Amendment to General Assistance Agreement for Services between the Weld County Housing
Authority and Greeley Transitional House Contract No: FY10-GA-600 entered into on the 13th
day of September 2010
Exhibit B Payment Schedule paragraph 1 is amended to read:
Housing Authority agrees to reimburse to the Contractor, in consideration for the work and
service performed a total amount not to exceed Forty one thousand four hundred thirty eight
dollars, ($41,438) through the Weld County General Assistance Program. The Housing
Authority will reimburse the contractors as follows.
All other provisions and requirements of the contract remain unchanged.

CRE: Human Services CSBG Allocation to Homeless Shelters
COLORADO
Human Services receives Community Services Block Grant funds and has provided$60,000 annually to help
support the homeless shelters in Weld County. The current Grant period is from Mar 2010 to Feb 2011.
Contracts were executed between the Housing Authority and A Woman's Place,Greeley Transitional House
and Catholic Charities. In February I was contacted by John Kruse and informed that the CSBG grant was
going to be significantly reduced for the 2011-2012 grant year. He indicated that they would most likely not
be able to support the shelters in the 2011-2012 grant year. They did however have$45,000 dollars they
could add to the Grant for the current year if the shelters could provide documentation. The shelters were
contacted and they all indicated they could provide back up for additional reimbursement of salaries. The
$45,000 was received by the Housing Authority prior to Feb 28,2011. The documentation has been received
from the Shelters. The current contracts with the Shelters need to be modified to include the increased
funding.
Catholic Charities FY 10-GA-400 from$14,560 to$29,560
Greeley Transitional House FYI0-GA-600 from$26,438 to$41,438
A Woman's Place FY10 GA-500 from$19,002 to$34,002
This modification can be accomplished by amendment to Exhibit B Payment Schedule in each contract
