HomeMy WebLinkAbout20122742.tiff RESOLUTION RE: APPROVE CONTRACT FOR COLORADO HEALTH ASSESSMENT AND PLANNING SYSTEM (CHAPS) IMPLEMENTATION GRANT AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Contract for the Colorado Health Assessment and Planning System (CHAPS) Implementation Grant between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Public Health and Environment, and the Colorado Department of Public Health and Environment commencing October 1, 2012, and ending September 30, 2013, with further terms and conditions being as stated in said contract, and WHEREAS, after review, the Board deems it advisable to approve said contract, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Contract for the Colorado Health Assessment and Planning System (CHAPS) Implementation Grant between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said contract. Wallace, MD, MPH, Director Department of Public Health and Environment DATE: September 6, 2012 SUBJECT: CDPHE Office of Planning and Partnerships CHAPS Implementation Grant Enclosed for Board review and approval is a contract for funding between the Colorado Department of Public Health and Environment (CDPHE) and Weld County Department of Public Health and Environment to implement Phases 6 (Create the Plan), 7 (Implement and Monitor Plan), and 8 (Inform Statewide Plan) of the Colorado Health Assessment and Planning System (CHAPS) in Weld County. The total amount of the agreement is $20,000.00 for the time period of October 1, 2012 through September 30, 2013. Four equal payments will be made upon receipt of quarterly reports that document adequate performance toward the deliverables and documentation of expenses. This one-time grant will fund local public health improvement plan activities including a) facilitating coordination of the local plan, b) ensuring engagement of local stakeholders, c) sharing the local plan with statewide partners, d) communicating the plan, e) implementing activities of the plan and f) evaluating plan progress regularly. The key deliverable for this contract is to submit Weld County's PHIP including Phase 6 work plan templates to Weld County's BOH/BOCC and the Office of Planning and Partnerships for subsequent review by the Colorado Board of Health. I recommend your approval. Enclosure Weld County Department of Public Health and Environment Office of Planning and Partnerships Colorado Health Assessment and Planning System (CHAPS) Implementation Grant 2012-2013 Scope of Work 1. Project Description: Contract Term: October 1,2012 to September 30,2013 The Office of Planning and Partnerships is in the process of co-developing and facilitating a standardized, statewide public health assessment and planning system as required by the Public Health Act of 2008, (C.R.S. 25-1-501 et seq.). The purpose of this contract is for WELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT to participate in a local public health improvement process focused on CHAPS phases 6-8, including use of available templates and processes or other applicable tools according to local county needs for developing and implementing a public health plan, in accordance with the Act. Contractor shall implement the necessary steps and processes to complete the local public health improvement process and achieve the activities identified under this Scope of Work. Contractor shall ensure collaboration and communication with the Office of Planning and Partnerships. Contractor shall designate at least one staff member and/or project manager as the point of contact for public health assessment and planning activities, public information, communications, and/or media relations. If planning to seek accreditation through the Public Health Accreditation Board (PHAB) within the next five years, agency will monitor related PHAB requirements and modify/augment assessment and planning process accordingly. [I. Performance Requirements/Deliverables: Local Agency Activity/Deliverables Approximate Preparation and Implementation of the Local Public Health Improvement Plan(PHIP) Completion CHAPS Phases 6-8 Date Ensure coordination of a local PHIP oversight body(e.g. steering committee, etc). Oct 15,2012 Determine agency/region's organizational structure and process for implementing the work(e.g. Oct 31, 2012 priority-focused task forces, etc.). Ensure engagement of stakeholders in both planning and implementation activities. Nov 1,2012 and ongoing Facilitate related planning meetings with PHIP team and related committee(s). Nov 1, 2012 and ongoing Prepare a local/community Public Health Improvement Plan, that,at a minimum: Jan 1, 2013 • Examines data about health status and risk factors in the local community; • Assesses the capacity and performance of the county or district public health system; • Identifies goals and strategies for improving the health of the local community; • Describes how representatives of the local community develop and implement the local plan; • Addresses how county or district public health agencies coordinate with the state department and others within the public health system to accomplish goals and priorities identified in the comprehensive , statewide public health improvement plan;and 2012-2742 Page I of 3 Local Agency Activity/Deliverables Approximate Preparation and Implementation of the Local Public Health Improvement Plan (PHIP) Completion CHAPS Phases 6-8 Date • Identifies financial resources available to meet identified requirements for the provision of core public health services. Submit completed PHIP to local board of health for review. Feb 1,2013 Submit local PHIP and completed Phase 6 Work Plan Templates to the Office of Planning and Feb 15, 2013 Partnerships for aggregation of information and presentation to the Colorado Board of Health. Share local/regional PHIP content with statewide partners(though collaboration, plan submission, Mar I, 2013 and reporting). Participate in statewide planning opportunities. Communicate the plan with stakeholders. Ongoing Implement activities of the plan,evaluate progress regularly, and make mid-course corrections Ongoing when necessary. Contract Budget: Contractor's total award includes$20,000 in funding to ensure accomplishment of the public health assessment and planning tasks for WELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT. Contractor will utilize agency funding within budget constraints to support activities and all position related costs including salary and fringe. Contractor will ensure that deliverables are accomplished to meet the related needs of Weld County. Contractor shall dedicate the necessary funds to support the accomplishment of activities identified under this Scope of Work. This contract will fund the completion of the deliverables identified in this Scope of Work. Contractor shall establish a process to track all obligations and expenditures related to this contractual agreement separately from other funds awarded, including but not limited to activities of project management,process planning and facilitation,data analysis and interpretation and reasonable operational and/or personnel expenses in the completion of the Scope of Work. Contractor is encouraged to supplement this work with other funding sources to leverage resources and to ensure coordination with other efforts in this area of activity. Contractor shall ensure that reimbursement requests are not duplicated under any other funding or utilized to supplant non-related activities or programs. Contractor shall ensure appropriate distribution of costs in direct relation to the activities performed. Contractor understands and agrees that this funding made available pursuant to this contractual agreement cannot be used for any work or project other than that specified in this Scope of Work.Contractor further understands that Contractor is not to include any work not related to the Scope of Work on any reimbursement statements to the State. Contractor shall ensure that all deliverable activities will be completed by September 30,2013. Payment: Payment equal to(1/4)of the total allocation shall be paid in the first month of each of the four quarters. III. Invoicing: Not applicable. Invoices are not submitted as payment terms and conditions are laid out in the above Performance Requirements/Deliverables. IV. Monitoring/Reporting: Page 2 of 3 CDPHE's monitoring of this contract for compliance with performance requirements will be conducted throughout the purchase order period by the Program's Public Health Planner. The contractor shall maintain records to provide the information herein required by the State and submit to such reasonable audits and evaluations as may be necessary. Using a template provided by the Office of Planning and Partnerships, Contractor shall submit a report of activity on the following dates: December 31, 2012 March 31, 2013 June 30,2013 September 30,2013 - Final Report V. Resolution of Non-Compliance: The Contractor will be notified by email or telephone within 30 calendar days of discovery of a compliance issue. Within 30 calendar days of discovery,the Contractor and the State will collaborate,when appropriate, to determine the action(s)necessary to rectify the compliance issue and determine when the action(s) must be completed. The action(s)and time line for completion will be documented by email and agreed to by both parties. If extenuating circumstances arise that requires an extension to the time line,the Contractor must email a request to the Public Health Planner and receive approval for a new due date. The State will oversee the completion/implementation of the action(s)to ensure time lines are met and the issue(s) is resolved. If the Contractor demonstrates inaction or disregard for the agreed upon compliance resolution plan,the State may exercise its rights under the Terms and Conditions of this Purchase Order. Sean P.
