Dear Brad:
Enclosed please find a Mylar site plan for the noted USR for recording along with our check no.
23957 for recording fees. Please note the status of the following requests from your e-mail of
October 23 2006.
1) Windsor/Severance Fire Protection District: Petro-Canada(PCR)will adhere to
the Windsor/Severance Fire Protection District's comments dated May 11 2006.
PCR field supervisor, Dave Gipson will acquire a special hazard permit from the
Windsor/Severance Fire Protection District.
2) All 21 wells noted on the plat are located more than 60' from the Poudre Public
trail,therefore chain link fencing around the facilities will not be installed.
3) CDOT: Timothy Bilobran of CDOT advised that an access permit for State
Highway 257 will not be required as PCR is utilizing existing access roads.
CDOT to provide PCR with written notice to proceed upon.
4) Prior to drilling,the drilling contractor will obtain the necessary permits for
overweight and over width vehicles.
5) Attached is evidence of APEN permits for the(3) existing tank batteries
associated with this USR. APEN permits for the(2) proposed tank batteries (1
each in section 34 &35) will be provided 45-days after first production.
Thank you again for your help on this project and please provide a copy of the recorded site plan
for our records.

Susan,
Thanks again for bringing in the revised USR plan. With a couple changes to the mylars, and completion of some
outstanding conditions, we'll be able to recorded the plans.
The plan looks good as is—just a couple changes needed. First, please label the roads on the plan, specifically
CR 23 and Hwy 257. Also, please add a graphic scale to complement the written one (i.e., 1 inch equals 500 ,j
feet).
One of the outstanding conditions (Item 1.F.5) is that chain-link fencing be called out for the facilities that are
within 60 feet of the public trail. This information should be added to the plan set.
With these changes to the plan set, please run a mylar copy, get it signed and returned to me, along with the $11
recording fee.
Before we can record, however, you'll also need to address the outstanding conditions. Specifically, from the v
Resolution, we need info from CDOT(1.C), evidence of an APEN application or the alternative that one isn't
needed (1.D), and written evidence of having address Windsor-Severence Fire's comments (1.E).
