Home My WebLink About20110295.tiff RESOLUTION RE: APPROVE NINE (9) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with nine (9) Amendments to Child Protection Agreements for Services between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and the providers listed below, commencing June 1, 2010, and ending May 31, 2011, with further terms and conditions being as stated in said amendments, and 1. Reflections for Youth, Inc. (Day Treatment Services) 2. Greeley Counseling Center, P.C. (Mental Health Services) 3. Lutheran Family Services of Colorado (Life Skills— Home-Based Parent Coach) 4. Lutheran Family Services of Colorado (Home-Based Pre-Adoption Services) 5. Lutheran Family Services of Colorado (Home-Based Services— Option B) 6. Transitions Psychology Group, LLC (Life Skills) 7. Child Advocacy Resource and Education, Inc. (CARE) (Life Skills -Visitation) 8. Lutheran Family Services of Colorado (Life Skills—Therapeutic Visitation) 9. Transitions Psychology Group, LLC (Mental Health Services) WHEREAS, after review,the Board deems it advisable to approve said amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the nine (9) Amendments to Child Protection Agreements for Services between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services,and the providers listed above be, and hereby are, approved.
