RE: APPROVE AMENDMENT TO 2010-2011 PURCHASE OF SERVICES AGREEMENT
FOR BUILDING HEALTHY MARRIAGES PROGRAM AND AUTHORIZE CHAIR TO
SIGN -COMMUNITY MEDIATION PROJECT, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the 2010-2011
Purchase of Services Agreement for the Building Healthy Marriages Program between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and the Community Mediation Project,
Inc., commencing October 1, 2010, and ending September 30, 2011, with further terms and
conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the 2010-2011 Purchase of Services
Agreement for the Building Healthy Marriages Program between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, and Community Mediation Project, Inc., be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of January, A.D., 2011, nunc pro tunc October 1, 2010.

For Services between the Weld County Department of
Human Services' and Community Mediation Project
Enclosed for Board approval is an amendment to the Purchase of Services Agreement for
Services between the Department and Community Mediation Project. This Amendment was
presented at the Board's January 24, 2011, Work Session.
This Amendment is to the 2011 Agreement covering the period of October 1, 2010 through
September 30, 2011. Due to the reallocation of funds as explained in the Budget Narrative
Amendment and Amended Budget, Community Mediation Project's agreement is to be amended
by $34,774 to reimburse for additional salaries and programming.

PURCHASE OF SERVICES AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT
OF HUMAN SERVICE AND COMMUNITY MEDIATION PROJECT

This AMENDMENT to Purchase of Services Agreement for Services made and entered into this 0!O -clay
of-fan 2011, by and between the Board of County Commissioners, on behalf of the Weld County
Department of Human Services' recipients of the Healthy Marriages Demonstration Grant, hereinafter
referred to as "Human Services," and Craig Conrad on behalf of Community Mediation Project, Inc,
hereinafter referred to as"CMP."
WITNESSETH
WHEREAS, Human Services and CMP entered into a Purchase of Services Agreement for Services
on October 2010 to provide services to individuals participating in the Building Healthy Marriages
Program (BHM).
WHEREAS,the monies provided under the agreement is not sufficient for the term of the Agreement to
serve the educational of Building Healthy Marriage participants.
WHEREAS, Human Services desires to adjust its payment schedule to respond to the increased number
of individuals participating in BHM.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows:
1. Exhibit A, Payment Schedule, Funding and Method of Payment, shall be amended as follows:
Human Services agrees to reimburse CMP in consideration for the work and services performed, a total
amount not to exceed Three hundred ninety four thousand,fifty four ($394,054.00) dollars. This is an
increase to the original budgeted amount by$ 34,774. This will be funded by the Healthy Marriages
Demonstration Federal Grant budget for year 5 (2010-2011).
2. CMP agrees to employ a person to assist the Intake Unit in referral management due to loss of
staff. This employee will work with the Intake Unit for 30 hours/week until the end of the grant year
(September 30, 2011) at an hourly rate of$14.00/hour for a total salary of$14,700. CMP also agrees to
increase the FTE of the Marriage Education Coordinator from .75 FTE to 1.0 FTE until the end of the
grant year at an hourly rate of$20.47 for a total of$7,574. The additional 10 hours will be utilized to
assist the Intake Unit in referral management. CMP agrees to provide supervision to these employees;
while Human Services will provide office space, training, and guidance at the Building Healthy Marriages
office.This amount is included in the above mentioned payment schedule.

3. CMP agrees to schedule additional classes and serve additional 30 couples in the Marriage
Education Seminars and an additional 10 couples in the Relationship Inventories. For these services,
Human Services agrees to pay CMP $12,500. This amount is included in the above mentioned payment
schedule.
4. This Amendment,together with the original Purchase of Services Agreement for Services signed
in 2009,constitutes the entire understanding among the parties with respect to the subject matter
hereof,and may not be changed or modified except as stated in the Purchase of Services Agreement.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day, month, and
year first above written.
