Monitoring and Permit Deviation Report — Part I
1. Following is the required format for the Monitoring and Permit Deviation report to be
submitted to the Division on a semi-annual basis unless otherwise noted in the permit. The
Table below must be completed for all equipment or processes for which specific Operating
Permit terms exist.
2. Part II of the Appendix B shows the format and information the Division will require for
describing periods of monitoring and permit deviations, or upset or emergency conditions as
indicated in the Table below. One Part II Form must be completed for each Deviation.
Previously submitted reports (e.g. EERs or Upsets) may be referenced and the form need not be
filled out in its entirety.

credible evidence that indicates non-compliance, then the Responsible Official can certify that the emission point(s) in
question were in continuous compliance during the applicable time period.

Dear Mr. Gathman,
As per your request (included), I would like to request Weld County to, release all
collateral being retained for Waste Management projects. In answer to your questions, as
you mentioned we will put rock in the dirt area between the H2O vault and the AC
parking lot. Also, with regards to the North berm area. The drawings show that a berm is
to be constructed where we place it along the North driveway. From there to the East a
swale was to be constructed to maintain the drainage. We move the fence North to the
WAPA easement line in order to maintain that drainage, while allowing the farmer
adequate room to farm North of the fence. If there is any other clarification or
information you need, let me know. You had asked for a drawing reflecting the change.

As waste Managements representative, Landmark Construction Services is requesting a Certificate of
Occupancy or a Temporary Certificate of Occupancy, for the maintenance facility on CR 25 outside of Ault. We

severance fire District on April 5, 2007, as well as the final electrical and building inspections. Tracy Dyer with
the county public works is appraised and I believe has prepared his final report also.Please let me know what if

MEMORANDUM OF UNDERSTANDING
This Agreement shall be entered into and agreed upon by Waste Management Disposal Services of
Colorado, Inc. (WMDSC), 8310 S Valley Hwy, Suite 200 Englewood, CO 80112, and Windsor-Severance
Fire Protection District, (WSFPD) 100 7th Street, Windsor, CO 80550.
WMDSC requests that WSFPD approve a temporary fire flow variance for its proposed Waste Management
Operations Facility located at 40950 WCR 25 Ault, CO 80610. WMDSC desires to begin construction of the
facility but cannot commit to a schedule without assurance that WSFPD will approve a Temporary
Certificate of Occupancy at the end of construction, assuming that all other applicable code requirements are
met. The Temporary Certificate of Occupancy will be valid through the term of the Temporary Fire Flow
Variance and this MOU. With the variance, WMDSC and WSFPD acknowledge that the facility will be
constructed knowing that only 1,100 (of the required 1,500) gallon per minute fire flow is available at the
site.
Currently, North Weld County Water District's (NWCWD) distribution system cannot satisfy the minimum
1,500 gallon per minute fire flow, required by WSFPD at the site. NWCWD plans to install improvements to
their water distribution system, which will exceed the minimum fire flow requirements at the facility
location. The improvements are scheduled to be completed by December 31, 2008.
According to a recent fire flow test, the NWCWD distribution system can supply the site with fire flows as
follows:
• 645 gallons per minute at 20 psi, when supplied solely from the zone 6 reservoir;
• 1,039 gallons per minute at 20 psi, when supplied from the zone 6 reservoir and one pump at the
zone 6 pump station, and
• 1,100 gallons per minute at 20 psi, when served from the zone 6 water storage tank and two parallel
pumps at the zone 6 pump station.
WSFPD understands that the required water supply for full protection of this facility is not currently
available. Based on the available supply of water and the required built-in fire protection systems, WSFPD
agrees to approve a temporary fire flow variance for the proposed facility with the following stipulations.
1. The temporary fire flow variance for the facility will be in effect until NWCWD has completed
construction of water distribution system improvements which will provide the required fire flow
(1,500 gpm at 20psi) at the site or until December 31, 2008, whichever occurs earlier.
2. Upon completion of facility construction, WSFPD will approve a temporary certificate of occupancy
for the WMDSC Operations Facility valid through the term of the temporary fire flow variance and
this MOU.
3. In the event that NWCWD has not completed scheduled water distribution system improvements
which would provide the required fire flow (1,500 gpm at 20 psi) by December 31, 2008, WMDSC
agrees to evaluate the facility fire protection systems with WSFPD and shall install additional site
improvements, to achieve the required fire flow (1500 gpm at 20 psi) at the site or obtain an
extension to the temporary fire flow variance and this MOU.

4. The parties have reviewed this (and other alternatives to providing the required flows) and that the
alternatives to the tire code requirements contained herein are reasonable and should be effective in
reducing the probability of loss of property and/or life. However, WSFPD makes no warranties or
representations, express or implied, that the alternatives set forth herein are fully equivalent to the
reduced risk of fire; which is provided by adequate fire flows and hydrant systems.
5. Progress of this agreement shall be reviewed at the 12-month and 18-month period starting from the
Weld County building permit issuance date. It shall be the responsibility of WMDSC, to contact the
appropriate agencies at these intervals.
IN WITNESS WHEREOF, the parties hereto have duly executed this Agreement the day and year first
above written.

Thank you for providing information on the wastewater system that will service Waste
Management of Northern Colorado. After reviewing your information, we have determined that
you are operating a Class V disposal system as defined by 40 Code of Federal Regulations (40
CFR) Section 144.6. You are therefore regulated under the Underground Injection Control (UIC)
Program found in 40 CFR Parts 144, 146, and 147, which were promulgated under Part C of the
Safe Drinking Water Act, 42 United States Code Sections 1421 through 1428. A copy of 40
CFR Part 144 Subpart G,which applies specifically to Class V injection wells, is enclosed.
Your Class V disposal system is currently "authorized by rule" under 40 CFR Sections
144.24 and 144.84(a). "Authorized by rule"allows you to operate your Class V disposal system.
Your Class V disposal system must comply with, among other requirements, 40 CFR Section
144.12(a), which prohibits any underground injection that may endanger an underground source
of drinking water(Section 144.12 is summarized in Section 144.82 of the Subpart G enclosure).
Your Class V disposal system is also subject to periodic compliance inspections, which may
include sampling and analysis of your fluids. Finally, be aware that under 40 CFR Sections
144.12(c), (d), and (e), we can require you to apply for a permit or close your disposal system
under certain circumstances (e.g., if all or part of the fluids you discharge changes from solely
sanitary to industrial wastes).

Failure to comply with the above requirements will result in violations of UIC regulations
and possible enforcement action. Also enclosed is a listing of environmental/small business
resources and a brochure addressing what septic systems fall under Class V regulations.

In accordance with the requirements of USR #1527, Waste Management Disposal Services of
Colorado, Inc. (WMNC) has substantially completed the conditions of approval set forth by the
Board of County Commissioners of Weld County during the December 21, 2005 hearing.
WMNC offers this submission for approval of the Plat and finalization of the Use by Special
Review process. As outlined in the conditions of approval, each condition has been addressed as
follows:
1. Prior to recording of the plat:
A. All sheets of the plat shall be labeled USR-1527
The plat consists of a single sheet which is labeled USR-1527. Two full sized plats are
attached for final approval.
B. The plat shall be amended to delineate the following:
1) The attached Development Standards
The Development Standards are reproduced on the plat. Two full sized
plats are attached for final approval.
2) Weld County Roads 25 and 84 are designated on the Weld County Road
Classification Plan as local gravel roads,which require 60 feet of right-of-
way at full build out. There is presently 60 feet of right-of-way. A total of
30 feet from the centerline of Weld County Roads 25 and 84 shall be
delineated as right-of-way on the plat. These roads are maintained by
Weld County.
A 60-foot wide right-of-way has been identified for Weld County Roads 25
and 84 on the plat. Two full sized plats are attached for final approval.
C. The applicant shall enter into a Long-Term Road Maintenance and
Improvements Agreement with the Weld County Department of Public Works

for the upgrade, paving, and future maintenance of Weld County Road 25.
Evidence of approval by the Department of Public Works shall be submitted to
the Department of Planning Services.
WMNC is in the process of completing a Long-Term Road Maintenance and
Improvements Agreement with the Weld County Department of Public Works.
WMNC requests that the Department of Public Works allow the County to move
forward with the approval of the plat in order to begin the permit review and approval
process. Before completion of the USR process,this agreement shall be in place.
D. The applicant shall complete all proposed improvements regarding access,
circulation, parking lot improvements, and landscaping to the satisfaction of the
Departments of Planning Services and Public Works, or enter into an
Improvements Agreement Regarding Collateral for Improvements-and-post
adequate collateral for all required on-site improvements. The agreement and
form of collateral shall be reviewed by County staff and accepted by the Board of
County Commissioners prior to recording the Use by Special Review plat.
WMNC is in the process of completing the Improvements Agreement Regarding
Collateral for Improvements with the Weld County Departments of Planning
Services and Public Works. WMNC requests that the Department of Public Works
allow the County to move forward with the approval of the plat in order to begin the
permit review and approval process. Before completion of the USR process, this
agreement shall be in place.
E. The applicant shall submit a Dust Abatement Plan to the Environmental Health
Services Division of the Weld County Department of Public Health and
Environment, for review and approval. Written evidence of such shall be
provided to the Department of Planning Services.
Due to the nature of the improvements, dust abatement activities will only be
needed during the construction phase of the project. All drive lanes, operational
areas and parking areas at the facility will be paved with concrete or asphalt. The
container storage area will be surfaced with road base or recycled asphalt. Non
paved areas including landscape berms and the stormwater detention pond will be
vegetated. As such, the Dust Abatement Plan provisions have been incorporated
into the Storm Water Management Plan for Construction. The Storm Water
Management Plan for Construction states that a water truck shall actively control
dust migration from the site through watering. During earth moving activities,
temporary seeding may also be used to control dust migration. Once earth
moving activities are complete, all areas shall be stabilized with pavement,
landscaping, or permanent seeding. All landscaped areas shall be closely
monitored during the period between planting the permanent vegetation and
realizing a healthy stand of native plants. A water truck shall be used as required
for dust control in landscaped areas during this interim period.
F. The applicant shall submit evidence of an Air Pollution Emission Notice
(A.P.E.N.) and Emissions Permit application for the Air Pollution Control
Division (APCD), Colorado Department of Health and Environment.
Alternately, the applicant can provide evidence from the APCD that they are

not subject to these requirements. Written evidence of such shall be provided to
the Departments of Public Health and Environment and Planning Services.
WMNC is currently Construction Permit and Air Pollutant Emission Notice (APEN)
exempt for its existing paint booth operation at the Fort Collins Facility. Painting
operations will be transferred to the new WMNC Operations Facility in Weld
County. Painting operations will continue to be monitored and if emission levels
trigger permitting / notification requirements, then the appropriate notifications will
be made to the Colorado Department of Public Health and Environment (CDPHE)
and Weld County.
WMNC also investigated the requirements for the APEN — Land Development and
has determined that this APEN will not be required since the project area to be
disturbed is less than 25 acres and that this area will not remain disturbed for a period
exceeding six months as discussed in Regulation 3 - Air Contaminant Emissions
Notices,Part A.II.D.l.j.
G. The impoundment is subject to Regulation No. 61 of the Water Quality Control
Commission of the Colorado Department of Public Health and Environment.
The facility shall provide evidence to the Weld County Department of Public
Health and Environment that it has either applied for a Colorado Discharge
Permit System (CDPS) permit or waiver. Written evidence of such shall be
provided to the Department of Planning Services.
WMNC will seek a waiver from the Ground-Water Discharge Permit Program
implemented by Water Quality Control Commission of the Colorado Department of
Public Health and Environment for the impoundment onsite. During construction of
the clay liner, WMNC will perform a permeability testing on the liner to demonstrate
compliance with the le cm/second seepage requirements set by the state. A letter
report documenting the permeability test results will be presented to Michael Lin77i
at the Water Quality Control Commission to show compliance and obtain a waiver.
The waiver and documentation report will be provided to the Weld County
Department of Health as evidence of compliance.
H. The applicant shall submit evidence of an Aboveground Storage Tank Permit
from the Colorado Department of Labor and Employment, Oil Inspection
Section, for any aboveground storage tanks located on the site. Alternately, the
applicant can provide evidence from the Colorado Department of Labor and
Employment, Oil Inspection Section, that they are not subject to these
requirements. Written evidence of such shall be provided to the Department of
Planning Services.
WMNC has chosen to store diesel fuel in a 10,000 gallon double walled UL listed
2080 fire rated above ground storage tank, but cannot procure the permit from the
Colorado Department of Labor & Employment - Division of Oil and Public Safety
until the site receives an address. The site cannot receive an address until Weld
County approves the plat. WMNC asks that Weld County proceed with the approval
of the plat in order to initiate the above ground fuel storage permit process. The
applicable Above Ground Storage Tank Registration and Above Ground Storage
Tank System Installation and Upgrade Application forms are attached for reference.
WMNC will provide Weld County with copies of the permit approval for the diesel
fuel tank before it is placed into service.

I. A detailed Design and Operation Plan for the truck wash and repair shop floor
shall be submitted to the Environmental Health Services Division of the Weld
County Department of Public Health and Environment, for review and
approval. The plan shall be reviewed and stamped by a registered professional
engineer, and demonstrate that construction of the truck wash and evaporation
pond will limit discharges to the environment within the limits described in the
rules and regulations of the Water Quality Control Division of the Colorado
Department of Public Health and Environment. The plan should include, at a
minimum,the following:
The Design and Operation Plan for the truck wash facility located within the Shop
and Container Repair Facility was combined with the Waste Handling Plan. The
truck wash facility is the major contributor to the lined evaporative lagoon located
onsite. This prompted the truck wash to be included in the Waste Handling Plan.
Please refer to item J. for additional information. The plan was submitted to Char
Davis on April 11, 2006 for approval by the Environmental Health Services Division
of the Weld County Department of Public Health and Environment.
1) A description of the materials used to construct the wash bay.
A description of the materials used to construct the wash bay is included in
the report.
2) The method of construction incorporated to prevent wastewater from
escaping the structure.
The construction design features used to capture all waste within the wash
bay is included in the report.
3) A schematic of the flow of water through the wash bay, screen, and
oil/grease trap to the evaporation pond.
A schematic flow diagram of the water cycling through the wash bay is
included in the report.
4) A water balance, or other demonstration, that the evaporation pond is
adequately sized to store and dispose of the anticipated,hydraulic load
from the trailer wash and the stormwater runoff.
A water balance is included with the report to show that the lined lagoon was
sized to handle the anticipated flows and storm water runoff.
.1. The applicant shall submit a Waste Handling plan, for approval, to the
Environmental Health Services Division of the Weld County Department of
Public Health and Environment. The plan shall include, at a minimum, the
following:
The Waste Handling Plan was submitted to Char Davis on April 11, 2006 for
approval by the Environmental Health Services Division of the Weld County
Department of Public Health and Environment. The Waste handling plan addresses
the Design and Operation Plan for the truck wash facility located within the Shop and
Container Repair Facility. These two reports were combined since the truck wash
provides the main source of discharge to the lined evaporation lagoon.

1) A list of wastes which are expected to be generated on the site (this
should include expected volumes and types of waste generated).
A list of wastes generated, including anticipated volumes, is included in the
report.
2) A list of the type and volume of chemicals expected to be stored on the
site.
A list of chemicals stored onsite, including types and volumes, is included in
the report.
3) The waste handler and facility where the waste will be disposed
(including the facility name,address, and phone number).
A list of waste handlers for the refuse and recyclables coming from the site is
included in the report.
4) Written evidence of approval by the Department of Public Health and
Environment shall be provided to the Department of Planning Services.
WMNC requests that the Department of Public Health and Environment
provide written approval to the Department of Planning Services.
K. The applicant shall attempt to address the requirements of the Windsor-
Severance Fire Protection District, as outlined in the referral received
September 15, 2005. Written evidence of such shall be provided to the
Department of Planning Services.
WMNC has attempted to address the referral comments from the Windsor-Severance
Fire Protection District. A copy of the letter sent to the Windsor-Severance Fire
Protection District addressing each comment has been attached for reference.
L. The applicant shall submit two (2) paper copies of the plat for preliminary
approval to the Weld County Department of Planning Services.
WMNC has attached two paper copies of the plat for preliminary approval. These
copies reflect the comments provided by Chris Gathman during a pre-preliminary
review.
To adequately address these conditions, RTW has created three individual review submittals
tailored to each of the following Weld County departments: Public Works, Health, and Planning
and Zoning. WMNC has submitted the following items directly to the Weld County Department

Collateral for Improvements with the Weld County Departments of Planning
Services and Public Works. WMNC requests that the Department of Public Works
allow the County to move forward with the approval of the plat in order to begin the
permit review and approval process. Before completion of the USR process, this
agreement shall be in place.
In addition to the items required by the Board of County Commissioners of Weld County, RTW
believes that the Weld County Department of Public works needs to review and approve the
Storm Water Management Plan for Construction, the Hydrology Report, and the roadway
construction design plans as part of the approval process. Accordingly, WMNC has included said
reports and design plans with this submission for review and approval by Weld County
Department of Public Works. The two reports will identify systems used to help prevent storm
water pollutants from escaping the site during construction and how post developed storm waters
are managed within County Road 25 right-of-way.
WMNC requests that the Department of Public Works approve the plat and release all related
permits. WMNC understands that approval of the plat by the Department of Public Works does
not free WMNC from actively pursuing an Improvements Agreement Regarding Collateral for
Improvements and a Long-Term Road Maintenance and Improvements Agreement with the County.
WMNC shall finalize both agreements with the Weld County Department of Public Works prior to
completion of the USR phase.

Due to the nature of the improvements, dust abatement activities will only be
needed during the construction phase of the project. All drive lanes, operational
areas and parking areas at the facility will be paved with concrete or asphalt. The
container storage area will be surfaced with road base or recycled asphalt. Non
paved areas including landscape berms and the stormwater detention pond will be
vegetated. As such, the Dust Abatement Plan provisions have been incorporated
into the Storm Water Management Plan for Construction. The Storm Water
Management Plan for Construction states that a water truck shall actively control
dust migration from the site through watering. During earth moving activities,
temporary seeding may also be used to control dust migration. Once earth
moving activities are complete, all areas shall be stabilized with pavement,
landscaping, or permanent seeding. All landscaped areas shall be closely
monitored during the period between planting the permanent vegetation and
realizing a healthy stand of native plants. A water truck shall be used as required
for dust control in landscaped areas during this interim period.
F. The applicant shall submit evidence of an Air Pollution Emission Notice
(A.P.E.N.) and Emissions Permit application for the Air Pollution Control
Division (APCD), Colorado Department of Health and Environment.
Alternately, the applicant can provide evidence from the APCD that they are
not subject to these requirements. Written evidence of such shall be provided to
the Departments of Public Health and Environment and Planning Services.
WMNC is currently Construction Permit and Air Pollutant Emission Notice (APEN)
exempt for its existing paint booth operation at the Fort Collins Facility. Painting
operations will be transferred to the new WMNC Operations Facility in Weld
County. Painting operations will continue to be monitored and if emission levels
trigger permitting / notification requirements, then the appropriate notifications will
be made to the Colorado Department of Public Health and Environment (CDPHE)
and Weld County.
WMNC also investigated the requirements for the APEN — Land Development and
has determined that this APEN will not be required since the project area to be
disturbed is less than 25 acres and that this area will not remain disturbed for a period
exceeding six months as discussed in Regulation 3 - Air Contaminant Emissions
Notices,Part A.Il.D.1.j.
G. The impoundment is subject to Regulation No. 61 of the Water Quality Control
Commission of the Colorado Department of Public Health and Environment.
The facility shall provide evidence to the Weld County Department of Public
Health and Environment that it has either applied for a Colorado Discharge
Permit System (CDPS) permit or waiver. Written evidence of such shall be
provided to the Department of Planning Services.
WMNC will seek a waiver from the Ground-Water Discharge Permit Program
implemented by Water Quality Control Commission of the Colorado Department of
Public Health and Environment for the impoundment onsite. During construction of
the clay liner,WMNC will perform a permeability testing on the liner to demonstrate
compliance with the 10-6 cm/second seepage requirements set by the state. A letter
report documenting the permeability test results will be presented to Michael Liuzzi

at the Water Quality Control Commission to show compliance and obtain a waiver.
The waiver and documentation report will be provided to the Weld County
Department of Health as evidence of compliance.
H. The applicant shall submit evidence of an Aboveground Storage Tank Permit
from the Colorado Department of Labor and Employment, Oil Inspection
Section, for any aboveground storage tanks located on the site. Alternately, the
applicant can provide evidence from the Colorado Department of Labor and
Employment, Oil Inspection Section, that they are not subject to these
requirements. Written evidence of such shall be provided to the Department of
Planning Services.
WMNC has chosen to store diesel fuel in a 10,000 gallon double walled UL listed
2080 fire rated above ground storage tank, but cannot procure the permit from the
Colorado Department of Labor & Employment - Division of Oil and Public Safety
until the site receives an address. The site cannot receive an address until Weld
County approves the plat. WMNC asks that Weld County proceed with the approval
of the plat in order to initiate the above ground fuel storage permit process. The
applicable Above Ground Storage Tank Registration and Above Ground Storage
Tank System Installation and Upgrade Application forms are attached for reference.
WMNC will provide Weld County with copies of the permit approval for the diesel
fuel tank before it is placed into service.
I. A detailed Design and Operation Plan for the truck wash and repair shop floor
shall be submitted to the Environmental Health Services Division of the Weld
County Department of Public Health and Environment, for review and
approval. The plan shall be reviewed and stamped by a registered professional
engineer, and demonstrate that construction of the truck wash and evaporation
pond will limit discharges to the environment within the limits described in the
rules and regulations of the Water Quality Control Division of the Colorado
Department of Public Health and Environment. The plan should include, at a
minimum,the following:
The Design and Operation Plan for the truck wash facility located within the Shop
and Container Repair Facility was combined with the Waste Handling Plan. The
truck wash facility is the major contributor to the lined evaporative lagoon located
onsite. This prompted the truck wash to be included in the Waste Handling Plan.

Davis on April 11, 2006 for approval by the Environmental Health Services Division
of the Weld County Department of Public Health and Environment.

Department of Public Health and Environment. The Waste handling plan addresses
the Design and Operation Plan for the truck wash facility located within the Shop and

In accordance with the requirements of USR #1527, Rothberg Tamburini and Winsor, Inc. on
behalf of Waste Management Disposal Services of Colorado, Inc. has attempted to address the
Windsor-Severance Fire Protection District comments outlined in the referral letter dated
September 15, 2005. For clarity, comments from the Windsor-Severance Fire Protection District
referral letter are addressed individually as follows:
1. There shall be a minimum of two points of access to this facility.
The facility was designed with two points of access that are able to accommodate the
turning radius of a fire engine,the visitor and employee entrance and the truck entrance.
The location and width of these entrances are shown on the attached plan.
2. Each access shall be designed to carry a designed wheel load of 18000
pounds(80000 pound gvw) (H2O loading).
The truck entrance has a H20 loading design capacity for a high volume
of truck traffic. This design is sufficient to support the fire trucks. The
visitor and employee entrance is designed for a high volume of light
vehicle traffic,but can support a low volume of H20 loading without
failure. We anticipate that the fire department will enter the employee
and visitor entrance infrequently.
3. The accesses shall be constructed of a permanent all weather road base.
Both site accesses are paved with all weather road surfaces. The employee and visitor
access is paved with an asphalt concrete surface. The truck entrance is paved with a
concrete surface.
4. Addresses shall be posted clearly on the road fronting the property.

The address will be posted clearly along County Road 25.
5. Addresses shall be posted clearly on the structure.
Both buildings onsite shall share the same address. Waste Management of Northern
Colorado will post the address on the buildings.
6. If any of the gates to this site are to be automatic they shall be
provided with a KNOX key override switch.
The gates to this site are not automatic. A KNOX box will be provided at
each building to accommodate fire department access.
7. Both buildings shall be provided with an approved Fire Sprinkler
System and the fire line shall be shown on the site utility drawings and
labeled as such and approved by the Windsor-Severance Fire Protection
District.
A 12-inch diameter fire main serves as the backbone for the onsite fire
service. This main connects directly to the 12-inch diameter North Weld
County Water District water main within the County Road 25 right of way.
From the onsite 12-inch diameter main, a 6-inch diameter water service
branches off to serve each building's fire sprinkler system. The sprinkler
systems are designed at a flow of 500 gallons per minute. To overcome the
minimal pressures found onsite, each building is equipped with a booster
pump. The sprinkler system design will be submitted to the District for
review and approval..
8. The minimum fire flows required for this project are 1500 gpm at a
minimum of 20 psi and a flow duration of three(3) hours.
North Weld County Water District can currently supply 500 gallons per
minute at 28 psi at the connection point to the facility. This does not permit
a flow of 1500 gpm at 20 psi at any onsite fire hydrant.
Waste Management has designed the onsite water system to optimize future
water flow and pressure conditions available from NWCWD. After
NWCWD constructs improvements to their system upstream of the WMNC
Operations Facility in 2006-2008, the water provider estimates that their
system can supply 2000 gpm at a pressure of 26 psi. With these
improvements, the onsite system will provide 1075 gpm at 20 psi from the
hydrant west of the Administration Building, and 1240 gpm at 20 psi from
each of the other two fire hydrants onsite. These flows represent what is
anticipated from each hydrant during a solo hydrant test.

Additional NWCWD improvements planned for a few years beyond 2008
will improve the pressure at the site connection point significantly. With
these secondary planned improvements, all onsite fire hydrants can provide
1500 gallons per minute at 20 psi.
9. The minimum number of fire hydrants required shall be four (4).
The layout of the buildings on the site has changed slightly since the approval of the USR.
With the changes, we have determined that three fire hydrants will provide adequate
service to the entire facility. The three hydrants are arranged so that one hydrant provides
fire protection coverage for both buildings onsite.
10. The minimum water line sizes for the fire hydrants shall be 8".
The 12-inch diameter main water feed for the property was oversized to minimize frictional
losses onsite.
11. The Windsor-Severance Fire Protection District shall be afforded the
opportunity to review and approve all building drawings before a
building permit is issued.
The Windsor-Severance Fire Protection District shall have the opportunity to
review the plans prior to issuance of the building permit.
12. The Windsor-Severance Fire Protection District shall be afforded the
opportunity to review and approve all construction drawings for the
installation of any fuel tanks or associated appliances.
The Windsor-Severance Fire Protection District shall have the opportunity to
review the plans for the installation of fuel tanks and associated
appurtenances.

Shallow waste disposal systems/wells(Class V injection wells)release waste fluids into or above shallow ground
water and include: commercial or industrial septic systems, sumps, drain fields, french drains, cesspools,abandoned
drinking water wells, dry wells, and infiltration galleries. The Underground Injection Control(UIC)regulations require
inventory information for all disposal systems/wells and additional information for certain types of systems/wells.
This form is designed to collect basic information for all systems/wells, to determine which are used for underground
disposal of waste fluids, and additional information for those systems/wells with a greater potential for contaminating
ground water.
