RE: APPROVE AMENDMENT TO AGREEMENT FOR PROFESSIONAL SERVICES AND
AUTHORIZE CHAIR TO SIGN - SUSAN BJORLAND
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Agreement for
Professional Services between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, and
Susan Bjorland, commencing July 31, 2010, and ending December 31,2010,with further terms and
conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Agreement for Services between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Human Services, and Susan Bjorland be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 8th day of September, A.D., 2010, nunc pro tunc July 31, 2010.

RE: Amendment to the Agreement for Professional Services
between the Weld County Department of Human Services
and Susan Bjorland
Enclosed for Board approval is an Amendment to the Agreement for Professional Services between
the Department and Susan Bjorland. This Agreement was presented at the Board's August 30,
2010, Work Session.
The major provisions of this Agreement are as follows:

AMENDMENT TO AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
SUSAN BJORLAND
This Amendment, made and entered into the day of August 2010,by and between the Board of
County Commissioners, on behalf of the Weld County Department of Human Services, hereinafter referred to as
"Department," and Susan Bjorland, hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Human Management Consultant
services(Contract No. 09-10-CPS-87) in order to increase the time period and maximum expenditure allowable under
the contract; and
WHEREAS,Human Services has budgeted, appropriated,and otherwise made available, and
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows:
L Exhibit B, Payment Schedule, Item I, is hereby revised to reflect the following:
Human Services agrees to reimburse Contractor in consideration for the work and services performed
for various Divisions within the Department under appropriate Human Services/Social Services
funding sources not to exceed$64,600.00.
2. Exhibit B, Payment Schedule, Item 2, is hereby revised to reflect the following:
Human Services agrees to reimburse the Contractor for the period ending December 31, 2010,pursuant
to the terms and conditions of Exhibit A and Exhibit B as amended.
These Amendments,together with the original Agreement for Professional Services, constitutes the entire
understanding among the parties with respect to the subject matter hereof, and may not be changed or modified
except as stated in the Agreement for Professional Services.
IN WITNESS WHEREOF, the parties hereto have duly executed this Agreement as of the day, month, and
year first above written.
