RE: APPROVE TWENTY-SEVEN (27) AMENDMENTS TO CHILD PROTECTION
AGREEMENTS FOR SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR
TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with twenty-seven (27) Amendments to Child
Protection Agreements for Services between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and the providers listed within Exhibit"A", commencing June 1, 2010, and ending May 31,
2011, with further terms and conditions being as stated in said amendments, and
WHEREAS, after review,the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the twenty-seven (27) Amendments to Child Protection Agreements for
Services between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the providers
listed within Exhibit "A", be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 29th day of June, A.D., 2011, nunc pro tunc June 1, 2010.

RE: Amendments to Child Protection Agreements for Services
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and Various Providers. These amendments were presented at either the
Board's March 23, 2011, or May 16, 2011, Work Session.
These Amendments to the original allocations are necessary to reimburse providers for services
performed or to be performed in the current SFY 10-11 Core Program Year. Previous contract
amendments during this Program Year have involved only a shifting of funds from one contract
to another, maintaining the same grand total of contracts. These amendments represent an
absolute increase to the total Core Services contracts for Program Year 2010-11 of$285,975.
This overall increase has been necessitated by increased demand for services rendered by these
providers. The major provisions of these amended Agreements are as follows:

This Amendment made and entered into the day of March,2011, by and between the Board of
County Commissioners, on behalf of the Weld County Department of Human Services,hereinafter referred to
as"Department" and Alternative Homes for Youth, hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS, the parties desire to modify their current Agreement for Day Treatment (PY-10-11-
CORE-0111) in order to decrease the maximum reimbursement under this contract; and
WHEREAS, required approval, clearance, and coordinator have been accomplished from and with
appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule, Item 1, is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the worker and services
performed from Core Services funding, not to exceed $0.00.
This Amendment,together with the original Child Protection Agreement for Services, constitutes the entire
understanding among the parties with respect to the subject matter hereof, and may not be changed or modified
except as stated in the Child Protection Agreement for Services.
