Included with this submittal, required proofs of publication, notification and
notification/agreements with owners of structures, and proof of placement with Weld
County.
Also included is a revised Exhibit C - Surface Owners of Record, to reflect the correct
name and address of Qwest Communications, now Century Link. Please note that the
first notice went to a bad address. The second attempt followed the address from a
recent but unrelated submittal on a return green card as shown in the submittal copy. A
green card did not make it back for the second attempt, but the certified mailing shows
the second attempt was properly labeled and sent. It is not uncommon according to the
U.S. Postal Service for green card returns to become lost in transit. We believe all
necessary notifications have been made as provided under this correspondence.
This should satisfy this part of the Amendment requirement. Please contact my office,
above, should you need clarification or additional information.

Exhibit C — Pre-Mining & Mining Plan Map
6.4.3 EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands
One or more maps may be necessary to legibly portray the following
information:
(a) all adjoining surface owners of record;
(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines,
and power and communication lines on the area of affected land and within two
hundred (200) feet of all boundaries of such area;
(c) the existing topography of the area with contour lines of sufficient detail to
portray the direction and rate of slope of the affected land;
(d) the total area to be involved in the operation, including the area to be mined and
the area of affected lands (see definition of "Affected Land");
(e) the type of present vegetation covering the affected lands; and
(f) in conjunction with Exhibit G - Water Information, Subsection 6.4.7, if required by
the Office, further water resources information will be presented on a map in this
section.
(g) Show the owner's name, type of structures, and location of all significant,
valuable, and permanent man-made structures contained on the area of affected
land and within two hundred (200) feet of the affected land.
(h) In conjunction with Exhibit I - Soils Information, Subsection 6.4.9, soils
information may be presented on a map in this section;
(i) Aerial photos, if available, may be included in this section.

Greeley,Tribune for four(4)consecutive weeks,commencing Friday 19 August 2011,as
shown below.
PUBLIC NOTICE
Varra Companies, Inc.: 8120 Gage Street; Frederick, Colorado 80516; (303) 666-6657,has filed
an Amendment application for a Regular Impact(112) Construction Materials Operation
Reclamation Permit with the Colorado Mined Land Reclamation Board under provisions of the
Colorado Land Reclamation Act for the Extraction of Construction Materials. The proposed
mine is known as the Heintzelman Project (OMLR Permit#: M-2009-018)and is located at or
near the E/2,E/2, Section 32; and SW/4,SW/4, Section 28; all in Township 3 North, Range 67
West; 6th P.M.;Weld County, Colorado.
The proposed date of commencement is 31 October 2011, and the proposed date of completion is
1 January 2037. The proposed future use of the Amended land is Agricultural. Additional
Information and tentative decision date may be obtained from the Division of Reclamation,
Mining, and Safety: 1313 Sherman Street, Room 215; Denver, Colorado 80203; (303) 866-3567,
or at the Weld County Clerk to the Board; 915 Tenth Street; Greeley, Colorado 80631, or the
above named applicant.
Comments must be in writing and must be received by the Division of Reclamation, Mining, and

You have received this notice because we believe you are the owner of one or more significant, valuable and
permanent man-made structure(s)located within two hundred(200)feet of the planned extraction and related
affected land. A qualified man-made structure may include a fence,a residence or out building,a road,utility,or
other significant,valuable and permanent man-made structure.
As the owner of a qualified man-made structure located within 200 feet of our planned extraction activity, the
Colorado Division of Reclamation,Mining and Safety requests we provide evidence of our efforts to protect
such structures from injury during the planned extraction activities; and to assure you that damage caused
to your structures as a direct result of our active operations will be compensated for, consistent
with established law.
Also included is a map showing the approximate area of land affected under our planned Amended activities,
our Amended permit area and the relationship to your property. A line showing the approximate extent 200
feet from our permit boundary is shown for your reference.
