RECORD OF PROCEEDINGS MINUTES BOARD OF COUNTY COMMISSIONERS WELD COUNTY, COLORADO NOVEMBER 14, 2012 The Board of County Commissioners of Weld County, Colorado, met in regular session in full conformity with the laws of the State of Colorado at the regular place of meeting in the Weld County Administration Building, Greeley, Colorado, November 14, 2012, at the hour of 9:00 a.m. e ROLL CALL: The meeting was called to order by the Chair and on roil call the following members were present, constituting a quorum of the members thereof: Commissioner Sean P. Commissioner Kirkmeyer seconded the motion, and it carried unanimously. El READ ORDINANCE BY TAPE: Commissioner Garcia moved to read Code Ordinance #2012-13 by tape. Commissioner Kirkmeyer seconded the motion, which carried unanimously. El CERTIFICATION OF HEARINGS: Commissioner Kirkmeyer moved to approve the Certification of Hearings conducted on November 7, 2012, as follows: 1) USR120056 — Leonard and Tammie Ball, Go Noble Energy, 2) USR120057 - Steven and Teresa Wells/Wells Ranch, 3) COZ120001 — Butterball, LLC, and 4) USR120039 - Prospect Valley Grain, LLC. Commissioner Long seconded the motion, which carried unanimously. El AMENDMENTS TO AGENDA: Commissioner Garcia moved to add the following under Bids and renumber: 7) Approve Bid #61200150, Banking Services — Treasurer's Office (con't from 11/07/2012) [2012-3037]. Commissioner Kirkmeyer seconded the motion, which carried unanimously. Fl PUBLIC INPUT: Kelly King, home child care provider, discussed the Child Care Assistance Program (CCAP) rates, which are set by the Board of Commissioners, and expressed concern with the program which does not equally reimburse home daycares, versus child care centers. Commissioners Long and Kirkmeyer commended Ms. King for her quality certifications and rating and asked her to leave the documents, marked Exhibit A, with the Board for further review. \-17 Minutes, November 14, 2012 2012-3264 Page 1 BC0016 Fl CONSENT AGENDA: Commissioner Long moved to approve the Consent Agenda as printed. Commissioner Garcia seconded the motion, and it carried unanimously. PROCLAMATIONS: El 1) WELD COUNTY ADOPTION DAY — NOVEMBER 16, 2012, AND NATIONAL ADOPTION DAY - NOVEMBER 17, 2012: Various Human Services case workers (Brandi Puckett, Desi Flores, Lisa Travis, Heather Walker, Gabrielle Brighton) were present to accept the Proclamation, which Chair Conway read for the record. Commissioners Kirkmeyer and Long and Chair Conway commended the employees within the Child Welfare Services whose work results in long-term benefits for the children, families and society in general. Ms. Walker recognized the work of the adoption workers who strive to find the best matches for the children. Ms. Flores thanked the Board for their support of out-of-state placements, in addition to the local placements. Fl COMMISSIONER COORDINATOR REPORTS: Commissioner Garcia discussed the community meeting held at the Spanish Colony and commended the various staff working on the improvement project. BIDS: Fl 1) APPROVE BID #B1200129, CR 49 AND CR 44 INTERSECTION DESIGN - DEPARTMENT OF PUBLIC WORKS: Monica Mika, Director of Finance and Administration, introduced the matter. Wayne Howard, Department of Public Works, explained this is a Quality Based Services (QBS) proposal and he requested a continuance to allow for further review of the submittals. Commissioner Kirkmeyer moved to continue the matter to December 5, 2012. Commissioner Garcia seconded the motion, which carried unanimously. El 2) APPROVE BID #B1200135, 3/4 TON 4X4 CREW CAB PICKUP WITH UTILITY BOX-(BI- FUEL COMPRESSED NATURAL GAS-CNG)-TURNKEY -- DEPARTMENT OF PUBLIC WORKS/MAINTENANCE SUPPORT DIVISION: Ms. Mika requested this duplicated item be withdrawn. Commissioner Garcia moved to accept the withdraw request. Commissioner Kirkmeyer seconded the motion, which carried unanimously. Fl 3) APPROVE BID #B1200136, 3/4 TON 4X4 EXTENDED CAB PICKUP-(BI-FUEL COMPRESSED NATURAL GAS-CNG)-TURNKEY - DEPARTMENT OF PUBLIC WORKS/GRADER DIVISION: Ms. Mika reviewed staff's recommendation. Commissioner Kirkmeyer moved to approve the low bid from Spradley Barr Ford of Greeley, as recommended by staff. The motion was seconded by Commissioner Garcia, which carried unanimously. Fl 4) APPROVE BID #B1200137, 1/2 TON 4X4 EXTENDED CAB PICKUP-(BI-FUEL COMPRESSED NATURAL GAS-CNG)-TURNKEY - DEPARTMENT OF PUBLIC WORKS/TRUCKING DIVISION: Ms. Mika reviewed staff's recommendation. Commissioner Kirkmeyer moved to approve the sole bid from Spradley Barr Ford of Greeley, as recommended by staff. Seconded by Commissioner Garcia, the motion carried unanimously. Fl 5) APPROVE BID #B1200138, LIQUEFIED NATURAL GAS (LNG) TANDEM TRUCK TRACTOR - DEPARTMENT OF PUBLIC WORKS/PAVEMENT MANAGEMENT DIVISION: Ms. Mika reviewed staff's recommendation. Curtis Hall, Department of Public Works, explained the low bid did not meet specifications. Commissioner Garcia moved to approve the second low bid from MHC Kenworth, as recommended by staff. Commissioner Kirkmeyer seconded the motion, and it carried unanimously. Minutes, November 14, 2012 2012-3264 Page 2 BC0016 ED 6) APPROVE BID #81200151, EAST DAVIS DUST CONTROL WELL - DEPARTMENT OF PUBLIC WORKS: Ms. Mika reviewed staff's recommendation. Commissioner Kirkmeyer moved to approve the low bid from Quality Well and Pump, LLC, as recommended by staff. The motion, which was seconded by Commissioner Long, carried unanimously. En 7) APPROVE BID #61200150, BANKING SERVICES - TREASURER'S OFFICE (CON'T FROM 11/07/2012): John Lefebvre, Treasurer, reviewed his recommendation for the low bid for general banking services and indicated he is agreeable to either bid for the bond trust account services. Commissioner Garcia moved to approve the low bids from Wells Fargo and First Bank, as recommended by staff. Seconded by Commissioner Kirkmeyer, the motion carried unanimously. (Clerk's Note: First Bank submitted an e-mail, dated November 15, 2012, withdrawing its bid; therefore, the bond trust account services will be provided by U.S. Bank) E5 8) PRESENT BID #1200158, COPIER/MULTIFUNCTION DEVICE - DEPARTMENT OF PUBLIC WORKS: Ms. Mika presented the bids received and stated this matter will be brought back to the Board with a recommendation on November 28, 2012. NEW BUSINESS: on 1) THREE PURCHASE OF SERVICES AGREEMENTS FOR OPERATIONAL ACTIVITIES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN: Judy Griego, Director, Department of Human Services, reviewed the terms of the agreements. Commissioner Long moved to approve said agreements and authorize the Chair to sign. The motion was seconded by Commissioner Kirkmeyer, and it carried unanimously. El 2) CONSIDER PURCHASE AGREEMENT FOR SOFTWARE AND AUTHORIZE CHAIR TO SIGN - NORTHWOODS CONSULTING PARTNERS, INC.: Ms. Mika explained this item was reviewed by the Governance Committee a year ago to provide electronic imaging for the Department of Human Services. Ms. Griego explained the intent is to have the records more efficient and user friendly for the community, as well as meet increased demand. Bob Choate, Assistant County Attorney, reviewed the negotiated terms and addressed questions from the Board regarding the terms of litigation. Commissioner Kirkmeyer moved to approve the agreement, with instruction to staff to make changes to paragraph #13 to reflect that any litigation will be subject to the laws of Colorado and take place within the jurisdiction of Weld County, Colorado, to reduce potential costs to the County. Commissioner Garcia seconded the motion, which carried unanimously. E5 3) CONSIDER INSTALLATION OF TRAFFIC CONTROL DEVICES ON CR 65 AT CR 52: Janet Carter, Department of Public Works, reviewed the provisions of the sign installation and responded to questions from the Board stating the request is warranted by sight distance limitations from recently installed tank batteries. Commissioner Kirkmeyer moved to approve the sign installation request. Commissioner Long seconded the motion, which carried unanimously. El 4) CONSIDER BYLAWS FOR THE WELD COUNTY DRUG TASK FORCE ASSET SEIZURE/FORFEITURE OVERSIGHT COMMITTEE AND AUTHORIZE CHAIR TO SIGN: Mr. Choate explained the need for the proposed bylaws and the purpose of the Oversight Committee. He answered questions from the Board concerning consultation with the other parties involved, funding distributions, and representation on the committee. Following discussion, Commissioner Kirkmeyer moved to continue the matter to December 19, 2012, to allow additional time for a work session to Minutes, November 14, 2012 2012-3264 Page 3 BC0016 discuss the issue of financial oversight by the Board. The motion was seconded by Commissioner Garcia, and it carried unanimously. E5 5) CONSIDER APPOINTMENTS AND REAPPOINTMENTS TO JUVENILE COMMUNITY REVIEW BOARD: Commissioner Garcia moved to approve the appointments and reappointments. Commissioner Kirkmeyer seconded the motion, which carried unanimously. E5 Upon further review, Commissioner Garcia moved to reconsider the matter. Seconded by Commissioner Long, the motion carried unanimously. Commissioner Kirkmeyer moved to approve the Resolution as drafted, clarifying approval of the alternate positions as well. Commissioner Garcia seconded the motion, which carried unanimously. E5 6) CONSIDER APPOINTMENT OF KARLA FORD AS DEPUTY CLERK TO THE BOARD: Karla Ford gave a brief summary of her background with the County Council before accepting a part-time position in the Clerk to the Board's Office, and stated she is enjoying learning the many aspects of Weld County government. Commissioner Kirkmeyer moved to appoint Ms. Ford as a Deputy Clerk to the Board. Commissioner Garcia seconded the motion, which carried unanimously. El 7) CONSIDER APPOINTMENT OF TAMMY WATERS AS DEPUTY CLERK TO THE BOARD: Tammy Waters provided a brief summary of her past work experience, stated she recently relocated back to Colorado, and she is excited to learn new things. Commissioner Kirkmeyer moved to appoint Ms. Waters as a Deputy Clerk to the Board. Commissioner Long seconded the motion, and it carried unanimously. E l 8) FINAL READING OF CODE ORDINANCE #2012-8, IN THE MATTER OF REPEALING AND RE-ENACTING, WITH AMENDMENTS, CHAPTER 3 HUMAN RESOURCES, OF THE WELD COUNTY CODE: Commissioner Garcia moved to read Code Ordinance #2012-8 by title only. Commissioner Kirkmeyer seconded the motion, and it carried unanimously. Mr. Yatabe read said title for the record. Pattie Russell, Director, Department of Human Resources, indicated no changes have been made since second reading. No public testimony was offered concerning this matter. Commissioner Kirkmeyer moved to approve Code Ordinance #2012-8 on final reading. The motion, which was seconded by Commissioner Long, carried unanimously. E l 9) FINAL READING OF CODE ORDINANCE #2012-10, IN THE MATTER OF REPEALING AND RE-ENACTING, WITH AMENDMENTS, CHAPTER 3 HUMAN RESOURCES, OF THE WELD COUNTY CODE: Commissioner Garcia moved to read the matter by title only. Commissioner Kirkmeyer seconded the motion, which carried unanimously. Mr. Yatabe read said title into the record. Mr. Choate indicated there have been no changes since second reading and he explained the revision is necessary to ensure compliance with federal law. No public testimony was offered concerning this matter. Commissioner Long moved to approve Code Ordinance #2012-10 on final reading. Seconded by Commissioner Garcia, the motion carried unanimously. Commissioner Garcia seconded the motion, which carried unanimously. Mr. Yatabe read said title for the record. Mr. Choate stated there have been no changes since second reading and he explained the revisions are necessary to comply with state law. No public testimony was offered concerning this matter. Commissioner Garcia moved to approve Code Ordinance #2012-9 on final reading. The motion, which was seconded by Commissioner Long, carried unanimously. Minutes, November 14, 2012 2012-3264 Page 4 BC0016 E 11) FIRST READING OF CODE ORDINANCE #2012-13, IN THE MATTER OF REPEALING AND RE-ENACTING, WITH AMENDMENTS, CHAPTER 2 ADMINISTRATION, AND CHAPTER 5 REVENUE AND FINANCE, OF THE WELD COUNTY CODE: Ms. Mika presented the proposed revisions for delegation of authority, and the various fee changes, which were previously discussed during review of the 2013 Budget. In response to Chair Conway, she indicated the department representatives will be present at the second reading to answer any questions and she will schedule a brief work session. No public testimony was offered concerning this matter. Commissioner Kirkmeyer moved to approve Code Ordinance #2012-13 on first reading. Commissioner Garcia seconded the motion, which carried unanimously. PLANNING: El 1) CONSIDER INITIAL HEARING CONCERNING RESCINDING USR PERMIT #1704 - SHELTON LAND AND CATTLE, LTD., AND HEARTLAND RENEWABLE ENERGY, LLC (CON'T FROM 11/14/2011): Chris Gathman, Planning Services, stated Use by Special Review (USR) Permit#1704 was approved on July 21, 2010, and he reviewed the permitted use and location. He stated the applicant is proceeding with some changes to the design and working with the Colorado Department of Public Health and Environment to complete a review of the Design and Operations Plan. He further stated the applicant is requesting a continuance to April, 2014; however, staff is in agreement with a continuance to April, 2013, at which time the state permitting should be completed, which is tied to the recording of the plat. F] Tim Naylor, AGPROfessionals, LLC, represented the applicant and provided a brief summary of the progress being made on the project. He explained the facility must be operational by April, 2014, or the entire project will be invalid. Commissioner Kirkmeyer proposed referring the matter back to staff to continue working with the applicant, with instruction to the applicant to provide quarterly updates to staff. Mr. Naylor indicated the applicant is committed to completing the remaining Conditions of Approval while waiting on the review by the State. Commissioner Kirkmeyer moved to dismiss the hearing and refer the matter back to Planning Services, with direction that the applicant provide quarterly updates to staff, which should be forwarded to the Commissioner Coordinator. Commissioner Long seconded the motion, which carried unanimously. RESOLUTIONS AND ORDINANCES: The resolutions were presented and signed as listed on the Consent Agenda. Code Ordinance #2012-13 was approved on first reading, and Code Ordinances #2012-8, #2012-9 and #2012-10 were approved on final reading. All laws and regulations pertaining to the providing of licensed childcare will be followed. Under no circumstances will a child be discriminated against because race, religion, handicap or social stature. An interview and tour of my home will be completed prior to your child being accepted into care. If after the interview and tour we feel that my home will be a good fit,you will need to provide the following information prior to or on your child's first clay of care: emergency medical authorization, enrollment forms with contact information for parents/guardians and emergency contact, immunization record, health care information and plan (if applicable). BUSINESS HOURS: 7:30 A.M. to 5:15 P.M. Monday through Friday,or by arrangement. FINANCIAL ARRANGEMENTS: RATES: Per Child Full Time: $175.00 per week for toddler(2 years)and up/4-5 days a wk/$35 per day Full Time: $200.00 per week for infant(8 weeks—24 months)4-5 days wk/$40 per day Part Time: $25 per 4 hours or less for toddler Part Time: $30 per 4 hours or less for infant Before/After School: $25.00 a day Drop Ins: $40.00 a day for 2+years, $45 a day for an infant. FEES: *Transportation Fee: If your child needs transportation to or from school or preschool while in my care, you will be charged: $10 for 3 or less trips and $20 for 4 or more trips per week. This covers my gas(YIKES!), wear and tear on the van, as well as the extra chaos involved with loading everyone up and heading out. © *Enrollment Fee: A fee equal to the amount of 1 week of childcare is due upon enrollment. . This is due the first week of care and is non-refundable after 2 weeks. Tuition Payment: Tuition is to be paid at least 1 week ahead of services rendered. Late pick up fee: $15.00 for the first 10 minutes you are late, and $2.00 a minute thereafter, from your contracted time of pick up. This will be paid at time of pick up. Please call if you are going to be late. ***In the event you are unable to pick your child up on time I will feed your child dinner. Your child may have to come with us to our evening events, such as: scouts and sports. If you and your emergency contacts cannot be reached by 6:30pm local authorities will be called. No Show Fee: $25.00 assessed if child is not brought as arranged without notice in addition to contracted rate. Please give one hour of notice if your child/rn will not be coming. Late Payment Fee: $25.00 per day that payment is not received. INITIALS DATE Returned Check Fee: $35.00, and additional costs incurred, along with my late fee if a check is returned to Provider. There after payments will be cash. This must:also be paid before your child/ren will be accepted back into the childcare home. No Payment: Should the need arise to take a parent to small claims court,the parent will be responsible for all expenses incurred to the provider including, but not limited to, substitute day care while the provider is at court, court costs and attorney fees. You will be liable for cost fees, late fees and any cost for loss of wages if there is a small claims case. The childcare home reserves the right to terminate services if payment procedures are not followed DEFINITIONS: Full Time: Childcare contracted on a set scheduled time slot 4 hours or more per day or 4-5 days per week. Part Time: Childcare contracted on a set scheduled time slot less than 4 hours per day or 3 days per week or less. Part Time Childcare occupies a Full Time position. Should a potential client need a full time position,termination notice may be given with the option of parent paying the full time rate to preserve the child's slot. Childcare can only be guaranteed to children that attend full time, or part time and pay full time rates. Before/After School: Includes breakfast and afternoon snack. Care will not begin before 7:30 AM or end later than 5:15 PM. Any day that school is not in session, a full day rate will be charged. No Show: If your child will not be attending Kelly's Kiddie Korral Childcare,a one-hour notice of absence is required. If notice is not received, a$25 fee will be assessed in addition to the regular fee for the day. Drop Ins: I do except children if I have the open spot for them. A family discount when two or more children are cared for is given. If longer hours are needed, arrangements must be made in advance and an extra fee will be charged. Arrangements for paydays can be made for each individual family, based on your paydays. Paydays and rates are as follows: Contracted hours of care,drop-off and pick-up: Please adhere to these times for planning and scheduling purposes. Parents receiving assistance with payments will abide by the rules and regulations regarding payment set by the Department of Human Services, as to parental fees. Each child's fee stated above, including holidays, sick and absent days, must be paid in full at the beginning of each week or as arranged by each family. Each child must be paid for as stated in this contract whether he/she is here or not on one of their scheduled days. You are paying for a position, as well as a service. No refunds are given for late arrivals or early departures. All childcare services will be contracted. This is necessary to hold a position for your child, as I cannot depend on drop-ins in your space for the times you will be gone. You will not receive any unpaid sick days. Please let me know as soon as possible when you plan on taking a vacation or your child is sick, so I may be able to plan accordingly. Please write down dates in calendar in the sign in/out book. PART-TIMERS:-)do not get vacation or sick days and only pay for the typical contracted days. PROVIDER :-) I will/may take a total of fifteen days vacation per year in addition to the holiday time. You will be given three weeks notice when possible, and you will not be charged for the time I take off. I will try to help you find suitable substitute care. © In the unlikely event that am too ill to care for your child I will notify you by 6:00am. HOLIDAYS :-) I take 6 paid holidays. No care will be provided on the following pre-arranged days: New Year's Eve(close at 1:00 PM) *New Year's Day—PAID The day after New Year's-Not paid *Independence Day-PAID *Memorial Day-PAID *Labor Day—PAID *Thanksgiving Eve-Not paid *Thanksgiving-PAID The day after Thanksgiving-Not paid Christmas Eve-Not paid *Christmas Day-PAID The day after Christmas-Not paid if one of these days is on Saturday, the Friday before will be the holiday. If one falls on Sunday, the Monday following will be the holiday. Full fees are charged for these holidays,for full time and part time children. This means you will pay for the day if the holiday falls on one ofyour regularly scheduled days. INITIALS DATE EXTRA FEES/FIELD TRIPS: We participate in many local programs during the week to give the children a variety of experiences. Most of these are free, but a few have a minimal fee which I do ask parents to pay. If this is an issue, please talk with me. I try to my best to plan well ahead and keep you informed. Every summer and most breaks, we take field trips to maximize our time off! Parents will be asked to provide their child with money to cover admission. If you do not want your child to attend you will need to make alternate arrangements for their care that day. I feel these trips add to their childhood experiences. We also take part in 1-2 sessions of swimming lessons at our local pool each summer. It is up to each family if the child participates or not,and you are responsible for class costs. Parents will need to sign permission for scheduled field trips and activities as well as the occasional unplanned excursion. Parents will be notified in writing of any field trip or scheduled activity. In the event that we need to run an errand such as a trip to the grocery store, I will call each parent on their primary phone number and let you know where we are going. Each parent will be given my cell phone number so that they can contact me when we are out. I use a Chevy Express Passenger Van that seats 13. I try to provide appropriate safety seats at all times,but may ask you to provide a seat for me if I am short. PICK UP AND RELEASE OF CHILDREN: Children will be released only to persons listed on their enrollment form. Anyone not known to the provider will be asked to provide and ID. If you have an emergency and need someone not listed on the enrollment form to pick up your child you will need to call and give verbal authorization. That person will need to show ID. If someone attempts to pick up your child and they are not authorized 911 will be called. A parent has the right to pick up their child whether they are listed on the enrollment form or not unless court orders do not allow for this. I will need to have a copy of any court orders. If you or any other person arrives to pick up a child and appear to be under the influence of drugs or alcohol I will encourage you to let me call someone to come get you. If you leave with your child I will call 911 and report you. EMERGENCY PROCEDURES: In case of a natural disaster, such as fire, an earthquake,a tornado etc.,children have been instructed to proceed to exits, meet at the front of the driveway,and then go to the next door neighbors, or other appropriate place, where 911 will be notified, if not previously notified. Parents will be called at that time. Fire drills are performed once a month. Tornado Drills quarterly. In the event of a blackout, children will be kept calm and I will explain to them what is happening. Candles and emergency supplies are kept in the basement. In any event, authorities will be notified. In the event that we have an emergency that causes evacuation there is an emergency kit in the car that contains food, water,blankets and emergency numbers for the children. If you arrive at the home and no one is there due to an emergency we will be at the Windsor Public Library, 720 3`d St,just north of my home. INITIALS DATE SUBSTITUTES: In the event that I will be away for a period of time less than three hours (Doctor appointments, quick errands, etc.)substitute care will be provided by my fully qualified subs: Jessica Ball, Sharon Bowles, Arlene Clark, or Cindy Romero. If there is an emergency where I am unable to get substitute day care,you are required to pick your child up. A picture of each child with emergency numbers is posted on the premises for EMT's or other persons to assist. Arrangements have been made with neighbors and with my regular substitutes in case of any medical or other emergencies. All emergency numbers have been posted on the premises. Substitutes have copies of all said numbers. Hence, it is very important for you to keep you information updated regularly. In the event a child is lost, parents and authorities will be notified IMMEDIATELY! FOOD PROGRAM AND TRAINING: I was previously part of the USDA program where I learned the standards of feeding your child nutritiously. I am no longer part of the program, but continue to feed your child the same. Breakfast is served at 8:00, mid-morning snack at 9:30, lunch at 11:00, and afternoon snack at 3:15. Food served will be nutritious, delicious and meet your child's daily nutritional requirements for the meal or snack served. Please see the whiteboard near the sign in books to see what we will be eating today. If your child has a food allergy I need to know so that access to that food can be limited. Infants: For infants that have only breastfed it will be an easier transition for your baby if you begin introducing a bottle at least two weeks before care begins. I do my best to help and encourage your breastfeeding relationship with your baby and treat your milk like gold! When you feel your baby is ready to begin the transition to a cup or sippy,please talk with me so that we can work together on this process. Same goes for bottle weaning. Training: I have completed First Aid/CPR courses and required to keep them current. The same goes for Meds Administration and Universal Precautions. I am required to have IS hours of continuing education each year in a variety of areas pertaining to early childhood development. I usually go far beyond that. QUIET TIME: Naps or quiet time will be taken at regular times each day. Preschool age children will have a rest on a nap mat with a sheet and blanket.If older children would like a rest or need a rest they will be provided with a nap mat, sheet and blanket. If preschool age children(and older)do not fall asleep after 30 minutes they will be offered an age appropriate quiet activity. Rest time in the home is from approximately 12:30pm until 3:30pm. I provide a nap mat or crib that meets National Safety Standards,for each child in care. I ask that you bring a blanket, a fitted crib sheet and an optional pillow for older children. Favorite stuffed animals are welcome for naptime, but prefer one that can stay here in their tubby. I ask that all bedding goes home weekly to be washed and brought back the following Monday. MEDIA: Parents need to give written permission for their child to participate in the following media use. We watch G rated movies and TV and they are mostly of an educational nature(some Disney, etc. O). Children will not watch TV for more than 30 minutes per day, except for the special occasion movie. INITIALS DATE There is a computer available for older children to do homework. There are very strict parental controls and children will be supervised while using it. They will not be allowed to use the computer for more than 30 minutes unless you give special permission for work on a big project. We do occasionally play video games on the Wii, like Just Dance for Kids, and Wii Fit, as a special treat every now and then. We listen to children's and instrumental music. DIAPERING AND TOILET TRAINING: When your child is in diapers you will need to provide diapers, diaper wipes, diaper rash ointment/cream and extra clothing. When your child begins to ask questions about using the toilet and is able to dress and undress unassisted and we both feel the child is ready, we can begin toilet training. You will then need to provide underpants and plenty of extra clothes(at least 4 sets). I do not have a specific method I use as my experience has shown me that each child's experience is unique. It is important for all of the child's caregivers to be consistent with the toilet training. Toilet training procedures will be discussed with each parent in full. When it is time for toilet training, please dress your child in"easy off"clothing. I will put any soiled clothing in a bag for you to take home and wash. Please make sure you take it with you at pick up! Speaking of Clothing...© Please bring each child at least one change of clothes to have on hand since I do not provide extras. More clothing for infants please. Please dress your child according to the weather. We live in a beautiful state,and barring temps outrageously high above 100 or below 15,we go outside every day! Please bring a bag with necessary items for each child. You are welcome to keep swimsuits and snow clothes here in their cubbies! Personal Items: The child is free to bring personal items, such as special stuffed animals, blankets,etc. for naptime. However, I reserve the right to take the item away from the child,temporarily, and return it to them at the end of the day if I feel the item is unsafe for the child or other children, or it causes undo fighting. Toys may be brought only for special occasions like show and tell. ADDITIONAL FORMS REQUIRED: You will be required to fill out or have filled out,medical record forms, enrollment forms and permission slips. These will have to be done annually. Please notify me immediately upon change of addresses, phone numbers, or place of employment. MEDICATIONS,ILLNESS,AND INJURIES: All medications that need to be given to the child during care have to be in their original container and I will need authorization in writing from you and the child's Dr.to administer ALL medications, including but limited to over the counter meds, such as Tylenol or cough syrup. Illnesses: In the event that your child becomes ill and needs to leave care you will be asked to come pick up your child within one hour. The child will be separated from other children until someone arrives. If your child is injured and requires medical attention you will be notified immediately. Small bumps and bruised are an everyday event for most children at some point in their lives. If you would like to be notified each time please let me know and you will be notified. Otherwise you will be notified at the end of the day. Other emergencies will be handled on a case by case basis. INITIALS DATE No child will be accepted with a contagious illness or fever,temperature of 100 degrees or higher, vomiting or diarrhea. Children can return after 24 hours of being symptom free and/or being on antibiotics, if meds were prescribed. I reserve the right to request a statement from the child's healthcare provider. Immunizations: My own children are fully immunized. I do sometimes accept children into care that may be exempt from some or all immunizations. Be advised that your children may be in care with children that are not fully immunized. SUNSCREEN: Parents must provide sunscreen for their child or go in on our community Rocky Mountain Sunscreen bottle. Written authorization for the application is needed. During summer months and nice weather please apply sunscreen to your child before you arrive. I will reapply according to our activity and manufacturers recommendations. Please provide a sunhat for your child. Infants will have outdoor playtime but will be in the shade whenever possible. DISCIPLINE AND GUIDANCE: Each child will learn and abide by our house rules: "Keep ourselves safe, Keep our friends safe, Keep our things safe!" In my family child care home the children who are too young to understand natural consequences will be redirected to an acceptable activity. Older children will be given choices that clearly explain the consequences of their actions. When a problem arises, children are encouraged to leave the activity or group that is contributing to the behavior that is not wanted. I try and catch children doing something"right"and celebrate their good choices. Our discipline is a verbal warning followed by time out if necessary. I will be fair with you and ask the same in return. PERSONAL PROPERTY: Reasonable steps have been taken to child proof our home, including an inspection from the State. However, if your child intentionally damages home furnishings or personal property belonging to someone else,you will be asked to pay for or replace those items damaged in a way that is fair and equitable. LEGAL: I am required by law to report any sign of neglect or abuse of the children in my care, and I will promptly do so. I am liable,as a childcare professional, for this responsibility and can be fined or jailed for a failure to report any abuse. TAX INFORMATION: At the end of each January, I will provide you a receipt stating the total income I received from you the previous year,along with my social security number. If your numbers differ from mine, please let me know ASAP!! INITIALS DATE OTHER: Children will NEVER be exposed to second hand smoke in the home or car. If visitors to our home smoke, they will not be allowed to smoke in the home or play yard. I have an open door policy. Parents are free to call any time to see how their child is doing. Breastfeeding moms may come by to nurse. I have an answering machine,which I use. I will call parents back as soon as possible. I also carry a cell phone to maximize my accessibility for all reasons. I do have an initial 2-week trial period. After the initial trial period,I require I a two-week notice in writing for termination of services. The parent shall be liable for two weeks childcare in the event the aforementioned notice is not given. The only time fees will be reimbursed by me is if your child is requested by me,to immediately leave the program. Only payment for future days will be returned and within 10 days. This contract will be renewed annually, as deemed necessary, or as the State laws change. RELEASE OF LIABILITY: You and your child as well as your heirs, executors and assigns do hereby release me and my family from any and all liability(with the exception of gross negligence or intentional wrong doing)for any injury, illness, or other damage to person or properly resulting from the placement of your child/ren in my care. This release also applies during any field trips or transportation. As parent/guardian I have read and understand the contents of this contract and agree to the same.
