RE: Notice of Injury
Dear Sirs and Madams:
The following notice of injury and loss is given pursuant to the requirements of C.R.S.
§24-10-109.

2. On March 6, 2011, at approximately 10:00 p.m., in Weld County, State of Colorado, Mr.
Bamford was a passenger in an automobile driving on Weld County Rd. 112
approximately 273 feet south of Weld County Rd. 107. At that place and time, said
automobile spun then rolled one and a half times off of the roadway. Mr. Bamford was
ejected from the vehicle and severely injured, as described below, as a result.

3. The names and addresses of the Weld County employees and agents involved in the
incident are unknown at this time.
4. The extent of the injuries suffered by Mr. Bamford are not yet fully known, however, his
injuries include, without limitation, multiple blunt traumas including facial fractures,
right orbital wall fracture with resulting eye injury, rib fractures, fractures of several
vertebrae, a left hand fracture and right foot fracture. Mr. Bamford was hospitalized for
three weeks following the accident, first at North Colorado Medical Center in Greeley,
Colorado and then at Presbyterian/St. Luke's Hospital (Spalding Unit) in Denver. Since
he discharge, Mr. Bamford has been recovering at home and undergoing physical
therapy. Mr. Bamford has not yet returned to work and when he does so it will be with
significant physical restrictions. The extent and nature of any permanent injuries and
necessary surgeries are unknown at this time.
5. Mr. Bamford's known damages at this time include, without limitation, over $100,000.00
in medical bills incurred and paid to date; costs of surgeries and treatments pursued
currently and into the future; lost wages in excess of$45,000.00 date and continuing into
the future; pain and suffering both past and future; and significant physiological
deficiencies both past and future. The extent of all damages that will be incurred into the
future are not known at this time. Estimated amounts will be updated as the information
becomes available.
