Notice is hereby given of the following claim:

C. The basis of the claim is:
On or about July 10, 2012, at approximately 3:00 p.m., the claimant was a
passenger in a vehicle which was struck by a vehicle making an improper left turn. This
collision occurred in a construction zone. The construction zone was negligently
designed and implemented by Weld County which was the cause of this incident. As
such, this improper construction zone constituted a dangerous condition of the public
road which physically interfered with the movement of traffic.

D. The name of the public employee(s) involved is:

E. Claimant has suffered injuries to her lower back, mid back, neck, right
foot and knee, and head, as well as other related injuries.
F. The Claimant will seek damages to compensate her for the cost of medical
treatment, prescriptions, and therapy for the severe pain and suffering
resulting from the subject accident, loss of enjoyment of life, loss of
earnings, and loss of property.
G. The approximate amount of Claimant's injuries are anticipated to be in
excess of$250,000.00.
