RE: APPROVE ASSUMPTION OF CHILD PROTECTION AGREEMENT FOR SERVICES AND
AUTHORIZE CHAIR TO SIGN - PERKLEN, INC., DBA PERKLEN CENTER FOR
PSYCHOTHERAPY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Assumption of Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and
Perklen, Inc., dba Perklen Center for Psychotherapy, which was previously entered into with
Individual and Group Therapy Services, commencing June 1, 2009, with further terms and
conditions being as stated in the assumption of said agreement, and
WHEREAS, after review, the Board deems it advisable to approve the assumption of said
agreement by Perklen, Inc., dba Perklen Center for Psychotherapy, a copy of which is attached
hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Assumption of Child Protection Agreement for Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and Perklen, Inc., dba Perklen Center for
Psychotherapy, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 11th day of January, A.D., 2010, nunc pro tunc June 1, 2009.

and Perklen Incorporated dba Perklen Center for
Psychotherapy
Enclosed for Board approval is an Assumption of Child Protection Agreement for Services
between the Department and Perklen Incorporated dba Perklen Center for Psychotherapy.
This Agreement was presented at the Board's January 6, 2010, Work Session.
Individual & Group Therapy Services (IGTS) Child Protection Agreements for Services
changed corporation and agency names. They are now known as Perklen Incorporated dba
Perklen Center for Psychotherapy.
The contracts referred to in this document include: PY-09-10-CPS-0093 (Anger
Management/Domestic Violence), PY-09-10-Core-0140 (Mental Health), and PY-09-10-
Core-0160 (Sex Abuse Treatment).

ASSUMPTION OF CHILD PROTECTION AGREEMENT FOR SERVICES`'•` .•,• ,,
Whereas, Individual & Group Therapy Services (IGTS)(the"Contractor"), and the Board of
Weld County Commissioners, sifting as the Board of Human Services, on behalf of the Weld County
Department of Human Services (the"Department"), entered into Child Protection Agreements for
Services(PY-09-10-CPS-0093, PY-09-10-CORE-0140, and PY-09-10-CORE-0160)dated June 1,2009,
incorporated herein by this reference (the "Agreements"), wherein Contractor will provider Anger
Management/Domestic Violence, Mental Health and Sex Abuse Treatment(Offense Specific) Services,
and;
Whereas, 1GTS desires to assign all of its rights and obligations under the Agreements to Perklen
Incorporated dba Perklen Center for Psychotherapy; and
Whereas, Perklen Incorporated dba Perklen Center for Psychotherapy intends to benefit the
Department by promising to perform all terms a,:.1 conditio:L of the Agreements.
Now, therefore, in consideration of the Department's approval and other mutual covenants and
agreements expressed in the Agreements, Perklen Incorporated dba Perklen Center for Psychotherapy
does hereby covenant and agree to assume all obligations, responsibilities, and terms of the Agreements
and hereby becomes the Contractor under the Agreements.
Further, Perklen Incorporated dba Perklen Center for Psychotherapy submits to the Department
the following notice of address pursuant to Section 16 of the Agreements:

Further, Perklen Incorporated dba Perklen Center for Psychotherapy designates to the Department
the following Contractor representative pursuant to Section 15 of the Agreements:

Further, Perklen Incorporated dba Perklen Center for Psychotherapy submits to the Department
herewith,the proof of insurance as required in Section 9 of the Agreements.
