RE: APPROVE THREE (3) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with three (3)Amendments to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and the
providers listed below, commencing June 1, 2010, and ending May 31, 2011, with further terms
and conditions being as stated in said amendments, and
1. Jubilee Retreat Center (Life Skills)
2. Collaborative Services for Change, P.C. (Sex Abuse Treatment/Offense Specific
Treatment)
3. Collaborative Services for Change, P.C (Mental Health Services)
WHEREAS,after review, the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the three (3) Amendments to Child Protection Agreements for Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the providers
listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and Various Providers. The following amendments were reviewed by
Pass-Around Memorandum and approved for placement on the Boards Agenda.
These Amendments to the original allocations are necessary to stay within the Core Allocation
for the 2010 program year.
The following amendment was reviewed by Pass-Around Memorandum dated January 14, 2011.
The major provisions of this amended Agreement are as follows:

This Amendment.made and entered into the day of January. 2011. by and between the Board of
County Commissioners, on behalf of the Weld County Department of Human Services. hereinafter referred to
as"Department.- and Jubilee Retreat Center, hereinafter referred to as"Contractor.`
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Life Skills(PY-10-11-CORE-
0125) in order to decrease the maximum reimbursement under the Agreement: and
WHEREAS,required approval. clearance.and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE. in consideration of the premises. the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule, Item 1, is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and services
performed from Core Services funding_not to exceed$90.000.00.
This Amendment. together with the original Child Protection Agreement for Services. constitutes the entire
understanding among the parties with respect to the subject matter hereof,and may not be changed or modified
except as stated in the Child Protection Agreement for Services.

COLLABORATIVE SERVICES FOR CHANGE, PC
This Amendment, made and entered into the day of February 2011,by and between the
Board of County Commissioners, on behalf of the Weld County Department of Human Services,
hereinafter referred to as"Department,"and Collaborative Services for Change,PC, hereinafter referred
to as the "Contractor."
WITNESSETH
WHEREAS, the parties desire to modify their current Agreement for Sex Abuse
Treatment/Offense Specific Treatment (PY-10-11-CORE-0158) in order to increase the maximum
reimbursement under this contract; and
WHEREAS, Human Services has budgeted, appropriated, and otherwise made available; and
WHEREAS, required approval, clearance, and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule, Item 1, is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding, not to exceed$4,500.00.
