Home My WebLink About770552.tiff RESOLUTION RE: APPROVAL OF EQUIPMENT LEASE AGREEMENT BY AND BETWEEN FOUR- PHASE SYSTEMS, INC. AS LESSOR AND THE WELD COUNTY GENERAL HOSPITAL AS LESSEE. WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Weld County General Hospital Administrator and the Weld County General Hospital Director of Fiscal Affairs were present and reviewed for the Board of County Commissioners, the Forty-two Month Lease Agreement No. L62-174 with Four-Phase Systems , Inc. , Lessor, and the Weld County General Hospital as Lessee, for the leasing of computer terminal equipment, and WHEREAS , it was explained to the Board of County Commissioners that the first seven items on said lease are presently leased on a twelve-month basis at $1 , 236 per month. The new Forty-two Month lease at $1, 057 per month saves $179 . 00. Items 8 through 10 are additional items needed immediately at a cost of $356 . 00 per month. The net increase in lease cost then is $177 . 00 . All of the above-described items are more fully set forth in a copy of the Lease Agreement which is attached hereto and incorporated herein by reference, and WHEREAS, it was requested by the Weld County General Hospital that the Board of County Commissioners approve an additional provision to expand the lease at a later date , possibly mid-1978 . The additional equipment would allow for increased efficiency and a smoother operation through remote data entry, and WHEREAS, after discussion of the matter, the Board of County Commissioners did approve the aforementioned Lease Agreement No. L62-174 , but felt that the Weld County General Hospital should make a separate request for additional equipment at the time it was needed. NOW, THEREFORE, BE IT RESOLVED by the Board of County Com- missioners of Weld County, Colorado, that the Lease Agreement with Four-Phase Systems , Inc. , Lessor and the Weld County General H S Q4 770 u5 Hospital, Lessee, as aforementioned, be, and hereby is , approved. BE IT FURTHER RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the additional request from the Weld County General Hospital regarding the provision for ex- pansion of the lease to include additional equipment, be pre- sented to the Board at the time said equipment is actually needed.
