SUBJECT: Addendum to USR-1769 Conquest Injection Well C-8 Resolution
Prior to operation the Property Owner shall be responsible for the construction of certain on-site and off-site
safety improvements which shall include:
• Construction of adequate turning radii sixty (60) feet, drainage and signage installations at the main
entrance and exit onto WCR 77, as indicated on the accepted Construction Plans.
• Construction of a right turn deceleration lane with tapers on WCR 77 prior to commencement of
operations at the site
• Construction of a left turn deceleration lane with tapers on WCR 77 prior to commencement of
operations at the site
• • The applicant, or its successor, shall construct a right acceleration lane on WCR 77 when the trigger of
50 vehicles per hour turn right out of the facility is met.

SUBJECT: USR-1769 Final Drainage Report Comments
The Weld County Public Works Department received a revised Final Drainage Report on
January 21, 2011. The report was signed, dated, and stamped by Eric Wernsman, P.E. License
33371.
Public Works would like to thank Mr. Wernsman for a concise drainage report. It appears that
all calculations have been completed and most of the requested information has been supplied.
Please see the comments below and the redline comments in the drainage report for items to
be addressed prior to acceptance of the Final Drainage Report.
This memo relates only to the Final Drainage Report. For other comments that are outstanding,
please refer to the January 19, 2011 memo.
Preliminary Drainage:
• 1. Please address the redline comments on the drainage report and drawings that were
returned to the applicant's engineer.
2. Please provide 2 copies of the Final Drainage Report and associated exhibits that have
been stamped, signed, and dated by a registered professional engineer licensed in the
State of Colorado.
3. Please include a map that shows the historic drainage basin boundary in the appendix of
the report.
Onsite and Road Construction Drawings
The applicant has provided construction drawings for both the on-site and off-site
improvements. The drawings have not been stamped, signed, and dated by a registered
professional engineer licensed in the State of Colorado.
1. Upon acceptance of the construction drawings, please provide 3 sets of drawings (2 24 x
36 and 1 11x17) that have been signed, stamped, and dated by a registered
professional engineer licensed in the State of Colorado.
2. Please address the redline comments provided on the construction drawings that have
been returned to the applicant's engineer.
3. In order to prevent tracking of gravel onto the County Road, please show a set of double
cattleguards extending across the access point into the site. The cattleguards will be
especially important in controlling debris on the County Road if the site is to undergo
early operations similar to those at the C-7 site.
