r CONDITIONS OF APPROVAL
1) This well shall be used in such a way as to cause no material injury to Sating water rights. The issuance of this permit
does not ensure that no injury will occur to another vested water right or preclude another owner of a vested water right from
seeking relef in a civil caul action.
2) The construction of this well shall be In'compliance with the Water Weil Construction Rules 2 CCR 402-2,unless approval
of a variance has been granted by the State Board of Examiners of Water Well Construction end Pump installation
Contractors In accordance with Rule 18.
3) Approved pursuant to CRS 37-80.107 end the Findings of the Groundwater Commission dated hot c- aft 2010
4) The use of water under this permit Is limited to commercial use in Weld County via truck hauling.
• 8) The pumping rate of this well shall not exceed 200 GPM.
8) The annual withdrawal of ground water from this well shall not exceed 322.8 ace-Mel
7) The total depth of the well shell not exceed 130 feet,which corresponds to the base of the White River aquifer. At a
minimum,plain casing shall be installed and grouted through an unconsolidated materials and shall extend a mkrimurn of
ten feet into the bedrock formation to prevent production from other zones.
8) The owner shall mark the well in a conspicuous place with the well permit number end name of aquifer as appropriate,
and shall take necessary means and precautions to preserve these markings.
9) A bteihdng flow meter or Commission approved measuring device must be installed on this wel and maintained In goof
working order. Permanent records of all diversions must be maintained by the well owner(collected at least annually)and
sutmilted tO the Ground Water Commission upon request
10) This well shall be constructed within 200 feet of the location specified on this penntL •
11) The use of ester from the well under this penult must have occurred,and a completed Statement of Beneficial Use
(Including the pumping rate and annual appropriation)must be Bed by the well owner,within three years of the date of
issuance of this permit.

CONDITIONS OF APPROVAL

11) The use of water from the well under this permit must have occurred,and a completed Statement of Beneficial Use
(including the pumping rate and annual appropdaion)must be lied by the wall owner,within three yeas of the date of
issuance of this permit.
