This Proposal was presented at the Board's October 25, 2010, Work Session. The Department's Area Agency on Aging is applying for a direct service funding grant for the Dental, Vision, and Hearing Program from the NCMC Foundation. The NCMC Foundation has supported this program in years past. The program helps cover the costs of basic services needed for minimal dental, vision and hearing needs that are not covered by most health plans, including Medicare and Medicaid. The needs may include exams, extractions, dentures, partials, hearing aids, and/or glasses. The Dental, Vision, and Hearing Program is offered by the Weld County Area Agency on Aging through the Older American Act and Older Coloradoan Fund. WCAAA shares their mission to assist recipients to live safely with maximum personal independence by planning and promoting an effective, integrated, accessible delivery system under the Older Americans Act and Older Coloradan Fund. For over thirty years, WCAAA has been a support system for Weld County residents 60 years of age and older and their caregivers. Our primary focus is to help maintain an individual's independence and dignity in their home and community. The WCAAA supports services for seniors either through contracts with community agencies or by providing services directly. These services may include adult day care, case management, home delivered meals, legal assistance, nursing home and assisted living ombudsman services, peer counseling, rural senior services, Senior Nutrition programs, Hispanic senior outreach, and information and referral. Description and Purpose of Program/Project Since 1998, the Dental, Vision, and Hearing Program has provided financial assistance to eligible sixty and older, low income and at risk senior residents of Weld County who are in need of services. Potential recipients submit an application and upon approval select the provider of their choice. WCAAA then sends authorization to that provider. Once the recipient's services are complete, the provider will bill WCAAA at which time the maximum allowable amount is paid. If the recipients do not anticipate needing other services, any remaining funds will be reallocated back into the program for other potential applicants. The recipient is responsible for any balance over the approved amount and any service(s) rendered or paid for prior to approval are not covered. According to the Compass Weld County Assessment, affordable health care and the availability of affordable providers is a major concern for aging seniors with difficult choices due to costs. The Dental, Vision, and Hearing Program's goal is to provide services to vulnerable populations before a situation reaches crisis level by providing aid to seniors experiencing difficulty accessing services. By providing assistance prior to a crises, we are supporting recipients in assuming responsibility of their own health by becoming self-sufficient by utilizing preventive care. - 1 - In September 2004, the Colorado Department of Human Services, Division of Aging and Adult Services completed a Strengths and Needs Assessment of Older Adults in the state of Colorado. Based on the information obtained from the study, over 2,500 Weld County adults age sixty or older stated that they needed but could not afford eyeglasses, hearing aids or dentures. Many health insurance plans, such as Medicare and Medicaid, do not even provide the necessary coverage for basic services needed for minimal dental, vision, and hearing needs. As the aging population increases and the baby boomer segment continues to grow; the need for this program and others like it will continue to expand. Essential partnering providers such as Salud Dental Health Clinic, Sunrise Dental Clinic, Catholic Charities Northern, WCAAA staff as well as private providers, continue to express ongoing needs for dental, vision, and hearing services and provide a steady flow of referrals to the program. The need in the community has been severely impacted based on the depletion of current funding prior to the end of the fiscal cycle, which in turn brought about a waiting list. Due to the current state of the economy, we have experienced across the board budget cuts. The current fiscal year, July 2010 through June 2011, received a 40% decrease of funding available to seniors, leaving over 75 clients without access to services on a waiting list. Along with the inability to serve everyone due to lack of funds, the U.S. Census Bureau statistics also suggest that society can continue to anticipate a much greater need as the population increases. According to the Bureau in 2009, the estimated percent of persons 65 years and over in Weld County below poverty level is 11.5%, almost a 4% increase from 2008, and 2.9% above the state's average. The mean income for persons collecting Social Security is $14,730 annually. According to statistics, 16.2% of Weld County residents were uninsured in 2009. This data confirms there is a definite need, now more than ever, for the Dental, Vision, and Hearing Program. Population Served The WCAAA adheres to all Older American Act policies and regulations which include serving a target population of adults sixty years of age and older with special emphasis on those seniors who are in greatest economic and social need such as the low income, minority, frail and rural. We receive referrals from community agencies such as case managers, churches, adult protective services, Catholic Charities, providers and other community technicians. Applicants must reside in Weld County; however, the flexibility of the program allows recipients residing in southern Weld County to receive services from a provider they select that may be near their home in Adams County. The same flexibility applies to recipients living near Boulder, Larimer and even Logan Counties. - 2 - Amount Requested In an effort to meet the growing needs of seniors, Weld County Area Agency on Aging is requesting $10,000 from the North Colorado Medical Center Foundation. Support the Foundation has provided WCAAA in the past several years has assisted nearly 40 Weld County residents. With this award of $10,000, WCAAA would be able to serve approximately 25% of the approved clients currently waiting to receive help. All funds received from NCMC Foundation will be used for direct services for seniors; there will be no administrative costs. These direct services may include, but are not limited to dental extractions, dentures, cleanings, exams, frames, lenses, hearing exams, and/or hearing aids. All other expenses, including personnel and supplies, will be covered by monies received through the Older Coloradan Fund. Committed Funding Sources During this current fiscal year 2010-2011 the Dental, Vision, and Hearing Program receives the majority of its funding from the Older Coloradan Fund ($60,000). In addition to the NCMC Foundation, we are seeking funding assistance from previous contributors Platte Valley Medical Center in Brighton Colorado through their Community Health Investment Program (CHIP) and Medical Center of the Rockies Foundation. The WCAAA staff, senior organizations and individual seniors continue to advocate on a state and national level for improved funding. Weld County Area Agency on Aging will continue to receive Older Coloradan Funds as the primary source of funding. However, with an award from the NCMC Foundation, we will be able to reach more of the target population of Weld County seniors in need. She had been in charge of securing funding for the Dental, Vision and Hearing Assistance Program for the Weld County Area Agency on Aging which assists low income seniors 60 and over who meet the eligibility guidelines. Folks who • otherwise could not pay for these much needed services have an opportunity to get all or part of the cost paid for through this program. As a case manager working with low income seniors in Weld County for the past 7 years I have noticed people struggling to pay for dental work, eye exams and glasses and hearing aids on incomes that typically range from $699 to just over a $1000 a month. If it wasn't for Holly's program many of these folks would have to do without and live with the consequences of poor eyesight, hearing loss, and dental problems in addition to dealing with a myriad of health problems, lack of transportation and social isolation. While there are a few other programs such as Friends of Man and AV Hunter Trust that seniors can apply to for help, it's especially difficult now since the recession began to secure funding. The number of people needing assistance has greatly increased in the last 2 years and funders have less money to work with. Recently our office has been asked by AV Hunter to submit no more than 4 applications per month now, and Friends of Man is requesting a lot more information in addition to their 4 page application before they will consider a request for funding. Seniors have also had to provide proof of a 15-20 year work history before being considered by Friends of Man which eliminates many women who have never worked outside the home or who have only a brief work history. The Greeley/Evans Lions Club has also seen an increase in request for eye exams and glasses and a decrease in funding so they've had to limit their funding to people making less than $731 per month. I haven't been able to submit an application to Dentistry for the Handicapped (Donated Dentistry) for almost 2 years due to an increase in people needing help. The University of Northern Colorado Audiology Clinic will no longer accept f for their hearing aids. Many of our seniors have endured and survived the Great Depression, World War II, and the Korean and Vietnam Wars. They have raised families and are now grandparents and great grandparents. They have held jobs, paid taxes and served their communities and country in many valuable and important ways and they need and deserve our help now. Section 2 The membership on the Weld County Area Agency on Aging Advisory Board shall be appointed by the Weld County Board of Commissioners, subject to the following requirements: a. Members of the Advisory Board shall be comprised fifty percent (50%) of persons age 60 and older. b. Other members may include but not be limited to representation of colleges and universities, elected or public officials, health care providers, senior centers, County Council on Aging, clergy, local business and service clubs, the media, and Nutrition Advisory Council members. c. 1_rs of services, employees, and members of the Boards of Directors receiving Title III funds under the Older Americans Act or planning to - 14- receive Title III funds shall not serve as Advisory Board members to the Area Agency. Section 3 Advisory Board members appointed in accordance with Section 2 shall generally be representative in teens of ethnic origin, sex, income level, geographic areas and population centers of Weld County. • Section 4 Advisory Board members shall be appointed in January and shall simultaneously serve three (3) year terms with eligibility for reappointment permitted. No person shall serve more than two (2) terms. Section 5 In the event of a vacancy or the failure of a member to attend three(3)consecutive regular meetings of the Advisory Board, the Board of County Commissioners shall reappoint a qualified individual pursuant to the requirements set forth in Article III, Section 2. Section 6 • • Each member shall be entitled to one (1) vote. ARTICLE IV The Advisory Board Section 1 The Advisory Board shall elect from its members a Chairperson and Vice- Chairperson. The elected officers shall serve one (1) year terms beginning in February. No officer shall serve more than two (2)consecutive terms. Section 2 The time and place of the Advisory Board meetings shall be determined by the Weld County Area Agency on Aging. Section 3 The Advisory Board shall meet monthly unless there is no other business to transact. - 15 - Section 4 Except as provided herein, a quorum shall be deemed formed when there are present fifty-one percent (51%) of the then presently constituted voting membership. However, in no case shall less than five (5) voting members present constitute a quorum. Section 5 A majority vote of the members present shall be considered a favorable vote upon all motions or resolutions considered a favorable vote upon all motions or resolutions considered by the Advisory Board. Section 6 The Advisory Board will establish committees to provide advice and assistance to the Advisory Board and staff in canying out the objectives of the Area Agency on Aging. At a minimum, two standing committees will be established, a Committee on Nutrition and Senior Centers and a Long Term Care Ombudsman Committee. Other committees may be appointed at the discretion of the Advisory Board. Membership on committees shall be comprised at a minimum of one person from the Area Agency Board who will serve as chairman of the committee and liaison to the board. Other members may be appointed from the board and from the community at large. Section 7 Robert's Rules of Order as revised, shall govern the conduct of the Advisory Board meeting,unless otherwise specified by the Bylaws. ARTICLE V DUTIES OF THE OFFICERS Section 1 The Chairperson shall preside at all meetings of the Area Agency Advisory Board and may call special meetings when needed. Notice to the members of special meetings shall no sooner than 48 hours. - 16 - Section 2 The Vice-Chairperson shall preside and perform the duties of the Chairperson in the Chairperson's absence and shall perform all other duties that may be assigned by the Chairperson. Section 3 . An Executive Committee shall be comprised of the Chairperson, Vice- Chairperson, and subcommittee chairperson. The Chairperson shall appoint any other member(s) from the general membership to serve on the Executive Committee. This Committee shall have the authority to act on those matters requiring immediate decision and action prior to regular Area Agency on Aging Advisory Board meetings. However, the continuing effect of such interim action requires ratification by the Area Agency Advisory Board at the next regular Board meeting. • ARTICLE VI ADVISORY BOARD FUNCTIONS Section 1 The Area Agency Advisory Board shall perform the following functions in an advisory capacity to the Weld County Commissioners: a. Develop and submit annually, to the state agency responsible for approval, an Area Plan on Aging which conforms to federal requirements. b. In addition to the development and administration of an Area Plan, the Advisory Board shall carry out directly, to the maximum extent feasible, the following functions: (I) Provide leadership and advocacy on behalf of all other persons within the county. (2) Determine the need for social services in the region, with special attention being given to the needs of low-income and minority elderly. (3) Inventory the resources available within the region to meet the needs of the elderly and perform an evaluation of the effectiveness - 17 - of the services provided by the public and private agencies within the county in meeting such needs. (4) Establish measurable program objectives and priorities for implementation of the Area Plan in keeping with the objectives established by the State Division of Services for the Aging. (5) Plan with existing agencies and providers of services in the county who are concerned with the needs of the elderly. (6) Either directly or through contract or grant, provide for an action program designed to: Coordinate the delivery of existing services for the elderly. Pool untapped resources of public and private agencies in order to strengthen or initiate new services for older persons. (7) Make periodic evaluation of activities carried out pursuant to the Area Plan, including the views of older persons participating in such activities. (8) Conduct periodic public hearings concerning the needs of the elderly. • (9) Collect and disseminate information concerning the needs of the elderly. (10) Make provision for technical assistance to providers of social services in the county. (11) Where necessary and feasible, enter into arrangements consistent with provisions of the Area Plan under which funds of Title III of the Older Americans Act, as amended, may be used to provide legal services to older persons in the region, carried out through federally-assisted programs or other public or non-profit agencies. (12) Where possible, enter into arrangements with organizations providing day care services for children so as to provide opportunities for older persons to aid or assist on a voluntary or paid basis in the delivery of such services to children. (13) Take into account, matters of general policy arising i the development and administration of the Area Plan, and the views of recipients of services under the Area Plan. - 18- c. Develop a comprehensive and coordinated system for the delivery of social services to the elderly in the county. d. Review and comment on all A-95 type applications forwarded to the Advisory Board by the Regional Clearinghouse, which pertain to programs and planning for the elderly. ARTICLE VII AMENDMENT OF BYLAWS Section 1 These Bylaws may be amended at any regular meeting of the Weld County Area Agency Advisory Board with subsequent approval of the Weld Board of Commissioners. Section 2 • At least twenty-five (25) days notice in writing shall be given of any proposed amendments of these Bylaws.
