RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENTS FOR SERVICES AND
AUTHORIZE CHAIR TO SIGN - REFLECTIONS FOR YOUTH, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and
Reflections for Youth, Inc., commencing June 1, 2011, and ending May 31, 2012,with furtherterms
and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Child Protection Agreements for Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and Reflections for Youth, Inc., be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 9th day of April, A.D., 2012, nunc pro tunc June 1, 2011

RE: Amendments to Child Protection Agreements for Services
between the Weld County Department of Human Services
and Reflections for Youth, Inc.
Enclosed for Board approval are Amendments to Child Protection Agreements for Services
between the Department and Reflections for Youth, Inc. These amended agreements were
reviewed under the Board's Pass-Around Memorandum dated March 19, 2012, and approved for
placement on the Board's Agenda.
The amendments are to the current Reflections for Youth contracts for Day Treatment Services,
Sex Abuse Treatment Services and Home Based Services. A new location for services has been
added. Reflections for Youth recently received their official license from the State for a new
facility in Brighton and we are utilizing that site for our South County children. The
amendments only reflect the addition of the Brighton site; there are no changes to rates or
services. All locations will have the same services at the rates currently under contract.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
REFLECTIONS FOR YOUTH, INC.
This Amendment,made and entered into the day of March 2012,by and between the Board of
County Commissioners,on behalf of the Weld County Department of Human Services,hereinafter referred
to as"Department,"and Reflections for Youth, Inc.,whose address is P.O.Box 1860,Berthoud,CO 80513,
hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Day Treatment Services in
order to update available service sites under this agreement;and
WHEREAS,Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit A, Item 4,is hereby revised to include the following:
CONTRACTOR will provide Day Treatment services at 1000 S.Lincoln Avenue,#190-
200, in Loveland,CO 80537,and at 60 South 27th Avenue, in Brighton,CO 80601.
2. Exhibit A,Item 5,is hereby revised to include the following:
CONTRACTOR is licensed by the Colorado Department of Human Services State Child
Care Licensing under Provider ID 1545846(Loveland)and 1609468(Brighton).
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.

(Informed Supervision Only)in order to update available service sites under this agreement;and
WHEREAS, Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit A,Item 1,is hereby revised to include the following:
CONTRACTOR will provide informed supervision services to individuals,parents,
family members and caretakers,who supervise juvenile sex offenders at 1000 S.Lincoln
Avenue, in Loveland,CO,(Provider ID 1545846)or at 60 South 27th Avenue,in
Brighton,CO,(Provider ID 1609468)as referred by the Department. Provider will
consider providing services in home or in a community setting on a case-by-case basis.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day,

(Behavioral Coach)Support Program(Home Based Intensive Services) in order to update available service
sites under this agreement;and
WHEREAS, Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
1. Exhibit A, Item 1,is hereby revised to include the following:
CONTRACTOR will provide intensive home and community based behavioral coaching
services to youth 10 years of age or older,and their caregiver(s), under Provider ID
1545846 and 1609468, as referred by the Department, except when any of the following
exist:
a. The referred youth has an IQ below 55.
b. There is active sexual abuse is occurring in the home.
c. There is recent or current domestic violence.
d. A high lethality suicide attempt has occurred within the last 30 days(from the
start date of the referral).
e. There are felony assault charges by youth or family member within the last 30
days(from the start date of the referral).
f. The youth or another participating family member are actively psychotic.
g. There are any legal or behavioral patterns that present significant issues of
danger to staff providing services.
h. The youth identified for services is younger than six(6)years of age.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
