RE: APPROVE THREE AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with three Amendments to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and the
providers listed below, commencing June 1, 2009, and ending May 31, 2010, with further terms
and conditions being as stated in said amendments, and
1. Victor Cordero, Psy.D., P.C. (Mental Health Services)
2. Poudre Valley Health Care, Inc.,dba Poudre Valley Hospital(Home-Based Services)
3. Transitions Psychology Group, LLC (Mental Health Services)
WHEREAS,after review,the Board deems it advisable to approve said amendments,copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the three Amendments to Child Protection Agreements for Services between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and the providers listed above be, and
hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

COLORADO RE: Amendment to Child Protection Agreements for Services
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval is an Amendment to Child Protection Agreements for Services
between the Department and Various Providers. These amended agreements were
presented at the Board's February 3, 2010, Work Session.
The major provisions of these Agreements are as follows:

This Amendment,made and entered into theeay of Jmma./2010,by and between the Board
of County Commissioners,on behalf of the Weld County Department of Human services,hereinafter
referred to as"Department,"and Victor H.Cordero,Psy.D.,P.C.,hereinafter referred to as the
"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(PY-09-10-CORE-0136)in
order to increase the maximum reimbursement under this contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule,Item 1,is hereby revised to include the following
additional rate:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$38,200.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in th 'on Agreement for Services.

order to increase the maximum reimbursement under this contract;and

with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
