HomeMy WebLinkAbout20102392.tiff RESOLUTION RE: APPROVE MEMORANDUM OF AGREEMENT FOR AMERICAN RECOVERY AND REINVESTMENT ACT (ARRA) CHRONIC DISEASE SELF-MANAGEMENT PROGRAM GRANT FUNDING AND AUTHORIZE CHAIR TO SIGN -CENTRAL COLORADO AREA HEALTH EDUCATION CENTER WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with a Memorandum of Agreement for American Recovery and Reinvestment Act (ARRA) Chronic Disease Self-Management Program Grant Funding between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on Aging, and the Central Colorado Area Health Education Center, commencing October 11,2010, and ending March 30, 2012, with further terms and conditions being as stated in said Memorandum of Agreement, and WHEREAS, after review, the Board deems it advisable to approve said Memorandum of Agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Memorandum of Agreement for American Recovery and Reinvestment Act (ARRA) Chronic Disease Self-Management Program Grant Funding between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on Aging, and the Central Colorado Area Health Education Center be, and hereby is, approved. Griego, Director, Human Se i es De artme COLORADO RE: Memorandum of Understanding between the Weld County Department of Human Services' Area Agency on Aging and the Central Colorado Area Health Education Center (CCAHEC) Enclosed for Board approval is a Memorandum of Understanding between the Department's Area Agency on Aging and Central Colorado Area Health Education Center (CCAHEC). This Memorandum of Understanding (MOU) was presented at the Board's August 30, 2010, Work Session. In conjunction with the Colorado Department of Public Health and Environment's ARRA Chronic Disease Self-management grant, the Department of Human Services' Area Agency on Aging must sign a Memorandum of Understanding with the Central Colorado Area Health Education Center (CCAHEC) in order to receive payment for all participants. The CCAHEC will reimburse the Department S110.00 per client that complete the necessary number of Chronic Disease classes. CCAHEC has contracted with the Colorado Department of Health and Environment to implement and provide oversight for the Chronic Disease Self-management Expansion Program. The effective date of this MOU shall be as of the date of signing and shall be effective until March, 2012. If you have questions, please give me a call at extension 6510. 2010-2392 Developing Community-Academic Partnerships to Promote Healthier Colorado Residents )`e ��o �do P� This memorandum of agreement is entered into this 27 day of September, 2010 by Central Colorado Area Health Education Center(CC AHEC), a non-profit corporation incorporated in the state of Colorado with a principal business address of 12631 E. 17'h Ave, MS F496, Aurora, Colorado 80045 and Weld County Area Agency on Aging(WCAAA) ( 970 346-6950 Name of Agency Telephone PO Box 1805 / 315 North 11th Avenue#C Greeley, CO 80632 Address City, State Zip Code Whereas, the CC AHEC is the Implementation Agency for Colorado's Chronic Disease Self- Management Program - LIVE WELL, BE WELL and Whereas, WCAAA desires to participate as a Host Site for this initiative; Thus, through our mutual premises and the mutual undertakings herein contained, the parties agree: To implement the Colorado LIVE WELL, BE WELL Chronic Disease Self-Management Program (CDSMP) under a grant from the Administration on Aging and ARRA to the state of Colorado - State Unit on Aging and Colorado Department of Public Health and Environment, Preventative Services to build upon Colorado's existing evidence-based disability and disease prevention partnerships and programs by expanding availability and access to Stanford's Chronic Disease Self- Management Program (CDSMP). The program's goal is to empower older people to take more control of their health and reduce their risk for disease and disability. WCAAA has been selected to become a program implementation Host Site for LIVE WELL, BE WELL. Host Site Agency responsibilities include: a) When offering the general or diabetes specific workshop at your site, operate under a license held by CC AHEC and will jointly be accountable to follow the rules and regulations regarding instruction and administration of the CDSMP in accordance with the Stanford licensing requirements. b) Conducting workshops in accordance with the CDSMP Leaders Manual, without changes, and all reproductions of materials will be done by permission and/or through CDPHE - Prevention Services office. c) Provision of suitable space for workshops(complying with Americans with Disabilities Act standards), healthy snacks and water for attendees. d) Actively recruit participants and lay leaders. e) Maintain ongoing communication with local Lead Agency and CC AHEC. This includes the identified lay leaders(2/class minimum and 2/class for backup). Then, submit updates of all calendared workshop information two months in advance of any changes. g) Submit request for participant books and CDs to CDPHE once workshop is scheduled. h) Complete data collection webinar. i) Utilize state-wide marketing materials supplied by CDPHE- Prevention Services office with the opportunity to add local/regional identifying information. j) Ensure lay leaders follow Workshop Survey Packet directions. Submit required forms to CC AHEC (Workshop Information Cover Sheet, Attendance Log, Participant Information Survey, and Participant Evaluation forms) for all workshops within five business days after each workshop completion. k) Ensure lay leaders coordinate fidelity check for each workshop with the Implementation Agency (CC AHEC). Fidelity checks will be performed by a master trainer engaged by CC AHEC. I) Request payment of$110.00 for each participant"completer" (attendance at 4 of 6 sessions) of the six week fidelity authenticated CDSMP series through invoice to CC AHEC, with completed Workshop Survey Packet. m) Establish plan of action for sustainability of this program with board of directors or ruling board for this agency by end of month four and submit proposed business plan to Lead Agency, (if applicable), CC AHEC, State Unit on Aging and CDPHE -Prevention Services by month six. The Host Site Agency, WCAAA , in performing the services as specified in this service agreement, shall act as an independent contractor. Any and all employees of WCAAA shall be considered to be, at all times, employees of WCAAA , under its sole authority. Further, WCAAA shall indemnify and hold CC AHEC harmless from any and all claims, liabilities, losses and causes of action which may arise from acts, errors or omissions of WCAAA or any of its employees or agents in conjunction with its obligations under this service agreement. Responsibilities of the local Lead Agency: a) Identify and recruit Host Sites in their region and submit to CC AHEC. b) Clarify the number of lay leaders and master trainers within the approved Host Site and surrounding community. Identify additional community members for training as needed to meet the demand of their population for both chronic disease general and diabetes specific programs and delivery language of English and Spanish. Submit these needs. c) Prepare calendar of programs with accepted Host Site for three (3) six month blocks. d) Meet with or refer Host Site to Implementation Agency(CC AHEC)to complete MOU. e) When class schedule is prepared by accepted Host Site and submitted with their MOU, submit request for the class participant notebooks and CD's and marketing materials. Responsibilities of the Implementation Agency: a) Maintain communications with local Lead Agency and Host Sites. b) Coordinate with local Lead Agency contact with approved Host Site to establish MOU. c) Publish on website and update weekly the common calendar of each offered CDSMP series, including contact person and location. d) Schedule and assure fidelity check for each CDSMP series. e) Record evaluation and attendance data and validate number of completers per CDSMP series. f) Pay Host Site $110.00 for each confirmed participant completer, defined as finishing 4 of 6 workshops of a fidelity authenticated CDSMP series. g) Provide information and assistance to Host Sites for access to participant notebooks and CDs and marketing materials through CDPHE. The terms of this agreement will be effective as of the date of signing and shall be effective until March 2012. This agreement for services is contingent upon the receipt of adequate revenue by Grantee identified in Section 1.
