RE: APPROVE THIRTEEN (13) AMENDMENTS TO CHILD PROTECTION AGREEMENTS
FOR SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with thirteen (13) Amendments to Child
Protection Agreements for Services between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and the providers listed within Exhibit"A", commencing June 1, 2010, and ending May 31,
2011, with further terms and conditions being as stated in said amendments, and
WHEREAS, after review, the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the thirteen (13)Amendments to Child Protection Agreements for Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the providers
listed within Exhibit "A", be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 1st day of August, A.D., 2011, nunc pro tunc June 1, 2010.

Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and Various Providers. These amendments were presented at the
Board's March 23, 2011, or May 16, 2011, Work Session.
These Amendments constitute the final accounting of all increases to providers under the Core
Services Program of 2010-2011.

BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
CROSSROADS COUNSELING,LLC
This Amendment,made and entered into the day of June 2011,by and between the Board of
County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter referred
to as"Department,"and Crossroads Counseling, LLC,hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Sex Abuse
Treatment/Offense Specific Treatment(PY-10-1 I-CORE-0209)in order to increase the maximum
reimbursement under this contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule,Item 1,is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$2,310.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.

The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$78,000.00.
