AGREEMENT FOR PROFESSIONAL SERVICES
THIS AGREEMENT is made and entered into this 27th day of February, 2012, by and between the County
of Weld, State of Colorado, by and on behalf of Public Works Department, whose address is 1111 H St., P.O. Box
758, Greeley, Colorado 80632, hereinafter referred to as "County Department," and Felsburq Holt & Ulleviq, whose
address is 6300 South Syracuse Way, Suite 600, Centennial, CO 80111, hereinafter referred to as "Contract
Professional."
WITNESSETH:
WHEREAS, County Department is in need of hiring an independent Contract Professional to perform the
following "Professional Services": The construction inspection services of rehabilitation measures for Weld County
Bridge 6/25A over the South Platte River, as detailed in the Scope of Services, dated February 23, 2012, and
attached hereto as ordered by County Department and
WHEREAS, Contract Professional has the time available and is willing to perform the Professional
Services, according to the terms of this Agreement.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties
hereto agree as follows:
1. TERM: The term of this Agreement shall be from date of execution shown above, through and until
completion of the Professional Services as defined in the attached Scope of Work. The term may
be extended by change order to this contract as agreed to by both parties, in writing.
2. SERVICES PROVIDED BY CONTRACT PROFESSIONAL: Contract Professional agrees to
perform the Professional Services for the compensation provided below and as attached in the
Scope of Work.
COMPENSATION: County Department agrees to pay Contract Professional for all services
performed hereunder as follows: The Professional Services shall be provided at the rates set forth
in the Scope of Work and shall not exceed the maximum compensation of$35,340.00. This fee is
compensation for Professional Services as described in the attached Scope of Work. All
compensation amounts payable after the current fiscal year are contingent upon funds for that
purpose being appropriated, budgeted, and otherwise made available. Any additional services
completed beyond those described in the Scope of Work shall be authorized by the County
Department at a fee negotiated between the County Department and Contract Professional using
the established hourly rates. Invoices will be paid within 60 days of being received and accepted
by the County Department.
3. INDEPENDENT CONTRACTOR: Contract Professional agrees that he or she is an independent
contractor and will not become an employee of County Department, nor is he or she entitled to any
employee benefits from County Department as a result of the execution of this Agreement.
4. INDEMNIFICATION, DESIGN AND INSURANCE: Contract Professional shall indemnify and hold
harmless County Department and CDOT, its officers and employees, against liability for injury or
damage and losses, damages and expenses to the extent caused by any negligent act or omission
by Contract Professional or any person or organization for whom Contract Professional is legally
liable in the performance of services under this Agreement. Contract Professional shall be
responsible for the professional quality, technical accuracy, timely completion and the coordination
of all services rendered by Contract Professional and shall, without additional compensation,
promptly remedy and correct any negligent errors, omissions, or deficiencies. Contract
Professional shall maintain commercial general liability insurance in the amount of$1,000,000 per
occurrence and errors and omissions insurance in the amount of$1,000,000.
5. NON-ASSIGNMENT: Contract Professional may not assign or transfer this Agreement, any interest
therein or claim there under, without the prior written approval of County Department.

6. ACCESS TO RECORDS: County Department shall have access to Contract Professional's
financial records for the purposes of audit. Such records shall be complete and available for audit
90 days after final payment hereunder and shall be retained and available for audit purposes for at
least five years after final payment hereunder.
7. TERMINATION: Either party may terminate this Agreement at any time by providing the other party
with a 10-day written notice thereof. Furthermore, this Agreement may be terminated at any time
without notice upon a material breach of the terms of the Agreement. In the event of termination,
the Contract Professional shall supply the County Department with copies all work products
completed to date, including but not limited to electronic files, reports, computations, etc. All work
products completed on this project shall be owned by the County Department.
8. TIME OF THE ESSENCE: Time is of the essence in each and all of the provisions of this
Agreement.
9. ENTIRE AGREEMENT/MODIFICATIONS: This Agreement constitutes the entire understanding
between the parties with respect to the promises and covenants made therein. No modification of
the terms of this Agreement shall be valid unless made in writing and agreed to by both parties.
10. NON-APPROPRIATION: No portion of this Agreement shall be deemed to create an obligation on
the part of County Department to expend funds not otherwise appropriated in each succeeding
year.
11. WAIVER OF IMMUNITIES/THIRD PARTY LIABILITY: No portion of this Agreement shall be
deemed to constitute a waiver of any immunities of County Department or its officers or employees
may possess, nor shall any portion of this Agreement be deemed to have created a duty of care
with respect to any persons other than County Department and not a party to this Agreement.
12. COUNTY REPRESENTATIVE: County Department will designate, prior to commencement of
work, its project representative ("County Representative") who shall make, within the scope of his
or her authority all necessary and proper decisions with reference to the project.
13. MONTHLY REPORT: Commencing thirty (30) days after the date of execution of this Agreement
and every thirty (30) days thereafter, Contract Professional is required to provide County
Representative with a written report of the status of the work with respect to the Scope of Services,
Work Schedule, and other material information. Failure to provide any required monthly report
may, at the option of the County, suspend the processing of any partial payment request.
14. ACCEPTANCE NOT WAIVER: County Department's approval of drawings, designs, plans,
specifications, reports, and incidental work or materials furnished hereunder shall not in any way
relieve Contract Professional of responsibility for the quality or technical accuracy of the work.
County Department's approval or acceptance of, or payment for, any of the services shall not be
construed to operate as a waiver of any rights or benefits provided to County Department under
this Agreement.
15. CDOT REQUIREMENTS: The construction services work under this contract shall be compatible
with the requirements of a separate contract between the Local Agency and the State (which is
incorporated herein by this reference) for the design/construction of the project. The State is an
intended third party beneficiary of this contract for that purpose. Upon commencement of
construction, the Contract Professional shall make available services as requested by the State to
assist the State in the evaluation of construction and the resolution of construction problems that
may arise during the construction of the project. The Contract Professional shall review the
construction contractor's shop drawings for conformance with the contract documents and
compliance with the provisions of the State's publication, "Standard Specifications for Road and
Bridge Construction", in connection with this work.

Rehabilitation Project
Dear Mr. Bedell,
Thank you for the opportunity for Felsburg Holt& Ullevig to provide a proposal for construction
inspection services on the above referenced project. We believe that if our proposal is accepted,
FHU will provide the County with extraordinary value in construction inspection and contract
administration services.
For this bridge project, FHU will assign a civil engineer, Ms. Amy Stecyk, El, to assist the County.
Amy and FHU have a great deal of experience with administering CDOT Local Agency projects
and have an extensive knowledge of CDOTs construction specifications and documentation
procedures. Amy is currently completing similar work for Mr. Eric Bracke of the City of Greeley on
a federally funded local agency project having CDOT oversight by Ms. Nicki Upright. She will be
fully available to you for the start date of March 12, 2012.
Prior to her current assignment, she was Asst. Project Engineer on the SH 119/63id St. Intersection
Project for Boulder County, which was also a CDOT Local Agency project. Contacts for that
project were Mr. Mike Thomas of Boulder County and Mr. Tim Tuttle and Stuart Miller of CDOT
Region 4. Her extensive experience with local aaencv oroiects. especially in CDOT Region 4,
makes her perfectly qualified for your project. Furthermore, her residing in the Niwot/Longmont
area makes her a "local". Her commute between home and the jobsite is approximately 20 miles
each way. Her time spent commuting between home and the jobsite will be considered a normal
commute for any employee and the County will not be charoeq for her commute time; and, the cost
of the vehicle is included in our unit price. We believe that you will find that Amy's experience,
education and local presence will provide Weld County with an exceptional value for this project.

Work will consist of providing professional construction inspection services during the construction of Bridge
8/25A Rehabilitation near Weltenberg. The plsns/specifications for the construction project are avaieble on the
Weld County Webske (Purchasing Department). The time period for the construction is approximately 60
calendar days, and the estimated start data is March 12, 2012. The inspector must be available Monday thru
Friday during this period with possible weekend work as needed.
This project has Federal-Aid funds being administered by COOT. The Construction Inspector will be required to
have detailed understanding of to COOT Local Agency process, and shall hem knowledge and experience
with the CDOT Construction SpedAgdbM and documentation. (Please attach the Inspector's qualifications
and work experience along with your bid documents.) The Construction Inspector will be required to
complete all documentation requirements and maintain organized Nes throughout the construction. Material
quality acceptance testing will be arranged by the inspector and conducted by Ground Engineering Inc.
The Construction Inspector will be required to conduct weekly site meetings with the Contractor end Weld
County Staff. Communications with the County Ailing cellular phone and computer e-mail cepabilltles will be
required.
The lid schedule below Is based upon an estimated 44 working days with an average of 8 hrslay. This is only

REQUIRED EQUIPMENT AND RESOURCES:

any informality regarding the bids, and to accept the bid that is in the best interest of Weld County. Prior to
award, the County may require a resume and list of qualifications and certifications for the proposed
Construction Inspector.
INSURANCE AND CONTRACT REQUIREMENTS;
The Consul ant shall provide proof of adequate insurance coverage for their employee. The Consultant will be
required to enter Into a formal 'Agreement for Professional Services' contract with the County. A copy of this
standard document is available upon request.

Fiber Optic Communication System-Phase 1,Greeley,Colorado
Ms. Stecyk is currently completing an assignment for the City of Greeley where she is administering a CDOT Local
Agency ITS project to connect several signalized intersections in the City to a command center. Her responsibilities
include inspecting conduit boring operations,running weekly progress meetings,documenting quantities of work
completed,checking certified payrolls,conducting EEO interviews,and managing the resolution of a delay claim by
the contractor.
Powers&Dublin Intersection,Colorado Springs,Colorado
On this project for CDOT Region 2,Amy served as the Assistant Project Engineer under a CDOT Project Engineer.
She performed the inspection on the project and the management and documentation of quantities of work
performed by the Contractor. This project consisted of pavement widening for turn lanes,asphalt paving and traffic
signals. Amy's expertise as a CDOT Certified Asphalt Inspector allowed her to protect CDOT and recommend that
pavement not meeting density requirements be ordered byt the CDOT Project Engineer to be removed and replaced.
After performing the corrective work,the contractor filed a claim alleging that CDOT wrongfully rejected the
pavement. The claim was heard by a Dispute Resolution Board and the opinion of the DRB was that the state was
entitled to reject the work and the contractor's claim was without merit. Amy's thorough daily documentation (written,
recorded and photographic)was invaluable to the DRB having arrived at the conclusion fully in favor of FHU's client.
SH 119 at 63rd Street Intersection,Boulder,Colorado
Ms. Stecyk served as the Assistant Project Engineer in charge of the daily operations to widen the pavement,replace
the traffic signals,and improve the drainage in the area of the project.Amy was responsible for managing the efforts
of a Senior Construction Technician and a materials testing subconsultant. She coordinated regularly with BNSF for
work related to removing and replacing an existing railroad bridge, replacement of an at-grade crossing and also
railroad safety gates.Amy also coordinated regularly with the design firm,the City, Boulder County and the
Contractor to resolve issues on the project.She also performed observations and inspections of the work.As a
federal-aid project,Amy was responsible for ensuring that the federal reporting requirements were met.The project
was completed in late 2010 with no contractor claims and on budget.

CDOT Region 4 Pavement Marking Replacement Project
Performed inspection of a project to replace preformed pavement markings at various locations throughout western
Region 4. Responsibilities included performing full time observation of a crew and determining the specific areas of
failure requiring replacement. Inspection of the Contractor's traffic control was also performed for each location.
SH 2/88th Avenue Intersection,Commerce City,Colorado
Performed all project management duties as the owner's representative to realign the intersection and to reconstruct
and widen a half mile of SH 2 and a quarter mile of 88th Avenue.Administrative duties included responding to RFIs,
review and approval of submittals and pay applications, preparation of agendas,meeting minutes and facilitated the
preconstruction, prepave and weekly progress meetings and project documentation. Performed inspections of all
project work,which included: asphalt&concrete paving,drainage,erosion control,traffic control,seeding,traffic
signals,signing and striping and prepared daily diaries. Maintained up to date record drawings in the field for as-
builts.Worked closely with CDOT to ensure that the portion of the project within their jurisdiction was built in
conformance with CDOT standard specifications and plans.Coordinated walkthrough with the City,CDOT and
contractor, prepared punch list,ensured correction of punch list items and performed project close out.
C-470/Alameda Interchange,Lakewood,Colorado
Prepared all traffic control plans including the accommodation for continued bicycle traffic during 90 percent of the
construction. Reviewed traffic signal equipment submittals and MHTs and made recommendations for approval.
Performed inspection of project work,including paving,drainage,traffic control and retaining walls construction.
I-25/144th Avenue Interchange,Westminster and Thornton,Colorado
Performed construction management and observation duties on a project to construct a new interchange on 1-25,
widen the existing roadway and landscape the disturbed areas. Responsible for performing inspections of work,and
for coordinating the work of other project inspectors and materials testing technicians. Performed inspections of
project work, including: asphalt paving, bridge construction,drainage, retaining walls,water line system,traffic signal
and guardrail installation,signing,striping, native seeding and installation of irrigated landscaping with complimentary
lighting. Documented the days work and potential problems via digital photographs,written daily diaries and the
preparation of form 266s for quantities and the assessment of the schedule and project completion. Reviewed and
approved pay applications.Coordinated walkthrough with the Cities of Thornton and Westminster and the contractor,
prepared punch lists for the landscape phase of the project and a schedule for re-evaluation and completion of punch
list items the subsequent spring.
US 36/96th Sheet East and West Interchanges,Broomfield,Colorado
Performed as Assistant Project Engineer on a project to construct a new interchange on the Boulder Tumpike,a
state highway.Was responsible for performing inspections of work,and for coordinating the work of other project
inspectors and materials testing technicians. Coordinated with CDOT Oversight Engineer to ensure that traffic
control and other work was performed in accordance with the State's standards. Performed inspections of all project
work,which included: asphalt paving, bridge construction,drainage,erosion control,traffic control,seeding,striping,
guardrail etc. Was responsible for controlling project documentation. Reviewed submittals and contractor pay
requests.
US 34 Greeley Bypass,Weld County,Colorado
Performed as Assistant Consultant Project Engineer on a State project to reconstruct US 34 from SH 257 to 71st
Avenue in Greeley.Was responsible inspecting all aspects of the work, and documenting quantities of work

Experience Prior to Joining FHU
Transportation Capital Improvements for the City of Northglenn
Managed the design phase of the Grant Street capital improvement project. Coordinated with the design engineer to
design roadway improvements needed for expected redevelopment on Grant Street.The design and construction
phase were on a fast track to satisfy the need for the improvements in advance of redevelopment.
Managed the design phase of the 120th Avenue/Grant Street arterial intersection improvement project.The
intersection lies along the jurisdictional boundary between the Cities of Thornton and Northglenn and was a joint
project for both Cities.Worked cooperatively with Thornton to generate an acceptable design to improve traffic flow
and negotiated an acceptable cost sharing strategy for accelerated design and construction phasing.
Reviewed and commented on multiple drafts of the plans for the 112th Avenue bridge over 1-25 during the design
phase.Also,assisted in field engineering/inspection during the construction phase of the bridge.
City of Northglenn representative on the Church Ditch Company Board
As Vice-president of the board, reviewed proposed developments and proposed work by adjacent property owners
along the ditch to assess potential adverse impacts to the integrity of the ditch or water quality in the ditch water.
Also,coordinated design and construction of capital improvement projects with design engineers and contractors.
Worked with the board president,treasurer and attorney to protect the interests of the company via negotiation and
preparation of written correspondence to parties potentially adverse to the interests of the company.
Wastewater Treatment Plant Expansion Project,Northglenn,Colorado
Managed the construction of the City of Northglenn's wastewater treatment plant expansion project.The project
included the construction of a$12 million mechanical plant. Project management included regular use of"Expedition"
project management software to track requests for information,daily logs, proposed change orders and to approve
the 14 page monthly pay requests.Also, responsible for identifying cost saving measures, prioritizing the use of
contingency and coordinating with state and local agneicies. In one case, I coordinated with Weld County to
expeditiously install a portion of a new effluent pipeline prior to construction of a new county roadway.The timely
construction provided the City with a$45,000 savings by allowing the use of open cut methods rather than boring
techniques.Less than half of the$400,000 contingency was spent by the date of substantial completion.
Berthoud Pass Water Projects,Northglenn,Colorado
Managed the design, permitting and construction of a raw water pipeline replacement project and a stream
restoration project on United States Forest Service property at Berthoud Pass, Colorado. Responsible for all
coordination between the United States Forest Service,the design engineers,the City of Golden, Golden's water
attorney, Northglenn's water attorney and two contractors.Also, performed half of the field engineering for the

construction of the unique stream restoration design and the 40 foot cut in rock for the pipeline replacement project.
The projects were completed under the$750,000 budget.
Capital Improvements for the City of Greeley
As the owners representative, performed field engineering for the construction of a detention pond and additional
capital improvements. Responsible for inspection during the construction of the following in accordance with plans
and specifications:a triple box culvert and associated headwalls,wingwalls,toe walls and apron,a local street, inlets,
storm sewer,water line, pressure reducing valve and vault,sanitary sewer,channel improvements and lastly,
earthwork excavation of the detention pond with associated outlet structures. Also, responsible for review of
submittals,change orders and pay requests. Critically reviewed storm drainage and street design criteria manuals.
Assisted in pond drainage analysis and review of storm drainage reports.
Hydro-Search,Inc.
Assistant project manager for remedial investigation phase of a Remedial Investigation/Feasibility Study(RI/FS).
Designed sampling and analysis plan for solid and gas sampling and prepared scopes of work for performing soil gas
sampling,drilling,excavating and ambient air,gas,liquid,and solid analyses. Responsible for coordination between
field personnel and personnel from three laboratories. The analytical parameters included radio nuclides as well as
organic and inorganic compounds in gas,liquid,and solid sample media.
Prepared a report summarizing compliance and noncompliance of state and federal regulations for compressor
stations in several states as applicable. Reviewed NPDES reports,solid and hazardous waste disposal practices,
evaluated groundwater quality and assessed progress of groundwater remediation.

A fax bid request for construction inspection services for Bridge 6/25A Rehabilitation was sent out by the
Public Works Department to five vendors on February 15, 2012. Two bids were received prior to the
submittal deadline of February 23, 2012. Felsburg Holt & Ullevig submitted a bid of$35,340.00. Short
Elliott Hendrickson submitted a bid for$37,136.92. The other three vendors did not submit a bid.
This project has Federal-Aid grant funds being administered through CDOT. The construction inspector
will be required to have detailed understanding of the CDOT Local Agency process and the associated
documentation requirements. The construction inspectors proposed to be used by each vendor appear to
be equally qualified.
My recommendation is to award an Agreement for Professional Services to the low bidder, Felsburg Holt
& Ullevig, in the amount of $35,340.00. Construction is planned to take place during the months of
March and April, 2012.
Please prepare this bid for presentation and approval consideration by the BOCC on the same date. The
construction is scheduled to start soon and we need to try and award both the construction bid (Naranjo
Civil) and the construction services bid (Felsburg Holt & Ullevig) in the same time frame if possible.

inspection services on the above referenced project. We believe that if our proposal is accepted,
FHU will provide the County with extraordinary value in construction inspection and contract
administration services.
For this bridge project, FHU will assign a civil engineer, Ms. Amy Stecyk, El, to assist the County.
Amy and FHU have a great deal of experience with administering CDOT Local Agency projects
and have an extensive knowledge of CDOT's construction specifications and documentation
procedures. Amy is currently completing similar work for Mr. Eric Bracke of the City of Greeley on
a federally funded local agency project having CDOT oversight by Ms. Nicki Upright. She will be
fully available to you for the start date of March 12, 2012.
Prior to her current assignment, she was Asst. Project Engineer on the SH 119/63rd St. Intersection
Project for Boulder County, which was also a CDOT Local Agency project. Contacts for that
project were Mr. Mike Thomas of Boulder County and Mr. Tim Tuttle and Stuart Miller of CDOT
Region 4. Her extensive experience with local aaencv projects. especially in CDOT Region 4,
makes her perfectly qualified for your project. Furthermore, her residing in the Niwot/Longmont
area makes her a "local". Her commute between home and the jobsite is approximately 20 miles
each way. Her time spent commuting between home and the jobsite will be considered a normal
commute for any employee and the County will not be charged for her commute time; and, the cost
of the vehicle is included in our unit price. We believe that you will find that Amy's experience,
education and local presence will provide Weld County with an exceptional value for this project.

Ms. Stecyk is currently completing an assignment for the City of Greeley where she is administering a CDOT Local
Agency ITS project to connect several signalized intersections in the City to a command center. Her responsibilities
include inspecting conduit boring operations, running weekly progress meetings,documenting quantities of work
completed,checking certified payrolls,conducting EEO interviews, and managing the resolution of a delay claim by
the contractor.
Powers &Dublin Intersection, Colorado Springs,Colorado
On this project for CDOT Region 2,Amy served as the Assistant Project Engineer under a CDOT Project Engineer.
She performed the inspection on the project and the management and documentation of quantities of work
performed by the Contractor. This project consisted of pavement widening for turn lanes,asphalt paving and traffic
signals. Amy's expertise as a CDOT Certified Asphalt Inspector allowed her to protect CDOT and recommend that
pavement not meeting density requirements be ordered byt the CDOT Project Engineer to be removed and replaced.
After performing the corrective work,the contractor filed a claim alleging that CDOT wrongfully rejected the
pavement. The claim was heard by a Dispute Resolution Board and the opinion of the DRB was that the state was
entitled to reject the work and the contractor's claim was without merit, Amy's thorough daily documentation(written,
recorded and photographic)was invaluable to the DRB having arrived at the conclusion fully in favor of FHU's client.
SH 119 at 63rd Street Intersection,Boulder, Colorado
Ms. Stecyk served as the Assistant Project Engineer in charge of the daily operations to widen the pavement, replace
the traffic signals, and improve the drainage in the area of the project.Amy was responsible for managing the efforts
of a Senior Construction Technician and a materials testing subconsultant. She coordinated regularly with BNSF for
work related to removing and replacing an existing railroad bridge, replacement of an at-grade crossing and also
railroad safety gates.Amy also coordinated regularly with the design firm,the City, Boulder County and the
Contractor to resolve issues on the project. She also performed observations and inspections of the work.As a
federal-aid project,Amy was responsible for ensuring that the federal reporting requirements were met. The project

phase were on a fast track to satisfy the need for the improvements in advance of redevelopment.
Managed the design phase of the 120th Avenue/Grant Street arterial intersection improvement project.The
intersection lies along the jurisdictional boundary between the Cities of Thornton and Northglenn and was a joint
project for both Cities.Worked cooperatively with Thornton to generate an acceptable design to improve traffic flow
and negotiated an acceptable cost sharing strategy for accelerated design and construction phasing.
Reviewed and commented on multiple drafts of the plans for the 112th Avenue bridge over 1-25 during the design

City of Northglenn representative on the Church Ditch Company Board
As Vice-president of the board, reviewed proposed developments and proposed work by adjacent property owners
along the ditch to assess potential adverse impacts to the integrity of the ditch or water quality in the ditch water.
Also, coordinated design and construction of capital improvement projects with design engineers and contractors.
Worked with the board president,treasurer and attorney to protect the interests of the company via negotiation and
preparation of written correspondence to parties potentially adverse to the interests of the company.
Wastewater Treatment Plant Expansion Project,Northglenn,Colorado
Managed the construction of the City of Northglenn's wastewater treatment plant expansion project.The project
included the construction of a$12 million mechanical plant. Project management included regular use of"Expedition"
project management software to track requests for information, daily logs, proposed change orders and to approve
the 14 page monthly pay requests.Also, responsible for identifying cost saving measures,prioritizing the use of
contingency and coordinating with state and local agneicies. In one case, I coordinated with Weld County to
expeditiously install a portion of a new effluent pipeline prior to construction of a new county roadway.The timely
construction provided the City with a$45,000 savings by allowing the use of open cut methods rather than boring
techniques. Less than half of the$400,000 contingency was spent by the date of substantial completion.
Berthoud Pass Water Projects,Northglenn,Colorado
Managed the design, permitting and construction of a raw water pipeline replacement project and a stream
restoration project on United States Forest Service property at Berthoud Pass, Colorado. Responsible for all
coordination between the United States Forest Service,the design engineers,the City of Golden, Golden's water

Work will consist of providing professional construction inspection services during the construction of Bridge
6/25A Rehabilitation near Wattenberg. The plans/specifications for the construction project are available on the
Weld County Website (Purchasing Department). The time period for the construction is approximately 60
calendar days, and the estimated start date is March 12, 2012. The inspector must be available Monday thru
Friday during this period with possible weekend work as needed.
This project has Federal-Aid funds being administered by CDOT. The Construction Inspector will be required to
have detailed understanding of the CDOT Local Agency process, and shall have knowledge and experience
with the CDOT Construction Specifications and documentation. (Please attach the inspector's qualifications
and work experience along with your bid documents.) The Construction Inspector will be required to
complete all documentation requirements and maintain organized files throughout the construction. Materials
quality acceptance testing will be arranged by the inspector and conducted by Ground Engineering Inc.
The Construction Inspector will be required to conduct weekly site meetings with the Contractor and Weld
County Staff. Communications with the County utilizing cellular phone and computer e-mail capabilities will be
required.
The bid schedule below is based upon an estimated 44 working days with an average of 8 hrs/day. This is only
for bidding purposes.

REQUIRED EQUIPMENT AND RESOURCES:

• Copies of CDOT Standard Specifications for Road and Bridge Construction, CDOT M&S Standards,
MUTCD manual, CDOT Local Agency Manual, and CDOT Construction Manual.
• All required equipment, fuel, utility bills, etc. shall be included in the bid rates proposed.
BASIS OF AWARD:
This project shall be awarded to the Contractor that provides Weld County with the most reasonable cost as
defined herein and inspector's qualifications. Weld County reserves the right to reject any or all bids, to waive
any informality regarding the bids, and to accept the bid that is in the best interest of Weld County. Prior to
award, the County may require a resume and list of qualifications and certifications for the proposed
Construction Inspector.
INSURANCE AND CONTRACT REQUIREMENTS:
The Consultant shall provide proof of adequate insurance coverage for their employee. The Consultant will be
required to enter into a formal "Agreement for Professional Services" contract with the County. A copy of this
standard document is available upon request.
