RE: APPROVE TWO AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two Amendments to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and the
following providers, with terms and conditions being as stated in said agreements, and
1. Turning Point Center for Youth and Family Development, Inc. (October 1, 2009 —
Until another group home is up and running)
2. Sionnach Counseling Services, LLC (December 31, 2009— May 31, 2010)
WHEREAS,after review, the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the two Amendments to Child Protection Agreements for Services between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and the above listed providers be, and
hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

RE: Amendment to Child Protection Agreements for Services
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval is an Amendment to Child Protection Agreements for Services
between the Department and Various Providers. These amended agreements were
presented at the Board's January 6, 2010, Work Session.
The major provisions of these Agreements are as follows:

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES / _,
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND j��'„1/
TURNING POINT CENTER FOR YOUTH AND FAMILY DEVELOPMENT,INC.
This Amendment, made and entered into the day of November 2009,by and between the Board of County
Commissioners,on behalf of the Weld County Department of Human services,hereinafter referred to as"Department,"and
Turning Point Center for Youth and Family Development, Inc.,hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(PY-09-10-CPS-0105)in order to amend the scope
of service and fee for service; and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and with appropriate
agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree to as follows as of
October 1,2009:
1. Exhibit A,Scope of Services,Item 1,is hereby revised to reflect the following:
CONTRACTOR will provide group therapy and emergency services to the four(4)group homes certified and
managed by the Department.
2. Exhibit B, Payment Schedule, Item 2,is hereby revised to reflect the following:
$3,600.00/Month(All inclusive rate for four(4)group homes)
This Amendment,together with the original Child Protection Agreement for Services,constitutes the entire
understanding among the parties with respect to the subject matter hereof,and may not be changed or modified except as
stated in the Child Protection Agreement for Services.

BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
SIONNACH COUNSELING SERVICES,LLC
This Amendment,made and entered into the day of December 2009,by and between the
Board of County Commissioners,on behalf of the Weld County Department of Human services,hereinafter
referred to as"Department,"and Sionnach Counseling Services,LLC,hereinafter referred to as the
"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(PY-09-10-CORE-0177)in
order to increase the maximum expenditure allowable under the contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule,Item 1,is hereby revised to reflect the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$18,000.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
