RESOLUTION
RE: AUTHORIZATION FOR CHAIRMAN TO SIGN CUSTOMER AGREEMENT WITH
MOUNTAIN BELL.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, a customer agreement with Mountain Bell concerning
equipment space has been presented to the Board of County Commis-
sioners, and
WHEREAS, the Board of County Commissioners has studied said
agreement and deems it advisable to authorize the Chairman of the
Board to sign the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commis-
sioners of Weld County, Colorado that the Chairman of the Board be,
and hereby is, authorized to sign the customer agreement with
Mountain Bell concerning equipment space.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 14th day of
November, A.D. , 1979 .

NOTE:Items in the bllowing list may be deleted or modified by the Telephone Company Equipment Engineer to satisfy specific situations.No other persons may alter,
modify or delete items without the customers and Telephone Company Equipment Engineers concurrence.The customers and Telephone Company Equipment
Engineers concurerce will be signified by their initials near each change on the original copy of the document.
ELECTRICAL REQUREMENTS
All electrical facilities shall be provided in accordance with the National Electrical Code and local requirements.The customer shall furnish installed,designated quantities
of electrical fioures us described by the following conventions and indicated by floor plan or associated sketches.
1 Standard double receptacle convenience outlet with ground.Mounted -7 • 14 ••above finished floor(A.F.F.).
2-0 Special Service outlet, 1 11 Volt± lit_percent,60 Hz-±t percent,1 phase,3 wire,Fused at 2-o amps.Circuit to have 24 Hour
Service to be used for no other purpose. Outlet to consist of; 1-Hubbell, Twist Lock Connector Receptacle. Catalog #2 3 d 0 Cov-
er# Receptacle to be located on(wall)or(ceiling)a maximum of 5 feet away or 6'6"A.F.F.

MECHANICAL REQUIREMENTS •
The area provided for telephone equipment will conform to the following additional requirements.
9. Width of doorways,passage ways and starkvays must be su8klent to permit the movement of assembled equipment frames or switchboards.Minimum width is
3'.0".The door shall be secured with a tumbler type lock
10. Floors must be level within 1/8 Inch in 8 meal feet and be has of excessive vibration.
11. Cover the equipment room ibor with tile,dnoleum,or sealer(paint)b ovide a durable,dust free surface.Carpet may be used in the attendant area but should
not be used h the equipment room due to the possibility of states charges causing electrical failures.
12 Walls and calling are to be dust free and of a suitable light color to aid Illumination Turpentlra must not be used for thinning paint or for any other purpose in
telephone equipment areas.
13. - Illmisete the equipment and attendant areas uniformy.Ughtlng fixtures must be above the maximum height of equipment and associated cabling.Provide 30
foot-caritas,3'-0"above the floor for the equipment area.

18. Ambient temperature and relative humidity must be maintained 24 hours a day,7 days a week
Temperature:Minimum 85°F,Maximum 91:°F -
- Relative Humidity:Minknum 20%.Maximum 55%
Ambient temperature is determined at a distant of 5'-0"above the floor after the equipment Ls in pace and in operation.
Note:When batteries are supplied the ambient temperature requirement becomes more stringent.In these cases the limits are:Minimum 65 F,Maximum 80 F.

MISCELLANEOUS REQUIREMENTS
In order to insure the security and operational integrity of the equipment during installation and future operation and well being of the installation and
maintenance personnel the following items are to provided:
20. The telephone equipment area shall be free of excessive electromagnetic radiation emanating from such sources as:Radar transmitters,radio communications
equipment x-ray-devices,non-synchronous motors,etc.
21. Copying machines SHALL NOT be situated kin the PBX equipment area.The area allocated to telephone equipment is dedicated for this purpose only.It is not
b be uttlzed for storage of materials,or for work operabohs not cprduc%oe to telephone service.
22. Secure storage for Equipment shall be provided before and during installation.
23. During installation the hollowing shall be provided;
A. Heating for Equipment Room.
B. Access into work area(equipment and storage).
C. Toilet facilities for installation personnel.
