IN THE MATTER OF APPROVAL OF LETTER OF AGREEMENT BETWEEN WELD
COUNTY HEALTH DEPARTMENT AND JUNE C. ESSING FOR PHYSICAL THERAPY
SERVICES.
BE IT ORDAINED BY THE BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY,
COLORADO:
WHEREAS, a letter of agreement between the Weld County Health
Department and June C. Essing for physical therapy services has
been presented to the Board of County Commissioners of Weld County,
l Colorado. A copy of said letter of agreement is attached hereto

and incorporated herein by reference at this point, and
WHEREAS, the term of said letter of agreement is from January 1,
1979 through December 31, 1979 , and

WHEREAS, the Board of County Commissioners of Weld County,

Colorado deems it advisable and in the best interests of Weld County
to approve said letter of agreement.
NOW, THEREFORE, BE IT ORDAINED by the Board of County Com-
missioners of Weld County, Colorado that the letter of agreement
between the Weld County Health Department and June C. Essing for
physical therapy services be, and hereby is , approved.
BE IT FURTHER ORDAINED by the Board of County Commissioners
of Weld County, Colorado that an emergency exists which requires
that this Ordinance be enacted forthwith in that said letter of
agreement must be in effect by January 1, 1979 and normal Ordinance
procedures would prevent this, and therefore, this Ordinance is
declared to be an emergency Ordinance under the provisions of
Section 3-14 of the Weld County Home Rule Charter.
The above and foregoing Ordinance No. 47 was, on motion duly

LETTER OF AGREEMENT
between
HOME HEALTH CARE SERVICES of WELD COUNTY HEALTH DEPARTMENT
and the
PHYSICAL THERAPIST
Revised 1-1-79
The following policy or procedure items are agreed upon:
1. The Physical Therapist is a graduate of a program in physical therapy approved
by the Council on Medical Education of the American Medical Association in collaboration
with the American Physical Therapy Association or its equivalent.
2. The Physical Therapist must have a current license and be registered in the
State of Colorado. License No. 514 .
3. The Physical Therapist assists the physician in evaluating patients by applying
diagnostic and prognostic muscle, nerve, joint and functional ability tests.
4. The Physical Therapist treats patients to relieve pain, develop or restore
function, and maintain maximum performance, using physical means such as exercise,
massage, heat, water, light and electricity; for example:
A. Directs and aids patients in active and passive exercises, muscle
re-education, and gait and functional training, activities of daily
living including transfer activities and prosthetic training.
B. Makes use of equipment such as ultraviolet and infrared lamps, low voltage
generators, diathermy and ultrasonic machines.
C. Gives whirlpool and contrast baths and applies moist packs.
5. The Physical Therapist will utilize Weld County Health Department equipment
to provide services.
6. The Physical Therapist observes, records and reports to the physician the
patient' s reaction to treatment and any changes in the patient's condition.

7. The Physical Therapist instructs patients in care and use of wheelchairs,
braces, crutches, canes and prosthetic and orthotic devices.
8. The Physical Therapist instructs other health team personnel, including,
when appropriate, home health aides and family members in certain phases of physical
therapy with which they may work with the patient.
9. The Physical Therapist instructs family on patient's total physical therapy
program.
10. The Physical Therapy Services agrees to supply physical therapy on a part-time
basis for patients referred to them by the Home Health Care Service in accordance
with a physician's order.
11. The Physical Therapist will be paid at $14.00 per visit and 14 cents per mile
for reimbursement.
12. The Physical Therapist will submit a daily report of activities, monthly
summary of travel expenses and such other reports and records as directed by the
Weld County Health Department Nursing Division.
13. The payment for this service will be made directly to the Physical Therapy
Service.
14. It is understood and agreed that this agreement is effective January 1, 1979
until December 31, 1979 and that either party may terminate this agreement at any
time provided, not less than sixty (60) days prior to the date of such termination,
notice in writing shall have been given the opposite party.

Dear Commissioner Steinmark:
This letter will bring you the approval of the Board of Health with four
members of the Board present, and transmit to the Board of County Commissioners
for your consideration the following:
Letter of Agreement between WCHD Home Health Care Service and Physical
Therapist June C. Essing, RPT for physical therapy services at the
current Medicare rates for the period beginning January 1, 1979 to
be reviewed annually. Contract may be terminated by either party on
60-days notice.
We recommend favorable action on the above agreement.
