Grant Reporting and Management The DCJ reporting forms that you are required to use for this project can be obtained from our website at "http://dcj.state.co.us/oajja" and by clicking on "Grant Reporting Forms" and then by clicking "Juvenile Accountability Block Grant (JABG)." Please destroy any outdated forms. Submission of any outdated forms will delay any requests made to DCJ. Quarterly financial and narrative reports are due fifteen (15) days after the end of each calendar quarter- April 15, July 15, October 15, and January 15. Final financial and narrative reports are due 45 days after the grant end date. Please note that there is a separate form for the final narrative, DCJ Form 2JB Subgrant Narrative Report - Final. In addition to narrative reports, data forms specific to the purpose area in which services are being provided are to be submitted quarterly. A blank form has been included within this packet. Electronic versions of the forms can be obtained from the website address listed above. Cash requests (draw downs of awarded funds) are to be submitted quarterly on a reimbursement basis.
