RE: APPROVE SIX (6) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with six (6) Amendments to Child Protection
Agreements for Services with Various Providers between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, and the following providers, commencing June 1,2010, and ending May 31, 2011,
with further terms and conditions being as stated in said amendments, and
1. Jubilee Retreat Center (Life Skills/Bonding Program)
2. Child Advocacy Resource Education, Inc. (Life SkillsNisitation)
3. Perklen Center for Psychotherapy (Mental Health Services)
4. Family Matters of Colorado, LLC (Mental Health Services)
5. Perklen Center for Psychotherapy (Sex Abuse Treatment/Offense Specific
Treatment)
6. Ackerman and Associates, P.C. (Sexual Abuse Treatment/Safe-T Program)
WHEREAS,after review,the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the six (6) Amendments to Child Protection Agreements for Services with
Various Providers between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, and the
above listed providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

RE: Amendments to Child Protection Agreements for Services
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and Various Providers. These amended Agreements were reviewed
by the Board by Pass-Around Memorandum dated December 13, 2010, and approved for
placement on the Board's Agenda.
These amendments to original allocations are necessary to stay within the Core Allocation for
the 2010 program year. The amendments to these agreements will increase funding to
providers who have an increased need for services while decreasing to others who's demand
for services is not as high as expected. The major provisions of these amended Agreements
are as follows:

hereinafter referred to as"Department,"and Child Advocacy Resource and Education,Inc.(c.a.r.e.),
hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Life Skills-Visitation(PY-
10-11-CORE-0122)in order to decrease the maximum reimbursement under this contract; and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:

The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed $60,000.00.

the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Mental Health Services(PY-
10-11-CORE-0140)in order to increase the maximum reimbursement under this contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
