other issues may arise during the remaining application process.

The Colorado Department of Transportation (CDOT) has jurisdiction over all accesses to the state
highways. Please contact Gloria Hice-Idler at the Greeley office (970-350-2148 or 970-350-2163) to
verify the access permit or for any additional requirements that may be needed to obtain or upgrade the

Traffic proposed is very minor. The existing paved access to the property will be used for the business.
There is no outdoor storage of materials proposed therefore a water quality area is not required.
• This area IS NOT in a Special Flood Hazard Area (SFHA) as determined by the Federal Emergency
Management Agency (FEMA).
REQUIREMENTS:

applicant/landowner shall be responsible for controlling the noxious weeds. All
vegetation, other than grasses, needs to be maintained at a maximum height of 12
inches until the area is completely developed.
b. The historical flow patterns and run-off amounts will be maintained on site in such a
manner that it will reasonably preserve the natural character of the area and prevent

MEMORANDUM
i h TO: KIM OGLE, PLANNING SERVICES
0 0 0 N T Y FROM: LAUREN LIGHT, ENVIRONMENTAL HEALTH
SUBJECT: USR12-0010 WHITTERN
DATE: MARCH 26, 2012
Environmental Health Services has reviewed this proposal for a dental office. There is an existing
residence on the property which will be remodeled to contain the dental clinic. The number of
employees was not specified in the application. There will be a maximum of 6 dental patients per day
according to the application.
There is an existing three bedroom home on the property that is served by an individual sewage
disposal system (Permit No. SP-0701028). According to an engineer evaluation from
AGPROfessionals, dated November 28, 2011, the existing septic system is adequately sized to
accommodate 6 dental patients per day. The evaluation does stipulate if the use is expanded an
updated analysis is required.
Water will be supplied by Longs Peak Water District. The District has indicated that a backflow
evention device must be installed to prevent contamination from the proposed clinic to the
esidence.
A waste handling plan was submitted and indicates that Western Disposal will be utilized for office
waste disposal. Stericycle will dispose of medical waste if needed.
The dust abatement plan states that the driveway is asphalt and concrete, traffic to the property will
be minimal and water will be utilized if necessary.
We have no objections to the proposal; however, we recommend that the following requirements be
incorporated into the permit as development standards:
1. All liquid and solid wastes (as defined in the Solid Wastes Disposal Sites and Facilities Act,
30-20-100.5, C.R.S., as amended) shall be stored and removed for final disposal in a manner
that protects against surface and groundwater contamination.
2. No permanent disposal of wastes shall be permitted at this site. This is not meant to include
those wastes specifically excluded from the definition of a solid waste in the Solid Wastes
Disposal Sites and Facilities Act, 30-20-100.5, C.R.S., as amended.
3. Waste materials shall be handled, stored, and disposed in a manner that controls fugitive dust,
fugitive particulate emissions, blowing debris, and other potential nuisance conditions.
•
4. The applicant shall operate in accordance with the approved "waste handling plan", at all times.
• 5. Fugitive dust and fugitive particulate emissions shall be controlled on this site. The facility shall
be operated in accordance with the approved "dust abatement plan", at all times.
6. This facility shall adhere to the maximum permissible noise levels allowed in the Non-Specified
Zone as delineated in Section 14-9-30 of the Weld County Code.
7. Adequate drinking, hand washing and toilet facilities shall be provided for clients and
employees of the facility, at all times.
8. Any septic system located on the property must comply with all provisions of the Weld County
Code, pertaining to Individual Sewage Disposal Systems.
9. Any additional hydraulic load to an existing septic system will require an Evaluation from a
Colorado Registered Professional Engineer. The engineer's evaluation shall be submitted to
the Weld County Environmental Health Department. In the event the system is found to be
inadequate, the system must be brought into compliance with current Weld County Individual
Sewage Disposal Regulations.
10.A permanent, adequate water supply shall be provided for drinking and sanitary purposes
(Longs Peak Water District).
11.All potentially hazardous chemicals must be handled in a safe manner in accordance with
• product labeling and in a manner that minimizes the release of hazardous air pollutants (HAP's)
and volatile organic compounds (VOC's). All chemicals must be stored secure, on an
impervious surface, and in accordance with manufacturer's recommendations.
12.The operation shall comply with all applicable rules and regulations of State and Federal
agencies and the Weld County Code.

Re: Water Service
Dear Ms. Whittern;
You asked that we provide you with information regarding the water service to your
residence(13876 Elmore Road)as part of the requirements to open a dental office in your house. The
following information is pertinent to your request:
I. Enclosed is our most recent"Consumer Confidence Report"which details all quality
aspects of the water we provide. In reviewing this document you will find that the
r • water provided to your residence is of the highest quality in the area.
2. You are provided water service via one standard 5/8" X 3/4" meter service. The
quantity you can expect is up to 25 gpm, with a static pressure of about 80 psi. This
should provide a more than satisfactory level of service (quantity/flow) to support
your proposed dental office.
You are required to have installed and maintain a backflow prevention device (in this case
a device called a"Reduced Pressure Zone Backflow Prevention Device" - RPZ) in your plumbing
system so as to prevent contamination of your house system from the dental office.Additionally, the
device must be tested annually by a certified tester with results furnished to Longs Peak.
1 hope this provides you with the information you requested. Should you have questions or
need additional information, please contact our office.
Best Regards.

Dear Mr. Ogle:
I have reviewed the submitted material pertaining to a dental office located within a residence at
13876 Elmore Road (Case Name: Whitten, Case Number: USR12-0010). The Fire District does
not object to the request, provided the requirements of the Fire District can be met: Based on my

If the applicant uses any oxidizing gases in excess of 504 cubic feet additional review will be
needed. When oxidizing gases are used in excess of 504 cubic feet the must be located in a 1-
