RE: APPROVE EDUCATIONAL AFFILIATION AGREEMENT AND AUTHORIZE CHAIR TO
SIGN - UNIVERSITY OF NORTHERN COLORADO
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Educational Affiliation Agreement
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on
Aging, and the University of Northern Colorado, College of Natural and Health Services,
commencing January 1, 2012,with further terms and conditions being as stated in said agreement,
and
WHEREAS,after review, the Board deems it advisable to approve said agreement,a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Educational Affiliation Agreement between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the University of Northern Colorado,
College of Natural and Health Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 16th day of January, A.D., 2012, nunc pro tunc January 1, 2012.

RE: Educational Affiliation Agreement be een the College
of Natural and Health Sciences and the Department of
Human Services' Area Agency on Aging
Attached is an Educational Affiliation Agreement between the University of Northern Colorado,
College of Natural and Health Sciences and the Department of Human Services' Area Agency on
Aging. This Agreement was reviewed under the Board's Pass-Around Memorandum dated
October 25, 2011, and approved for placement on the Board's Agenda.
The agreement outlines the relationship between the University and Area Agency on Aging as it
relates to internships and other clinical practicum experiences. This is a non-financial agreement.
The term of this Agreement begins January 1, 2012, and will remain in effect until any provisions
for termination contained in the Agreement may allow.

UNIVERSITY OF NORTHERN COLORADO
COLLEGE OF NATURAL and HEALTH SCIENCES
AND
WELD COUNTY AREA ON AGING
THIS EDUCATIONAL AFFILIATION AGREEMENT is made this day of October 13, 2011, between the State
of Colorado acting by and on behalf of the BOARD OF TRUSTEES OF THE UNIVERSITY OF NORTHERN COLORADO,
an institution of higher education organized and existing under the laws of the State of Colorado, for the use
and benefit of the College of Natural and Health Sciences, Gunter Hall 1000, Campus Box 134, 501 20th
Street, Greeley, Colorado 80639, hereinafter referred to as "College" or "State", and Weld County Area on
Aging, 315-C North 11th Avenue, Greeley, CO 80632 hereinafter referred to as"Agency" or"Contractor".
FACTUAL RECITALS
A. The purpose of this Agreement is to secure various clinical, practicum, or internship/externship experience
sites for students enrolled in the College's accredited academic programs and/or educational requirements
and/or as required for the award of a degree in a particular area of study, students must complete supervised
experience.
B. College has established and is a provider degreed programs for the education of students studying natural
and health sciences identified as Audiology Speech-Language Sciences, Biological Sciences, Chemistry and
Biochemistry, Community Health, Dietetics, Earth Sciences, Gerontology, Human Rehabilitative Services,
Mathematical Sciences, Nursing, Physics, Recreation, Tourism and Hospitality, and Sport and Exercise
Science, hereinafter referred to as "Programs", which require the educational facilities of Agency. College
desires students to have clinical educational experiences that can be provided by Agency under the terms
and conditions set forth in this Agreement.
C. Clinical or internship education and experience is a required and is an integral component of the College
Program, and College desires that certain of its students, and when appropriate, certain of its faculty
members be permitted to visit and utilize Agency's premises to afford such students and faculty the
opportunity to engage clinical educational experiences.
D. College desires to enhance educational programs and clinical education of College's students by and
through this Educational Affiliation Agreement because Agency recognizes the need for expansion of the
educational development of healthcare professionals, and desires to make its premises available for such
purposes. Agency is an established provider of such services. Agency has the facilities and professional staff
appropriate for this internship.
E. The parties agree that no money will be paid by either party to the other under the terms and conditions of
this Agreement and that the mutual benefits contained herein constitute sufficient consideration.
