RE: APPROVE STATEMENT OF WORK FOR COLORADO IMMUNIZATION PROGRAM
(CIP) CHILD CARE INSPECTIONS STANDARDS (CHINS) AND AUTHORIZE CHAIR
TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Statement of Work for the Colorado
Immunization Program (CIP) Child Care Inspections Standards (CHINS) between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Department of Public Health and Environment, and the Colorado
Department of Public Health and Environment, commencing November 1, 2012, and ending
June 30, 2013, with further terms and conditions being as stated in said Statement of Work, and
WHEREAS, after review, the Board deems it advisable to approve said Statement of
Work, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Statement of Work for Colorado Immunization Program (CIP)
Child Care Inspections Standards (CHINS) between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment, be, and hereby is, approved.

SUBJECT: Purchase Order for the 2012-13 Child
Care Inspections Standards (CHINS)
Enclosed for Board review and approval is a Statement of Work for a purchase order offered to
our Immunization Program from the Colorado Department of Public Health and Environment
(CDPHE) entitled the 2012-13 Child Care Inspections Standards (CHINS). This is an award
from the Colorado Department of Health and Environment to the Immunization Program of the
Public Health Services Division of the Weld County Department of Public Health and
Environment(WCDPI IE).
The Colorado Immunization Program (CIP) at the CDPHE has funds available to support the
2012-13 Child Care Inspections Standards (CHINS) and other local efforts to decrease the
burden of pertussis disease and improve immunization rates of child-care providers and the
children they serve. If approved, the funding for this purchase order will provide for
collaboration between Nursing and Weld County Environmental Health. This will be done
through inspections that will partner an Immunization Program Nurse and an Environmental
Health Inspector for 20 inspections, 20 educational presentations to Child Care Provider
agencies, 20 educational presentations to parents and family care-givers of child-care aged
children, and an audit of 1200-1500 immunization records of children in child care settings. In
addition, this funding will provide for 0.25 Immunization RN time (10 hours per week) to meet
these criteria.
The term of this contract is for one year, beginning November 12, 2012 and ending on June 30,
2013. Funding for this program period will not exceed $15,000. Funds are 100% State dollars.

I. Projects Description: In phase II of the CHINS Project, the Colorado Immunization Program
(CIP) plans to build on models that have emerged and create new opportunities for insuring that
children in child care settings have limited risks of infection due to increased efficiencies in
tracking, monitoring and assessing required immunizations. Because of decreased funding and
resources, it is critical to develop health promoting systems that are sustainable and efficient in
the child care setting.
Project Goal: To improve the quality of child care health inspections completed by local
public health agencies (LPHA) through implementing systems that address infection control and
improve immunization rates in the child care setting.

Objective#1-1 By 011/30/13, a WCDPHE immunization nurse will educate Environmental
Health staff with basic knowledge in immunization practices and schedules.
Steps to achieve this objective:
1. By 11/20/12, WCDPHE immunization nurse will coordinate a meeting
with Environmental Staff, whom conduct child care inspections, to
discuss plan and schedule child care center visits together.
2. By 12/09/12, WCDPHE immunization nurse provide education to
Environmental Health staff regarding the most recent ACIP
recommendations for childhood immunizations and the importance for
children to receive these immunizations. Regulations regarding
exemption status will be provided to Environmental Health staff as well.
Performance 1. This objective will be accomplished by documenting the number of
measures Environmental Health staff attending planning meetings
2. This objective will be accomplished by administering a pre and post test
of Environmental staff at the training
Objective#1-2 By 12/12/12, a WCDPHE immunization nurse will collaborate with
Environmental Health staff to schedule child care center inspection
appointments concurrently to conduct audits at a minimum of 20 different
child care centers.
Steps to achieve this objective:
1. By 06/30/13, the WCDPI IE immunization nurse will attend 20 different
child care centers in accompany with Environmental Health staff to
conduct immunization chart audits for children attending the child care
center.
2. By 06/30/13, approximately between 1200-1500 immunization records
will be reviewed by using CIIS and audited for children attending child
care centers, and recommendations will be made to child care center staff
regarding if child is up to date.
Performance 1. The objective above will be accomplished by documenting the number of
measures children up to date by child care center
2. The objective above will be accomplished by, if possible, documenting
the type and number of vaccines missing among children
Objective#1-3 By 01/31/13, a WCDPHE immunization nurse will collaborate with
Environmental Health staff to develop a system that will assist in future
inspections to increase Child Care Centers' accountability in immunization
compliance within the child care setting.
Steps to achieve this objective:
1. By 01/04/13, an educational session will be scheduled with
Environmental Health staff to discuss current ACIP recommendations and
other updated immunization information.
2. By 01/31/13, a guide/tool will be developed by WCDPHE Immunization
nurse and Environmental Health staff to use during future child care
center inspections that identify required/recommended immunizations
children should maintain at appropriate ages for child care staff to identify
if children are in compliance.

Care Centers and educate child care staff on the current immunization
schedules, vaccine safety, and tools to assess if child is up to date.
Steps to achieve this objective:
1. By 02/15/13, WCDPHE immunization nurse will collaborate with Child
Care Center staff at time of inspection to provide in-services with ACIP
recommendations and immunization updates.
2. By 06/30/13, 20 educational in-services will be provided regarding ACIP
recommendations, newest research on vaccine safety, and methods to
assure children are adhering to immunization requirements.

Objective#2-2 By 06/30/13, a WCDPHE immunization nurse will educate Child Care Center
staff about the importance of the staff maintaining up to date status on
immunizations, utilizing information from the CDC website during a
scheduled immunization record audit visit.
Steps to achieve this objective:
1. By 03/29/13, WCDPHE Immunization nurse will collaborate with 20
Child Care Centers and incorporate ACIP recommendations for Adult
immunizations with the in-services regarding adult immunization updates
at the time of scheduled inspections.
2. By 05/01/13, a collaborative effort will be created to work alongside with
Child Care Center providers with any concerns or questions regarding
immunization status for children attending or staff.

Objective#2-3 By 02/15/13cducational material that has been developed through other grant
opportunities will be revised to enhance the learning of both child care and
staff immunization status.
Steps to achieve this objective:
1. By 02/08/13, a pamphlet will be developed providing information for
parents and child care center staff to utilize regarding ACIP
recommendations for childhood immunizations, up through the age of 5
years. This pamphlet can be dispersed to parents of children attending the
facility.
2. By 02/15/13, a collaborative effort with pharmaceutical representatives
will be presented requesting informational brochures to provide to child
care facilities regarding vaccine safety and vaccine preventable disease

information. These supplies will be provided to hand out to parents and
for staff to increase their knowledge with vaccine preventable diseases.

Goal#3 An educational goal is to collaborate with child care center staff to
afford opportunities for Public Health nurses to educate patents on the
importance of their adult immunization status,specifically for the Tdap,
to ensure understanding of the risk of pertussis,the reasons for that risk,
and how immunizing the adults around infants and young children can
be life saving.
Objective#3-1 By 06/30/13, a WCDPHE immunization nurse will educate Parents of Child
Care Center children about the importance of being up to date with their
immunizations for Tdap, utilizing information from the CDC website during a
scheduled immunization record audit visit.
Steps to achieve this objective:
1. By 03/08/13, WCDPHE Immunization nurse will collaborate with 20
Child Care Centers and incorporate ACIP recommendations for Adult
immunizations with the presentations for parents regarding Tdap
immunization updates at scheduled times advertised to the parents.
2. By 03/15/13, will have 20 presentations scheduled. through 6/30/13

Nov 2012 At this time, collaboration between the WCDPHE Immunization nurse and
Environmental Health staff who conduct child care center inspections will be made
to coordinate scheduling inspections together. Information for ACIP
recommendations and immunization information will be collected from various
resources, including pharmaceutical representatives to help develop materials to
provide to child care center staff and parents of children attending these child care
centers. A meeting will be held with Environmental Health staff to provide based
education regarding immunization information and ACIP recommendations for
children up to age 5 years.
Dec 2012 Collaboration between the WCDPHE Immunization nurse and Environmental
Health staff who conduct child care center inspections will continue to coordinate
scheduling inspections together. Information for ACIP recommendations and
immunization information will be collected from various resources, including
pharmaceutical representatives to help develop materials to provide to child care
center staff and parents of children attending these child care centers. Pamphlets

ready to disburse.

for WCDPHE Immunization nurse and Environmental Health staff to conduct
immunization audits. At these inspections, educational in-services will be
scheduled with child care center staff' to provide basic immunization information
and ACIP recommendations. Pamphlets developed to provide to parents/staff of
child care centers, along with other immunization information, will continue to be

June Collaboration between the WCDPHE Immunization nurse and Environmental
2013 Health staff who conduct child care center inspections will continue to coordinate

for WCDPHE Immunization nurse and Environmental Health staff to conduct
immunization audits. At these inspections, educational in-services will be
scheduled with child care center staff to provide basic immunization information
and ACIP recommendations. Pamphlets developed to provide to parents/staff of
child care centers, along with other immunization information, will continue to be
disbursed at these inspections. Four parent presentations will be scheduled for June.

quarterly CDPHE Reimbursement Invoice Form. This form is accessible from the CDPHE

g) To be considered for payment, billings for payments pursuant to this Purchase Order must

time after the expiration or termination of this Purchase Order; but in no event no later than
sixty (60) calendar days from the effective expiration or termination date of this Purchase
Order.
VI: Monitoring:

CDPHE's monitoring of this Purchase Order for compliance with performance requirements
will be conducted throughout the Purchase Order period by the CIP Project Manager, Contract
Monitor and/or Evaluation Coordinator. Methods used will include review of documentation
reflective of performance to include progress reports and invoices.
VII: Resolution of Non-Compliance:
The Contractor will be notified by email within 15 days of discovery of a compliance issue.
Within 30 days of discovery, the Contractor and the State will collaborate, when appropriate, to
determine the action(s) necessary to rectify the compliance issue and determine when the
action(s) must be completed. The action(s) and time line for completion will be documented by
email and agreed to by both parties. If extenuating circumstances arise that require an
extension to the time line, the Contractor must email a request to the CIP Project Manager and
receive approval for a new due date. The State will oversee the completion/implementation of
the action(s) to ensure time lines are met and the issue(s) is resolved. If the Contractor
demonstrates inaction or disregard for the agreed upon compliance resolution plan, the State
may exercise its rights under the Terms and Conditions of this Purchase Order.
