RE: APPROVE TWO (2) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES AND AUTHORIZE CHAIR TO SIGN-TURNING POINT CENTER FOR YOUTH
AND FAMILY DEVELOPMENT, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two (2) Amendments to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and
Turning Point Center for Youth and Family Development, Inc., commencing June 1, 2010, and
ending May 31, 2011, with further terms and conditions being as stated in said amendments, and
WHEREAS, after review,the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the two (2) Amendments to Child Protection Agreements for Services
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and Turning Point
Center for Youth and Family Development, Inc., be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 18th day of May, A.D., 2011, nunc pro tunc June 1, 2010.

RE: Amendments to Child Protection Agreements for Services
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and Various Providers. These amendments were reviewed under the
Board's Pass-Around Memorandum dated December 13, 2010, and approved for placement on
the Board's Agenda.
These Amendments to the original allocations are necessary to stay within the Core Allocation
for the 2010 program year. The major provisions of these amended Agreements are as follows:

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES �If
TURNING POINT CENTER FOR YOUTH AND FAMILY DEVELOPMENT,I .APR 20 q 04
This Amendment,made and entered into the day of November 2010,by and between the
Board of County Commissioners,on behalf of the Weld County Department of Human Services,
hereinafter referred to as"Department,"and Turning Point Center for Youth and Family Development,
Inc.,hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Foster Parent Consultation
(PY-10-11-CORE-0119)in order to increase the maximum reimbursement under this contract; and
WHEREAS, Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule,Item 1,is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$30,000.00.
This Amendment,together with the original Child Protection Agreement for Services, constitutes
the entire understanding among the parties with respect to the subject matter hereof, and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day,
month,and year first above written.
