This letter shall serve as notice to the State of Colorado, North Range Behavior
Health Center, the City of Greeley and Weld County pursuant to C.R.S. §24-10-109:

2. Statement of Facts:
On January 22, 2010 at approximately 11:30 p.m., I left the North Range
Behavior Health Center Detox Center. The sidewalks had not been cleaned and/or
treated for ice accumulation. The icy sidewalk was an unreasonably unsafe condition
about which the landowner knew or should have known. As a result of the dangerous
condition, I slipped and fell. The fall caused me injuries and damages.

4. As a direct and proximate result of the negligent acts and omissions of the
employees, representatives and agents of the State of Colorado, North Range Behavior
Health Center, the City of Greeley and/or Weld County, I suffered and will continue to
suffer injuries and damages, including, but not limited to, permanent physical impairment
and injury, pain, emotional distress, mental anguish, physical suffering, medical expenses
and potential loss of home services. I experience numbness and tingling in my right hand,
pain in my neck and back, as well as muscle spasms and decreased range of motion in the
cervical, thoracic and lumbar areas.
5. Statement of the amount of monetary damages:
I am seeking recovery of monetary damages to compensate me for my out of
pocket losses, impairment and non-economic losses. It is difficult to assess the monetary
amount of these losses as I am still receiving medical treatment. It's my hope I will fully
recover from my injuries. I am working hard in therapy to maximize my chances for full
recovery. In an abundance of caution, I will say the amount of monetary damages is up
to $100,000.

I discovered my injuries on January 22, 2010.
