Re: CDBG#09-010D—Weld Emergency Service Software
Dear Commissioner Conway,
On December 8, 2011, Regional Assistant Robert Thompson and I conducted a monitoring on the above
referenced project. The monitoring was conducted with your Director of Finance and Administration,
Monica Daniels-Mika and Intern, Glen Czaplewski. Included, but not limited to, in the monitoring were the
following areas of compliance:
• Completeness of Project Files
• Financial Management
• Environmental Review
• Community Development
• Citizen Participation
• Civil Rights
We found the project to be in compliance with applicable federal laws. The files and accounting records
were easily accessible for review due to the systems in place.
To the best of my knowledge, all required paperwork has been received and submitted to the CDBG
Compliance Officer. Should this not be the case, I will contact appropriate staff to assist with any issues.
Once the Compliance Officer has approved your final Financial Status Report(FSR)and Project
Completion Report(PCR), all monitoring will be considered complete and the project will be
administratively closed-out.
