RE: APPROVE FOUR (4) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with four (4) Amendments to Child Protection
Agreements for Services with Various Providers between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, and the following providers, commencing June 1,2010, and ending May 31,2011,
with further terms and conditions being as stated in said amendments, and
1. Griffith Centers for Children-Chins Up Center (Home-Based Services)
2. Savio House (Home-Based Services)
3. Savio House (Sex Abuse Treatment)
4. Turning Point Center for Youth and Family Development, Inc. (Day Treatment)
WHEREAS,after review, the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the four(4) Amendments to Child Protection Agreements for Services with
Various Providers between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, and the
above listed providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

COLORADORE: Amendment to Child Protection Agreementervices
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval is an Amendment to Child Protection Agreements for Services
between the Department and Various Providers.
The following two amended agreements were presented at the Board's August 30, 2010,
Work Session and the major provisions of these Agreements are:

The following two amended agreements were presented at the Board's September 27,
2010, Work Session. These Amendments are necessary to stay within the Core Allocation.
The major provisions of these Agreements are:

GRIFFITH CENTERS FOR CHILDREN-CHINS UP CENTER
This Amendment,made and entered into the day of August 2010,by and between the Board
of County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter
referred to as"Department,"and Griffith Centers for Children-Chins Up Center,hereinafter referred to as
the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Home Based Services(PY-
10-11-CORE-0183)in order to increase the maximum reimbursement under this contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available:and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule,Item 1,is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$20,000.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.

WHEREAS,the parties desire to modify their current Agreement for Community Based Services
(PY-10-I 1-CORE-0150)in order to decrease the maximum reimbursement under this contract;and
WHEREAS, Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval, clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as
follow:

Inc.,hereinafter referred to as the"Contractor."

WHEREAS,the parties desire to modify their current Agreement for Day Treatment(PY-10-11-
CORE-0175)in order to decrease the maximum reimbursement under this contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule,Item 1,is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding, not to exceed$105,700.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
