RE: APPROVE ADDENDUM TO CHRONIC DISEASE SELF MANAGEMENT PROGRAM
MASTER TRAINER AGREEMENT AND AUTHORIZE CHAIR TO SIGN - CENTRAL
COLORADO AREA HEALTH EDUCATION CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Addendum to the Chronic Disease Self
Management Program Master Trainer Agreement between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the Central Colorado Area Health Education Center,
commencing October 20, 2011, and ending March 31, 2012,with further terms and conditions being
as stated in said addendum, and
WHEREAS, after review, the Board deems it advisable to approve addendum to said
agreement, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Addendum to the Chronic Disease Self Management Program Master
Trainer Agreement between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging, and the Central Colorado Area Health Education Center, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said addendum
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 28th day of November, A.D., 2011, nunc pro tunc October 20, 2011.

RE: Addendum to the Host Site Agreement between the Weld
Weld County Department of Human Services' Area Agency
on Aging and the Central Colorado Area Health Education
Center
Enclosed for Board approval is an Addendum to the Host Site Agreement between the Weld
County Department of Human Services' Area Agency on Aging and the Central Colorado Area
Health Education Center. This Addendum was reviewed under the Board's Pass-Around
Memorandum dated November 17, 2011, and approved for placement on the Board's Agenda.
The Addendum more accurately reflects the number of clients that will be participating in wellness
classes that we are providing, as it relates to our agreement with (CC AHEC) and the ARRA
evidence program grant.
We will be serving 40 more clients than previously anticipated for a total of 75. The Department
will be reimbursed $110.00 for each confirmed participant completer. The term of this Agreement
will be from the signing of the agreement until March 2012.

Host Site agreement Be Well Colorado Workshops - Addendum
This memorandum of agreement is entered into this 20th day of October, 2011 by Central Colorado
Area Health Education Center(CC AHEC), a non-profit corporation incorporated in the state of
Colorado with a principal business address of 12631 E. 17th Ave, MS F496, Aurora, Colorado 80045

Whereas, the CC AHEC is the Implementation agency for Colorado's Chronic Disease Self
Management Program-Be Well Colorado workshops and
Whereas, WCAAA desires to participate as a Host Site for this initiative;
Thus, through our mutual premises and the mutual undertakings herein contained, the parties agree:
To implement the Be Well Colorado workshops, Chronic Disease Self Management Program
(CDSMP) under a grant from the Administration on Aging and American Reinvestment and
Recovery Act(ARRA) to the Colorado Department of Public Health and Environment to expand
access to Stanford's CDSMP. The program's goal is to empower older people to take more control
of their health and reduce their risk for disease and disability.
WCAAA has been selected to become a Host Site for Be Well Colorado workshops.
Host Site Agency responsibilities include those listed in the original signed MOU, as well as the
following addendum:
a) Pay Host Site $110.00 for each confirmed participant completer, defined as finishing 4 of
6 workshops of a fidelity authenticated CDSMP series, for a total of 40 participant
completers. These are additional participant completers from original signed MOU.
The Host Site Agency, WCAAA, in performing the services as specified in this service agreement,
shall act as an independent contractor. Any and all employees of WCAAA shall be considered to be,
at all times, employees of WCAAA, under its sole authority.
Further, WCAAA shall indemnify and hold CC AHEC harmless from any and all claims, liabilities,
losses and causes of action which may arise from acts, errors or omissions of WCAAA or any of its
employees or agents in conjunction with its obligations under this service agreement.

Responsibilities of CCAHEC remain as defined in the original signed MOU.
The terms of this agreement will be effective as of the date of signing and shall be effective until
March 2012.
This agreement for services is contingent upon the receipt of adequate revenue by Grantee identified
in Section 1.
