RE: APPROVE APPOINTMENT OF PRIVACY OFFICERS UNDER HEALTH INSURANCE
PORTABILITY AND ACCOUNTABILITY ACT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, by Resolution#2003-0902,the Board of County Commissioners approved the
Health Insurance Portability and Accountability Act (HIPAA) Compliance Plan for Weld County for
Dental,Vision, and Flexible Spending Plans, and the HIPAA Compliance Plan for the Departments
of Public Health and Environment, Paramedic Services, and Human Services as health care
providers, and
WHEREAS, said HIPAA Compliance Plans require HIPAA Privacy Officers be appointed,
and
WHEREAS,the Board deems it advisable to appoint Anita Scrams as the Privacy Officer for
the ACS HIPAA Plan;to appoint Mitch Wagy to replace David Bressler as the Privacy Officer for the
Weld County Paramedic Services' HIPAA Plan; and to appoint Sandy Hasch to replace Eva Jewell
as the Privacy Officer for the Department of Human Services, Area Agency on Aging Division's
HIPAA Plan.
NOW, THEREFOR BE IT RESOLVED by the Board of Weld County Commissioners that
Anita Scrams be, and hereby is, appointed the HIPAA Privacy Officer for the ACS HIPAA Plan.
BE IT FURTHER RESOLVED by the Board that Mitch Wagy be, and hereby is, appointed
the HIPAA Privacy Officer for the Weld County Paramedic Services' HIPAA Plan, to replace Dave
Bressler.
BE IT FURTHER RESOLVED by the Board that Sandy Hasch be, and hereby is, appointed
the HIPAA Privacy Officer for the Department of Human Services,Area Agency on Aging Division's
HIPAA Plan, to replace Eva Jewel.

The above and foregoing Resolution was, on motion duly made and seconded, adopted by
