The following change is hereby made to the Contract Documents: The original scope of services related to the Agreement for Professional Services dated May 21,2012 included the work items necessary to complete the design of a 2-lane roadway. Recent direction from the BOCC has resulted in the need to amend the scope of services to now include work items necessary to complete the design of a 4-lane roadway. SCOPE OF WORK FOR WELD COUNTY ROAD 49 EXTENSION PROJECT FROM SH 34 TO WELD COUNTY ROAD 60.5 CHANGE ORDER#1 -4-LANE ROADWAY September 28,2012 I. GENERAL Description of the project The original scope of services included the work items necessary to complete the design of Weld County Road 49 as a 2-lane roadway including a 2-lane bridge over the Cache la Poudre River from SH 34 to Weld County Road 60.5. Upon completion of the traffic study, it was determined and direction was provided by the Weld County BOCC to proceed with the design and construction of a 4-lane facility from approximately 1,000' south of SH 34 to 1,000' north of WCR 60.5. The resulting additional scope of work items include the following: • 4-lane Bridge over Cache la Poudre(original scope was for 2-lane) • Additional structure over S. Platte River, 593' by 37' wide(not included in original scope) • Additional structure or widening of existing over S. Platte River overflow, 83' x 37' (not included in original scope) • Modification/extension of Plum Ditch structure crossing (not included in original scope) • S. Platte River floodplain permitting,CLOMR/LOMR (not included in original scope) • Preparation of two additional bid packages, three total (original scope included one package) • WCR 49 construction with concrete pavement over aggregate base • Include access control where possible including grass/unpaved median, no median, and/or relocation of accesses II. SCOPE OF SERVICES AND COST ESTIMATE The scope of services for the modification includes project management, structural design of S. Platte River Bridges, floodplain and hydraulic analysis/modeling of S. Platte River, Weld County Hood Hazard Development Permit for S. Platte River, geotechnical investigation and design of additional structures. Weld County has provided the original topographic surveying and mapping and supplemental surveys were completed previously. Weld County will provide the preliminary HEC-RAS Model for the S. Platte River Floodplain, environmental assessment study, permits, and environmental mitigation, USACE 404 Permit, right-of-way ownership and plan mapping, and right-of-way acquisition. The following additional work tasks will be completed under this modification for three construction plan packages for the proposed improvements. A. PRELIMINARY DESIGN PHASE The preliminary design submittal scope modification tasks include all work necessary for the preparation of a preliminary plan package (approximately 60% complete). The original scope included the preparation of a preliminary (CDOT FIR level) plan submittal for the entire proposed improvements and has not changed. Following the preliminary design submittal the work will be separated into project bid packages. Perform and/or furnish the following additional engineering design services for the modification work tasks into the preliminary design submittal: 1. Project Management—No modification to original scope of work. Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) Modification#1 —4-Lane Roadway 2. Utility Locating—No modification to original scope of work. 3. Potholing—No modification to original scope of work. 4. Surveying and Mapping— Additional supplemental surveys are anticipated for the completion of S. Platte River cross-sections, as-built surveying of existing S. Platte River and Overflow Bridges, and topographic surveying of the Plum Ditch structure crossing. Fifty hours of surveying is included for this work. 5. Geotechnical and Pavement Design - No modification to original scope of work for the pavement design. For the structure selection and preliminary design of the S. Platte River Bridge, Overflow Bridge and Plum Ditch modification, as-built plans and associated geology sheets will be used. Additional geotechnical boring will be completed during the final design phase. 6. Traffic Study—No modification to original scope of work. 7. Preliminary Roadway Design (ultimate geometric review and 60% complete) — Complete preliminary roadway design for the modification work elements associated with the 4-lane ultimate section. Existing I8th Street will be reconstructed where the original scope assumed shoulder widening only. 8. Preliminary Storm Drainage Design and Drainage Report —Complete the hydraulic design of the proposed facilities that will include hydrologic analysis, storm drain hydraulics and miscellaneous hydraulic calculations. The following presents the additional work items that will be completed under the drainage study, including specific assumptions required to prepare the described work products. a. Plum Ditch cross-culvert—The existing plum ditch cross-culvert requires extension for the widening of the roadway. All widening will occur to the north of existing 18th Street. Contact ditch owner for their requirements and the times they will allow construction to take place. b. Plan Sheets—Incorporate the proposed Plum Ditch modification on the preliminary plan and profile sheets. Structure crossing plan and profile sheet shall show the minor and major HGL (hydraulic grade line)on the profile. 9. S. Platte River CLOMR/LOMR—Prepare hydraulic models for S. Platte River and provide the necessary floodplain permitting documents for the roadway encroachment and bridge construction. The following presents the work items that will be completed: a. Hydraulic Modeling— Update the existing USACE S. Platte River HEC-2 model to include additional existing conditions cross sections at the proposed bridge crossing. This model will provide more accurate existing flooding conditions and serve as a basis for comparison to determine proposed impacts. The existing conditions model will be revised to include the proposed bridge structure and roadway encroachments within the floodplain. The proposed bridge design and potential impacts will be coordinated with Weld County and adjacent property owners. b. Coordination-The proposed bridge designs and potential impacts will be coordinated with Weld County and adjacent property owners. Impacts to adjacent property owners will be minimized. c. FHDP and CLOMR--A Flood Hazard Development Permit and Conditional Letter of Map Revision will be prepared and submitted to Weld County and FEMA for review and approval. The submittal will include hydraulic modeling, calculated impacts to the S. Platte River floodplain and floodway, detailed floodplain mapping of existing and proposed conditions, descriptive report and necessary forms. Coordinate with Weld County staff review as well as FEMA review personnel to facilitate the review and approval process. 2 of 4 Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) Modification#1 —4-Lane Roadway d. LOMR—Following completion of the bridge and roadway improvements within the floodplain and receipt of as-built survey information, a Letter of Map Revision will be prepared and submitted to FEMA. The LOMR will document the as-built conditions of the floodplain and floodway encroachments and provide post-construction hydraulic modeling and floodplain mapping. The LOMR submittal will include pre-project and post-project hydraulic modeling, detailed floodplain mapping of the revised floodplain and floodway, descriptive report and necessary forms. Coordinate with Weld County staff review as well as FEMA review staff so that the updated floodplain information can be approved as effective. 10. Structure Design/Structure Selection Report—Prepare a structure selection report to evaluate bridge type and span configuration alternatives. The intent is to add a second separate structure similar to existing for the S. Platte River Bridge. For the Overflow Bridge, a separate structure or extension of existing will be considered. Evaluation items such as constructability, hydraulic performance, impacts to environment, geometries, maintenance and durability,and construction costs will all be considered as a part of the selection process. At least two alternatives will be analyzed in details with the above factors and a preferred alternative will be recommended. A Structure Selection Report will be prepared, including the development of a general layout and typical section for the recommended alternative. 11. Preliminary Utility and Irrigation Ditch Coordination & Design—No modification to original scope of work for utility coordination and design. Coordinate and contact the Plum Ditch for modification of crossing structure. Support Weld County with drawings needed to formulate agreements for the required relocation. 12. Ownership Mapping and Right-of-way Plans—By Weld County, coordinate, as needed. 13. Preliminary Engineering Design Plans and Meetings - Preparation and submittal of the completed plans and specifications for review will include preliminary quantities and construction cost estimate. The following will be completed: a. The design plan submittal includes the following sheets for the modification tasks: • S. Platte River and Overflow Bridge Sheets • Plum Ditch Modification Sheets • Grading and Drainage Sheets— 18th Street Reconstruction • Roadway Cross Sections - 18th Street Reconstruction B. FINAL DESIGN The final design including construction bid plans, specifications, cost estimates, and contract documents for the construction of the 4-lane arterial typical section will be completed based on comments received at the Preliminary meeting. For the final design, three separate phases will be identified and three bid plan packages will be prepared. The original scope of work was for one bid plan package only. 1. Aesthetics Concept Development—No modification to original scope of work. 2. Final Design Plans and Meeting — Prepare a 95% complete construction plan set and include revisions per comments made during the Preliminary meeting. Two additional bid plan package submittals and associated review meetings are required. 3. Preparation of construction permits - Obtain Conditional Letter of Map Revision for S. Platte River floodplain. 4. Preparation of Contract Documents and Specifications — Prepare two additional project contract documents and specifications using the CDOT standards and specifications. 3 of 4 Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) Modification#1 —4-Lane Roadway 5. Project Advertisement Package Preparation — Make corrections to plans and contract documents after the final design review meetings and prepare the two additional project advertisement packages. 6. Assistance with Project Bidding - Attend two additional pre-bid meetings and answer questions during bidding. 7. Preparation of revised "For Construction" Plan Set—Prepare two additional final plan set and contract document packages that incorporate any revisions required during the bid process. Plans shall be marked "FOR CONSTRUCTION, (DATE)" with each sheet stamped by the consultants Professional Engineer of Record for the project. III.DELIVERABLES -The following is an estimate of the deliverables required for this project. Preliminary Engineering Design • Structure Selection Report including structure layout—S. Platte River Bridge, Overflow Bridge, and Plum Ditch structure modification • Flood Hazard Development Permit and Conditional Letter of Map Revision (CLOMR)—S. Platte River Final Design • Final Geotechnical Report with bridge foundation design recommendations—S. Platte River Bridge, Overflow Bridge, and Plum Ditch structure modification • Two additional final Plan Package(plans, specifications, and cost estimate) (5 half-size copies) • Two additional "For Construction" Plan Packages (plans, specifications, and cost estimate) (1 11"x17'and 1 specification book stamped/signed for each) • Structure Field Package and Rating • LOMR(as-built survey to be provided by Weld County) IV. LIST OF ASSUMPTIONS 1. The new WCR 60.5/WCR 47/WCR 49 intersection will be stop controlled. The future location of signals poles will be considered. Traffic signal design is not included and would be additional work is determined to be required. • 2. All environmental studies, permitting, and mitigation design will be completed by others. Coordination and design impacts will be completed to assist with this effort. These environmental work items are to be complete by FHU under a separate contract with Weld County. 3. The preliminary (CDOT FIR level) submittal will be for the entire project improvements. The intent of this submittal and review is to reach agreement on the proposed right-of-way for the purposes of Weld County initiating the acquisition process as quickly as possible. Following the preliminary design submittal, a determination will be made for project packaging. Three bid plans, specifications, and estimate (PS&E) are anticipated. 4. For the preliminary plan submittal, as-built geology will be used for the structure selection design layouts. Additional geotechnical borings will be completed during the final design. 5. Box 758, Greeley, Colorado 80632, hereinafter referred to as "County Department," and Atkins North America Inc., whose address is 4601 DTC Boulevard, Suite 700, Denver, CO 80237, hereinafter referred to as "Contract Professional." WITNESSETH: WHEREAS, County Department is in need of hiring an independent Contract Professional to perform the following "Professional Services:" The design of WCR 49 Extension, as detailed in the Scope of Work attached hereto as ordered by County Department and WHEREAS, Contract Professional has the time available and is willing to perform the Professional Services, according to the terms of this Agreement. NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties hereto agree as follows: 1. TERM: The term of this Agreement shall be from date of execution shown above, through and until completion of the Professional Services as defined in the attached Project Schedule. The term may be extended by change order to this contract as agreed to by both parties, in writing. 2. SERVICES PROVIDED BY CONTRACT PROFESSIONAL: Contract Professional agrees to perform the Professional Services for the compensation provided below and as attached in the Fee Estimate. COMPENSATION: County Department agrees to pay Contract Professional for all services performed hereunder as follows: The Professional Services shall be provided at the rates set forth in the Fee Estimate and shall not exceed the maximum compensation of$774,252.00. This fee is compensation for Professional Services as described in the attached Scope of Work. All compensation amounts payable after the current fiscal year are contingent upon funds for that purpose being appropriated, budgeted, and otherwise made available. Any additional services completed beyond those described in the Scope of Work shall be authorized by the County Department at a fee negotiated between the County Department and Contract Professional using the established hourly rates. Invoices will be paid within 60 days of being received and accepted by the County Department. 3. INDEPENDENT CONTRACTOR: Contract Professional agrees that he or she is an independent contractor and will not become an employee of County Department, nor is he or she entitled to any employee benefits from County Department as a result of the execution of this Agreement. 4. INDEMNIFICATION, DESIGN AND INSURANCE: Contract Professional shall indemnify and hold harmless County Department, its officers and employees, against liability for injury or damage and losses, damages and expenses to the extent caused by any negligent act or omission by Contract Professional or any person or organization for whom Contract Professional is legally liable in the performance of services under this Agreement. Contract Professional shall be responsible for the professional quality, technical accuracy, timely completion and the coordination of all services rendered by Contract Professional and shall, without additional compensation, promptly remedy and correct any negligent errors, omissions, or deficiencies. Contract Professional shall maintain commercial general liability insurance in the amount of $1,000,000 per occurrence and errors and omissions insurance in the amount of$1,000,000. NON-ASSIGNMENT: Contract Professional may not assign or transfer this Agreement, any interest therein or claim there under, without the prior written approval of County Department. 6. ACCESS TO RECORDS: County Department shall have access to Contract Professional's financial records for the purposes of audit. Such records shall be complete and available for audit 90 days after final payment hereunder and shall be retained and available for audit purposes for at least five years after final payment hereunder. 7. TERMINATION: Either party may terminate this Agreement at any time by providing the other party with a 10-day written notice thereof. Furthermore, this Agreement may be terminated at any time without notice upon a material breach of the terms of the Agreement. In the event of termination, the Contract Professional shall supply the County Department with copies all work products completed to date, including but not limited to electronic files, reports, computations, etc. All work products completed on this project shall be owned by the County Department. 8. TIME OF THE ESSENCE: Time is of the essence in each and all of the provisions of this Agreement. 9. ENTIRE AGREEMENT/MODIFICATIONS: This Agreement constitutes the entire understanding between the parties with respect to the promises and covenants made therein. No modification of the terms of this Agreement shall be valid unless made in writing and agreed to by both parties. 10. NON-APPROPRIATION: No portion of this Agreement shall be deemed to create an obligation on the part of County Department to expend funds not otherwise appropriated in each succeeding year. 11. MONTHLY REPORT: Commencing thirty (30) days after the date of execution of this Agreement and every thirty (30) days thereafter, Contract Professional is required to provide County Representative with a written report of the status of the work with respect to the Scope of Services, Work Schedule, and other material information. Failure to provide any required monthly report may, at the option of the County, suspend the processing of any partial payment request. 14. ACCEPTANCE NOT WAIVER: County Department's approval of drawings, designs, plans, specifications, reports, and incidental work or materials furnished hereunder shall not in any way relieve Contract Professional of responsibility for the quality or technical accuracy of the work. County Department's approval or acceptance of, or payment for, any of the services shall not be construed to operate as a waiver of any rights or benefits provided to County Department under this Agreement. GENERAL Description of the project The scope of services will include the work items necessary to complete the design of Weld County Road 49 from SH 34 to Weld County Road 60.5. This will include arterial roadway design along a virgin alignment, intersection improvements to SH 34 and WCR 60.5 and create new intersections with WCR 47.5/58, 18th Street,and East Holly Avenue.This approximately 3 mile roadway will cross multiple regional and local irrigation ditches as well as the Cache la Poudre River and South Platte River floodplains. This project will meet the Weld County, CDOT and AASHTO design criteria. The limits of the project are approximately 3 miles of WCR 49 alignment as well as up to approximately 1,500 feet west and east along both SH 34 and WCR 60.5 for turn, acceleration and deceleration lanes.The projects south termini is approximately 2 miles west of Kersey while the north termini is near the southeast side of the Greeley/Weld Airport in Weld County. SCOPE OF SERVICES AND COST ESTIMATE The scope of services for the project includes project management,arterial design, structural design of bridges and concrete box culverts, floodplain and hydraulic analysis/modeling, Weld County Flood Hazard Development Permit, storm drainage design,traffic engineering, constructability review,utility and irrigation ditch coordination/design, geotechnical/pavement design, supplemental survey,property owner coordination and potholing. Construction management services and post design support will be negotiated and provided if needed. Weld County will provide the topographic surveying and mapping, preliminary HEC-RAS Model for the Cache la Poudre Floodplain, environmental assessment study, permits,and environmental mitigation, USACE 404 Permit, right-of way ownership and plan mapping, and right-of- way acquisition. Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) The following work tasks will be completed for the preparation of one complete construction plan package for the entire proposed improvements. A. PRELIMINARY DESIGN PHASE The preliminary design submittal shall include all work necessary for the preparation of a preliminary plan package (approximately 75% complete). Perform and/or furnish the following engineering design services: 1. Project Management — This includes monthly billings and invoices as well as detailed progress reports including milestones and issues or concerns. Atkins will coordinate and manage the subconsultants work. a. Prepare a Project Control Plan(PCP) and Quality Control Plan (QCP) b. Attend a pre-design kick-off meeting and on-site field review with Weld County staff. c. Attend progress meetings to discuss the conceptual design layouts and alternatives developed. Prepare an agenda, meeting minutes and responses to comments. Eighteen progress meetings (approximately every month) are anticipated. d. Prepare and maintain a detailed Microsoft Project schedule of the design work. e. Communicate regularly with Weld County project manager throughout the project duration. f. Assist with CDOT coordination. g. Assist with stakeholder and property owner coordination. 2. Utility Locating — Detailed utility locating will include contacting the known providers and having the facilities field verified. This information will be included in the project topographic file for the project. 3. Potholing — Potholing will occur for all existing utilities following location and based on anticipated conflicts. A total of 60 potholes is anticipated. 4. Surveying and Mapping —The existing survey and mapping provided by Weld County has been reviewed and it is anticipated that additional survey may be required along SH 34 and WCR 60.5. This will be based on the required acceleration and deceleration lengths determined during the traffic study. The potholes and geotechnical borings will be surveyed and included in the project topographic file. Additional supplemental surveys required based on the proposed roadway and intersection designs including WCR 47.5 for potential closure will be accomplished. One week of surveying is included for miscellaneous as-needed work. 5. Geotechnical and Pavement Design - A preliminary geotechnical and pavement report will be based on borings along the proposed arterial alignment and at intersection tie-ins. It is understood that the virgin alignment portion of WCR 49 passes through private property that permission to enter has not yet been obtained. Once access is granted to the property the borings will take place and the final geotechnical and pavement report will be developed. Design the SH 34 connection with existing concrete pavement and WCR 60.5 widening including, coordination with CDOT and meeting CDOT requirements. Weld County will prepare the permit and submit to CDOT. 6. Traffic Study—A traffic study and Traffic Analysis Report will be prepared using the North Front Range MPO (NFRMPO)Traffic Model. The analysis will involve adding the new WCR 49 connection to the NFRMPO 2035 model and the resulting effects of the proposed roadway connection to adjacent roadways and intersections will be determined. The model will be refined as needed within the traffic study area between WCR 54 and SH 392 north/south and US 85 and WCR 59 east/west. Future traffic projections and geometric design recommendations will be provided. The following steps will be undertaken to fulfill this task: a. assemble, collect, and analyze existing traffic data including NFRMPO Traffic Model 2of7 Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) b. obtain and refine the regional travel demand model c. identify safety, geometric,and other traffic issues to roadways and intersections within the study boundary d. determine future traffic projections e. conduct a needs assessment and capacity analysis for roadways in the study area f. forecast traffic signal warrants and intersection level of service (LOS) g. recommend roadway sizing and intersection signalization and geometric improvements h. address phasing of project improvements to meet interim vs. ultimate year traffic demands The Traffic Study will be coordinated with Weld County staff throughout its preparation. Based on the traffic projections,lane requirements will be carried forward into the roadway design. 7. Preliminary Roadway Design (ultimate geometric review and 75% complete) — The preliminary roadway design for the 4-lane ultimate section shall be completed and reviewed by the project team. The design will be discussed at a progress meeting and comments incorporated. Prepare the following information (as a minimum): a. Prepare all necessary roadway, intersection and driveway sections. b. Design and plot horizontal and vertical alignments on roll plots(show existing conditions). c. Determine utility conflict locations. d. Determine preliminary earthwork data. e. Plot catch points and determine preliminary right-of-way and easement requirements. f. Plot current ownership information on plan/profile sheets. g. Plot the cross sections at 50' intervals and identify all proposed slopes. Prepare several conceptual horizontal and vertical roadway design layout options for review at the progress meetings prior to the preliminary plan package submittal. Specific design elements include WCR 49 horizontal alignment, vertical profile, intersection geometry, structure layouts, and floodplain analysis. A brief Conceptual Design Memo will be prepared and submitted that includes the project design data, conceptual plan drawings, right-of-way information, storm drainage, and cost estimate. Any other pertinent design information will be included in this report. 8. Preliminary Storm Drainage Design and Drainage Report — Complete the hydraulic design of the proposed facilities that will include hydrologic analysis, storm drain hydraulics and miscellaneous hydraulic calculations. The work includes proposed sizing for roadside ditches and any pipes and structures. The following presents the work items that will be completed under the drainage study, including specific assumptions required to prepare the described work products. a. Design Criteria—Review the County's criteria as well as any related agency criteria. A summary criteria memorandum will be completed discussing the criteria that will be followed for the design. This will include rainfall, design frequency information, pipe sizing and materials and all other pertinent criteria. b. Hydrology—Provide a detailed hydrologic analysis of the drainage basins within the project area. This will include basin delineation and peak runoff determination.Future and/or current development will also be evaluated for potential impacts to hydrology. Hydrologic methodology will consist of the Rational Method for basins less than 90 acres and CUHP/SWMM for larger basins. c. Alternative analysis—Analyze storm pipe crossings for proposed sizing including recommendation from master plans, proposed development submittals, and compliance with Weld County design standards. This work will include an analysis of existing conditions (storm flow onto existing properties) and documentation that future as-constructed conditions will be equal to or better than the before construction condition. The work will be coordinated 3 of 7 Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) with Weld County and adjacent property owners where required. d. Data Collection—Collect and review all relevant studies and master plans. Meet with County Engineering and Maintenance staff to discuss existing flooding problems and history and will visit project site to better understand flow paths and existing infrastructure. e. Ditches and pipe cross-culverts—The required ditch and cross-culvert sizes will be determined. Contact ditch/canal owners for their requirements and the times they will allow construction to take place. Investigate and design the possible realignment of Plum Ditch adjacent to 18th Street. 1. Irrigation Design—A design will be developed that will allow the remaining portions of the field bisected by the realignment to be returned to use using flood irrigation. g. Plan Sheets—Incorporate the proposed drainage facilities improvements on the preliminary plan and profile sheets. Storm drainage culvert plans shall show the minor and major HGL (hydraulic grade line) on the profile. 9. CLOMR/LOMR—Prepare hydraulic models for Cache la Poudre River and provide the necessary floodplain permitting documents for the roadway encroachment and bridge construction. The following presents the work items that will be completed: a. Hydraulic Modeling—Update the 2007 USACE Cache la Poudre River HEC-RAS model to include additional existing conditions cross sections at the proposed bridge crossing.This model will provide more accurate existing flooding conditions and serve as a basis for comparison to determine proposed impacts. The existing conditions model will be revised to include the proposed bridge structure and roadway encroachments within the floodplain. The proposed bridge design and potential impacts will be coordinated with Weld County and adjacent property owners. b. Coordination -The proposed bridge design and potential impacts will be coordinated with Weld County and adjacent property owners. Impacts to adjacent property owners will be minimized. c. FHDP and CLOMR—A Hood Hazard Development Permit and Conditional Letter of Map Revision will be prepared and submitted to Weld County and FEMA for review and approval. The submittal will include hydraulic modeling, calculated impacts to the Cache la Poudre floodplain and floodway, detailed floodplain mapping of existing and proposed conditions, descriptive report and necessary forms. Coordinate with Weld County staff review as well as FEMA review personnel to facilitate the review and approval process. d. LOMR—Following completion of the bridge and roadway improvements within the floodplain and receipt of as-built survey information, a Letter of Map Revision will be prepared and submitted to FEMA. The LOMR will document the as-built conditions of the floodplain and floodway encroachments and provide post-construction hydraulic modeling and floodplain mapping. The LOMR submittal will include pre-project and post-project hydraulic modeling, detailed floodplain mapping of the revised floodplain and floodway,descriptive report and necessary forms. Coordinate with Weld County staff review as well as FEMA review staff so that the updated floodplain information can be approved as effective. 10. Structure Design/Structure Selection Report—Prepare a structure selection report to evaluate bridge type and span configuration alternatives. Cost and constructability analysis will be performed to determine the recommended bridge layout and structure type. Evaluation items such as constructability, hydraulic performance, impacts to environment, geometries, maintenance and durability, and construction costs will all be considered as a part of the selection process. At least two alternatives will be analyzed in details with the above factors and a preferred alternative will be recommended. A Structure Selection Report will be prepared, including the development of a general layout and typical section for the recommended alternative. 4 of 7 Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) 11. Preliminary Utility and Irrigation Ditch Coordination & Design—Coordinate and contact each utility company with facilities within the project limits. Support Weld County with drawings needed to formulate agreements for the required utility relocations. This project will include but is not limited to overhead and underground electric, buried gas, water,telephone services (including fiber optic), and irrigation facilities. Conduct potholing to verify utility locations and vertical data, at potential conflict areas for shallow utilities, services and other field obstructions, and provide utility modification or relocation recommendations where there are conflicts. Assist with the developing the necessary schedule for the utilities to be adjusted prior to the roadway construction. Notify and mail to the utility companies the plan and profile, utility conflict, and cross-section sheets of the preliminary engineering submittal (and provide them with electronic files if requested). Follow up with the utility companies on a regular basis to ensure the designs and relocations are on schedule. 12. Ownership Mapping and Right-of-way Plans—By Weld County,coordinate, as needed. 13. Preliminary Engineering Design Plans and Meetings - Preparation and submittal of the completed plans and specifications for review will include preliminary quantities and construction cost estimate. The following will be completed: a. Prepare plans, project specifications, and cost estimate b. Prepare and distribute plans prior to the scheduled review date c. Send plans to the affected utility companies for their review and follow up with telephone calls and letters as needed. d. Conduct the meeting, prepare and distribute minutes from the meeting. e. The design plan submittal shall include as a minimum the following sheets: • Title Sheet • Typical Sections • General Notes and Legend • Survey Control Sheet(by Weld County) • Utility Conflict Plan • Special Details • Plan and Profile Sheets • Bridge Sheets • Concrete Box Culvert Sheets • Grading and Drainage Sheets • Construction Phasing Plan • Storm Water Management Plan • Roadway Cross Sections Attend a preliminary plan review meeting to discuss comments to the plan package submittal. Prepare meeting minutes and responses to comments. Prepare for, attend, and conduct a value engineering/constructability review meeting to identify areas for design improvement and cost savings. The construction phasing will be analyzed as well. Up to four consultant team members and Weld county staff will participate in the meeting. B. FINAL DESIGN The final design including construction bid plans, specifications, cost estimates, and contract documents for the construction of the 2-lane interim arterial typical section will be completed based on comments received at the Preliminary meeting. 5 of 7 Scope of Services Weld County Road 49 Extension (SH 34 to WCR 60.5) 1. Aesthetics Concept Development — Prepare up to 3 options for aesthetic elements at the Cache la Poudre Bridge and WCR 60.5 intersection. A rendering of the proposed bridge will be completed. 2. Final Design Plans and Meeting — Prepare a 95% complete construction plan set and include revisions per comments made during the Preliminary meeting. One submittal is proposed for the entire improvements. The scope of work for the final design shall include as a minimum the sheets included in the preliminary submitted plus the following: • Tabulation of Approximate Quantities(split into two columns) • Geometry Layout • Structure Cross Sections • Storm Water Management Plan • Erosion Control Plans • Signing and Striping Sheets 3. Preparation of construction permits - Complete the permitting necessary for construction of the project. This anticipates the preparation of a Conditional Letter of Map Revision. 4. Preparation of Contract Documents and Specifications — Prepare project contract documents and specifications using the CDOT standards and specifications. The County will provide a boiler plate that includes the General Conditions. 5. Project Advertisement Package Preparation — Make corrections to plans and contract documents after the final design review meeting and prepare the project advertisement package. 6. Assistance with Project Bidding-Attend pre-bid meeting and answer questions during bidding. 7. Preparation of revised "For Construction" Plan Set — Prepare a final plan set and contract document package that incorporates any revisions required during the bid process. Plans shall be marked "FOR CONSTRUCTION, (DATE)" with each sheet stamped by the consultants Professional Engineer of Record for the project. The future location of signals and conduit will be considered. Traffic signal design is not included. 3. Existing WCR 47.5 will be evaluated for closure at either SH 34 or the new WCR 49. A turnaround design will be completed. 4. The project design will be completed and bid as one project. There is the possibility for additional funding grants that may require separate bid projects and the design for these separate packages would be additional work. 5. The traffic study and initial geotechnical investigations to determine depth to bedrock will be used for decisions regarding initial improvements and geometry. 6. A phased geotechnical plan is proposed with 3 borings completed in June 2012 and remaining borings completed after bridge type selection and layout(2 drilling mobilizations and ultility clearances required). Weld County will coordinate the right to enter from the landowners. 7. The CLOMR submittal will be made after Weld County revises their floodplain regulations. 8. Weld County will complete all project staking required for ROW and utilities coordination. 9. Surveying will involve supplementing the existing mapping at irrigation ditches/laterals, for the Cache la Poudre River floodplain, and potholing locates 10. It is assumed that the new WCR 49 Bridge over the Cache la Poudre will be for an initial two lane with shoulders (approximately 47'-0" wide). 11. An access control plan is under development for WCR 49 and recommendations will be incorporated. 12. Improvements to existing 18th Street will include widening to add shoulders (8' paved and 2' gravel) and evaluation/replacement of bridge railing on the Bridge 58.0-047.0A. No modification of structures are proposed. 13. It is assumed that the improvements within the South Platte River floodplain will result in a zero rise condition and modeling and documentation of this is included. If it is decided to include improvements that require a CLOMR/LOMR this would be additional work. 14. Private irrigation facilities will be coordinated with the landowner or tenant. A preliminary plan design of the impacted facility will be completed for the purposes of completing the appraisal. The actual improvements above culvert crossing of the roadway will be included in the ROW negotiation and completed by others. 15. All environmental studies,permitting, and mitigation design will be completed by others. Coordination and design impacts will be completed to assist with this effort. 16. Weld County will answer Contractor questions and prepare addenda. Plan revisions will be completed if required. 17. Construction phase services for management, inspection, and design support are an option to be negotiated after design completion. 18. Aesthetic elements will be considered; up to three conceptual options will be prepared for consideration.
