Box 215 Greeley, CO 80632 Dear Mr. Gordy, Pursuant to our lease of 12 April 1993, I, on behalf of the Weld County Board of Commissioners, provide notice that we will be terminating our lease on the property at 1830 Eighth Avenue, Greeley, Colorado, described as follows: "The top floor of the building situated on Lots 33 and 34, Block 9, Arlington Heights, City of Greeley, Weld County, Colorado." The effective date of termination will be November 1, 2010. We will be locating the Weld County Veterans Service Office to 1551 North 17th Avenue, Greeley, CO. The building is adjacent to the Weld County Health Department Building in the Weld County Business Park. The new location is handicapped accessible and is serviced by City Bus and has ample parking. There are no stairs or ramps to complicate entry for our Veterans. We will cease utility payments on 1830 8th Avenue effective November 1, 2010. We will strive to continue our excellent relationship with your organization as well as other Veterans groups throughout Weld County. Box 215 Greeley CO 80632 E-Mail:jgordy@wildblue.net September 4, 2010 WELD COUNTY COMMISSIONERS 914- 10th Street Greeley, Colorado 80631 Dear Commissioners, With the approach of snow season, we have concerns about snow removal at the Weld County Veterans Service Office, located at 1830- 8th Avenue, Greeley. Many of the clients of the Service Officer are disabled, and can not navigate steps, and stairs. We request that snow removal be accomplished prior to 8:00 AM, Monday through Friday, from the following areas of said property: 1. Driveway, and the parking lot at rear of the building. 2. Front sidewalk, handicap ramp, and porch at front of building. 3. Sidewalk at rear of building, from driveway to back door. These actions will insure that all veterans, and family members can keep needed appointments with the Service Officer. Gore - I,� ERS Adjutant RESOLUTION RE: APPROVE LEASE OF BUILDING WITH DISABLED AMERICAN VETERANS GREELEY CHAPTER #8, INC. , AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Lease of Building between the Disabled American Veterans Greeley Chapter #8, Inc. , and Weld County Veterans Services Office, by and through the Board of County Commissioners, commencing May 1, 7993, and ending May 1, 1994, with the further terms and conditions being as stated in said lease, and WHEREAS, after review, the Board deems it advisable to approve said lease, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Lease of Building between the Disabled American Veterans Greeley Chapter #8, Inc. , and Weld County Veterans Services Office, by and through the Board of County Commissioners, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said lease. Box 215, Greeley, Colorado 80632, Lessor, and the Board of County Commissioners of the County Weld, Colorado, on behalf of the Weld County Veterans Services Office, whose address is 915 Tenth Street, Greeley, Colorado 80631, Lessee. IN CONSIDERATION of the payment of all utilities during the period of this Lease and the performance of the covenants and agreements by the Lessee set forth below, Lessor does hereby lease to the Lessee the following described premises located in the City of Greeley, Colorado, the address of which is 1830 Eighth Avenue, Greeley, Colorado 80631, described as follows: The top floor of the building situated on Lots 33 and 34, Block 9, Arlington Heights, City of Greeley, Weld County, Colorado. TO HAVE AND TO HOLD the same with all the appurtenances unto the said Lessee from 12 :00 A.M. on the first day of May, 1993 and until 12 :00 A.M. on the first day of May, 1994 and shall continue on a year-to-year basis thereafter, subject to the termination provisions in paragraph "D", below. No rental shall be charged and/or collected by the Lessor. The Lessee, in consideration of the leasing of the premises, agrees as follows : 1. To pay all utilities for the period of this Lease including, but not limited to, gas, electric, water, trash, and telephone used by Lessee. 2. To keep the premises in a clean and sanitary condition as required by the ordinances of the City of Greeley, Colorado. 3. To sublet no part of the premises, and not to assign the Lease or any interest therein without the written consent of the Lessor. 4. To use the premises only as offices for the Weld County Veterans Services, and to use the premises for no purposes prohibited by the laws of the United States or the State of Colorado, or of the ordinances of the City of Greeley, Colorado, and for no improper or questionable purposes whatsoever, and to neither permit nor suffer any disorderly conduct, noise, or nuisance having a tendency to annoy or disturb any persons occupying adjacent premises. 5.
