RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES AND
AUTHORIZE CHAIR TO SIGN-KEMPE CENTER OF THE UNIVERSITY OF COLORADO
DENVER SCHOOL OF MEDICINE, AMC CAMPUS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services,and the
Kempe Center of the University of Colorado Denver School of Medicine, AMC Campus,
commencing June 1, 2010, and ending May 31, 2011, with further terms and conditions being as
stated in said amendment, and
WHEREAS,after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to the Child Protection Agreement for Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and the Kempe Center of the University of
Colorado Denver School of Medicine, AMC Campus, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 5th day of January, A.D., 2011, nunc pro tunc June 1, 2010.

COLORADO RE: Amendment to Child Protection Agreement for Services
between the Weld County Department of Human Services and
the Kempe Center of the University of Colorado Denver
School of Medicine
Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and the Kempe Center of the University of Colorado Denver School
of Medicine. This amended Agreement was presented at the Board's October 25, 2010,
Work Session.
This Amendment is necessary to stay within the Core Allocation. The major provisions of this
amended Agreement are as follows:

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
THE KEMPE CENTER OF THE UNIVERSITY OF COLORADO
DENVER SCHOOL OF MEDICINE,AMC CAMPUS
This Amendment,made and entered into the day of October 2010,by and between the Board
of County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter
referred to as"Department,"and The Kempe Center of the University of Colorado Denver School of
Medicine,AMC Campus,hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Foster Parent Consultation
(PY-10-1 1-CORE-0200)in order to increase the maximum reimbursement under this contract;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as
follow:
1. Exhibit B,Payment Schedule, Item 1,is hereby revised to include the following:
The Department agrees to reimburse CONTRACTOR in consideration for the work and
services performed from Core Services funding,not to exceed$6,000.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF,the part' a to have duly executed the Agreement as of the day,
month,and year first above written.
