The District provides water service within its service area as defined by the District. The provision of water service by extension of existing water lines of the District to the above property and the installation of taps for lots is done under the terms of the Rules and Regulations of the District established by the Board of the District from time to time. You may obtain a copy of the Rules and Regulations from the District. This letter outlines the provisions of the Rules and Regulations; however this letter does not change any provisions of the Rules and Regulations. The Board of the District may alter and amend the Rules and Regulations at any time, and the provisions of this letter are subject to alteration and amendment based on changes in the Rules and Regulations of the District. We currently serve the existing address through tap # 359 and have an 8" diameter water line located along WCR 3; we will continue to provide service to the above property and proposed USR subject to the rules and regulations of the District. The tap will be cataloged as a commercial use and you will need to provide an appropriate backflow device and annual testing. Proposed use of the property: The intended use of this property is as a mixed animal veterinary facility. The proposed facility will initially be used by two veterinarians, and I anticipate acquiring two additional employees (one receptionist and one technician) soon after opening the veterinary clinic. In the distant future, I would like to leave options open to hire one more veterinarian or have an intern, but this clinic will remain small and quiet. The veterinarians will see mostly dogs, cats, and horses, but will probably also see the occasional goat,cow, or other animal. It is my vision and intent that this clinic be a service and an asset to the community, and operate in a manner that is consistent with the existing neighborhood. This will be a general, primary care facility, and will not serve as a specialty or referral center. We will perform some surgeries (e.g. spays and neuters, large animal castrations, some emergency abdominal surgeries)but will mostly perform preventative medicine and routine general practice veterinary procedures. The building design is not yet complete,but the building will be approximately 3000—5000 square feet. It will be two levels, with veterinary reception, exam rooms,and treatment areas on the first level and offices on the second level. 2. Consistency with intent of Weld County Code,Chapter 22 of the Comprehensive Plan: I feel that this proposal is consistent with this section of the Comprehensive Plan, and will support the agricultural heritage of Weld County. This facility will serve the local community by providing easily accessible veterinary care for pets and livestock. My husband and I live adjacent to the property that I am proposing to build on, so I have a personal interest in maintaining the quiet rural atmosphere of the area. The proposed facility will in no way be intrusive to nearby landowners, and the property will be landscaped and maintained in a way that adds to, not detracts from,the natural beauty of the area. 3. Consistency with intent of Weld County Code, Chapter 23 (Zoning) and Zone District As stated in Section B, 5 of Chapter 23, a veterinary clinic is one of the establishments that may be constructed upon approval of a Use by Special Review Permit. I feel that a veterinary clinic, especially the quiet mixed-animal clinic that I propose, will be an asset to the community, and will not cause landowners annoyance or discomfort. 4. Type of Uses: The land surrounding the proposed building site is used for agricultural production (crops and cattle)and residential dwelling. The proposed veterinary clinic will aim to serve those landowners by offering convenient, high quality veterinary care for both pets and working/production animals at a reasonable cost. The proposed facility will not generate large amounts of traffic or noise that would compromise the quality of life of our neighbors, and will operate in a manner consistent with the current norms of the area(e.g. no loud music, maintenance of landscaping, etc). 5. a)As stated in#1, initially there will be two veterinarians using the site,possibly to expand to five people at some point in the future. It is expected that (on the high side) 10-12 clients(animal owners) will use this site each business day. b) It is proposed that, initially, two full-time veterinarians be employed at this site. As both veterinarians will also provide ambulatory service (farm calls),this will decrease the amount of clients and traffic that use the facility each day. It is anticipated that shortly after opening,we will acquire two employees(one receptionist and one technician). As discussed in#1, I think it is reasonable to think that some time in the future the business may hire one associate veterinarian, or have an intern. However, the space of the facility (approximately 3000-5000 square feet) will limit the number of employees. c) The hours of operation will begin with appointments during typical business hours (Monday-Friday 8am-5pm). Veterinary emergencies do occur,and will be seen outside of normal business hours. d) It is proposed that one structure be built on this site. The veterinary clinic building will most likely be a Morton metal building,which will allow us to construct a high quality structure that is easily maintained and aesthetically pleasing. e) No animals will be permanently kept on this site,and this will not be used as a boarding facility or kennel. Occasionally, a small number of animals may be kept on the site (hospitalized)and the facility will abide by the county regulations with regards to the number of animals. 1) It is expected that 10-12 client vehicles will access this site each business day, with 4-5 of those being horse trailers (that is a high estimate for both numbers). Two or three veterinary vehicles (two initially)will be parked on the premises in a covered area for part of the day. g) Berthoud Fire Department h) The City of Berthoud provides domestic water to the proposed building site. i) There is currently a very old septic tank on the property (permit number SP-0000424). A new septic tank will be engineered and plans will be submitted with building permit applications (please see "Septic/Sewage Disposal"). j) There will be no storage or warehousing on the site. 6. Proposed Landscaping: Landscaping design is not yet complete. Landscaping will be minimal as to avoid excessive water use,and will be regularly maintained and groomed. 8. Storm Water Drainage: Storm water is diverted into a ditch that drains into the Little Thompson River. There have been no previous drainage problems with this property or the property immediately to the east(where I live). 9. Construction and Landscaping Time: Construction of the facility will take approximately nine months, and will commence as soon as the Use by Special Review Permit is approved,and building permits are approved. My plan is to begin the building permit process soon so that construction can begin in 4-6 months, as soon as this application is approved. Landscaping will take approximately two weeks, and will begin as soon as possible(while construction is underway). 10. Storage of Wastes: Waste will be stored temporarily in a dumpster that will be emptied regularly. Sharps will be properly disposed of in sealed containers,to ensure that no worker or person is exposed to them. Drugs and medication will be disposed of by an appropriate medical waste disposal service(e.g. Stericycle). Waste Handling Plan: Animal waste and garbage will be disposed of into a dumpster(Waste Management)on site that will be emptied regularly. Care will be taken to ensure that the dumpster is emptied at an adequate frequency. Sharps will be disposed of properly into the dumpster in sealed sharps containers to ensure that no person is exposed to sharp objects. Drugs and medication will be disposed of via an appropriate medical waste disposal service and will not be flushed down the toilet or sink. Stericycle: (866) 783-7422 Corporate Headquarters: 28161 N. Keith Dr., Lake Forest IL 60045 As stated in Weld County regulations, all floor drains will drain to a vault. Dust Abatement Plan: During the construction phase of the project,water sprinklers will be utilized to suppress dust. After construction, landscaping will mitigate dust around the building. Much of the property is currently covered in grass and natural vegetation that is regularly mowed. County Road 3 is paved from Highway 56 to the driveway to the proposed building site, so incoming traffic from the north(the most common way that people will access the property)will not present a dust problem. The drive and parking areas will be treated with a dust suppressant(most likely magnesium chloride)to minimize dust in those areas. Traffic won the property will be kept to low speeds. If required at any point during or after construction, a water truck will be utilized to maintain dust suppression. Septic/Sewage Disposal: There is an existing septic permit for the property (SP-0000424). A new septic tank will be engineered and installed in accordance with Weld County regulations, and the engineered plan will be submitted with building permit applications. Because this location is not served by the public sewer system, a septic tank is the preferred method of sewage disposal. Traffic Narrative: As stated in the Questionnaire ( #5), it is expected that approximately 10-15 vehicles(client and veterinary vehicles) will access the property each business day. I expect 4-5 of those vehicles to be horse or stock trailers. I expect 80%of traffic to come from the north (from Highway 56), and 2b%to come from the south along County Road 3. As both Highway 56 and County Road 3 north of 4{{ property to Highway 56 are paved, I do not believe that the traffic generated by this business will cause any undue wear to the road. For security reasons, a gate will most likely be erected on the driveway on the property.
