving access to healthy food and physical activity opportunities will contribute
staff time and political will to make the project a success.They include the staff of the town of
Milliken and it's community members and businesses; the planning, parks and recreation staff and
elected officials in the Weld Trails Coordination Committee (WTTC) and members of the Cultural
Tourism group; School District Six (Greeley/Evans), RE5J (Milliken/Johnstown), RE1
(Platteville/Gilcrest) and the other districts that are revitalizing their school food programs to
include scratch items and fresh local foods; the City of Greeley and the East Greeley Task Force
members.These partners also contribute meeting space,meeting supplies, contact and mailing
lists, GIS services,mentoring for gardens and school food purchasing, retail expertise, and good will
for the community.
The long term funding strategy for our project is to sustain it through our partners in the
communities and schools. We plan to work with our partners on a long term basis to make healthy
living a part of everyone's strategic plan. As we make improvements in the food supply system, it
will provide a lasting benefit for the citizens. Fifteen healthy eating and active living programs and
projects initiated through Steps and LiveWell are now sustained in our county. East Greeley
residents and food retailers have made significant progress in 2010 to improve access to healthy
foods through the East Greeley Task force and the LiveWell team. The Milliken town administration
is committed to improving the health of their community through ACE initiatives and access to
healthy foods. The WTTC is committed to moving forward to connect the trails and tourism
opportunities in northern Colorado.
The Board of Weld County Commissioners are elected officials who determine the amount of county
general funds the health department receives each year to support our work in Weld County. The
Board fully supports the work of the HCEP team and this application for funding.
