RE: APPROVE REQUEST FOR RELEASE OF FUNDS AND CERTIFICATION FOR
NEIGHBORHOOD STABILIZATION PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Request for the Release of Funds and
Certification for the Neighborhood Stabilization Program from the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Greeley/Weld County Housing Authority, to the U.S. Department of Housing and Urban
Development, Office of Community Planning and Development, with further terms and
conditions being as stated in said request and certification, and
WHEREAS, after review, the Board deems it advisable to approve said request and
certification, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Request for the Release of Funds and Certification for the
Neighborhood Stabilization Program from the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Greeley/Weld
County Housing Authority, to the U.S. Department of Housing and Urban Development, Office
of Community Planning and Development, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said request and certification.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of January, A.D., 2012.

This form is to be used by Responsible Entities and Recipients (as defined in 24 CFR 58.2)when requesting the release of funds, and

11. Program Activity/Project Description
ith NSP3 funds, specific single-family properties will be identified with in the city of
Fort Lupton for acquisition and rehabilitation or acquisition, demolition, clearance, and
reconstruction throughout the life of the grant; none have been identified to date. A
map of the area in which funds will be expended, and approved by the U. S. Department of
Housing and Urban Development, is attached. As individual properties are identified,
further review will be conducted on the Site Specific Checklist and issues mitigated as
needed.
Single-family housing rehabilitation activities will include full property rehabs, and may
include any or all of the following: testing and mitigation for environmental concerns,
interior and exterior rehabilitation, energy conservation measure, landscape work, rehab
or reconstruction of accessory structures, on-property water/sewer line work, and other
rehab work as required. In some cases and as allowable, there may be rehabilitation to
public infrastructure. Pre-disposition costs will also be paid for by NSP3, such as lawn
mowing and snow removal. Properties will then be sold to households earning 120% or less
than the Area Median Income .

Part 2. Environmental Certification (to be completed by responsible entity)
With reference to the above Program Activity(ies)/Project(s),I,the undersigned officer of the responsible entity,certify that:
I. The responsible entity has fully carried out its responsibilities for environmental review,decision-making and action pertaining to
the project(s)named above.
2. The responsible entity has assumed responsibility for and complied with and will continue to comply with, the National
Environmental Policy Act of 1969,as amended,and the environmental procedures,permit requirements and statutory obligations
of the laws cited in 24 CFR 58.5;and also agrees to comply with the authorities in 24 CFR 58.6 and applicable State and local laws.
3. After considering the type and degree of environmental effects identified by the environmental review completed for the proposed
project described in Part 1 of this request,I have found that the proposal did V did not require the preparation and dissemination
of an environmental impact statement.
4. The responsible entity has disseminated and/or published in the manner prescribed by 24 CFR 58.43 and 58.55 a notice to the public
in accordance with 24 CFR 58.70 and as evidenced by the attached copy(copies) or evidence of posting and mailing procedure.
5. The dates for all statutory and regulatory time periods for review,comment or other action are in compliance with procedures and
requirements of 24 CFR Part 58.
6. In accordance with 24 CFR 58.71(b),the responsible entity will advise the recipient(if different from the responsible entity)of any
special environmental conditions that must be adhered to in carrying out the project.
As the duly designated certifying official of the responsible entity, I also certify that:
