RE: APPROVE TWO PURCHASE OF SERVICES AGREEMENT AMENDMENTS WITH
VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two Purchase of Services Agreement
Amendments between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the following
providers, commencing July 1, 2012, and ending June 30, 2013, with further terms and conditions
being as stated in said amendments:
1) Commission Now, LLC
2) University of Northern Colorado
WHEREAS,after review, the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the two Purchase of Services Agreement Amendments between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, and the above listed providers be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 16th day of July, A.D., 2012, nunc pro tunc July 1, 2012.

RE: Amendments to Purchase of Services Agreement between the Weld
County Department of Human Services' Area Agency on Aging
and Various Providers
Enclosed for Board approval are Amendments to Purchase of Services Agreements between the
Department and Various Providers. These Agreements are funded through the Older Americans Act fund
and/or the Older Coloradoan funds. The Area Agency on Aging Budget was presented under the Board's
Work Session dated May 10, 2012, with the requirement of completing annual Purchase of Services
Agreements. These Agreement Amendments were reviewed under the Board's Pass-Around
Memorandum dated July 5, 2012, and approved for placement on the Board's Agenda.
Below are the major provisions of the attached Amended Agreements:

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of July,2012 by and between the Weld County
Department of Human Services' Area Agency on Aging,hereinafter referred to as "Nutrition Program"and
Commission Now, LLC,dba The Greeley Gopher,hereinafter referred to as "Vendor."
WITNESSETH:
WHEREAS,the "Nutrition Program" desires to continue to purchase transportation services from "Vendor", and
WHEREAS,the "Nutrition Program" and "Vendor" are desirous of amending the original agreement of July 1,2011;
NOW THEREFORE, it is mutually agreed:
1. The"Nutrition Program" will reimburse "Vendor" $6700.32 for the monthly base rate.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from July 1,2012 through June 30, 2013. This Amendment will be terminated
immediately in the event of loss of funding.
IN WITNESS WHEREOF,the parties hereto have executed this Amendment on the day first written above.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS,AMENDMENT made and entered into this 1st day of July,2012 by and between the Weld County Department

WHEREAS,the "Nutrition Program"and "Vendor"are desirous of amending the original agreement of January I,
2010;
NOW THEREFORE, it is mutually agreed:
1. The"Nutrition Program"will reimburse "Vendor"$5.40 for a complete meal, including '''/ pint of milk.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from July 1,2012 through June 30,2013. This Amendment will be terminated

ALL CONTRACTS MUST BE APPROVED BY THE STATE CONTROLLER
CRS §24-30-202 requires that the State Controller approve all state contracts. This Addendum #20 is not valid until the
State Controller, or such assistant as he may delegate, has signed it. Contractor is not authorized to begin performance
until the contract is signed and dated below. If performance begins prior to the date below, the State of Colorado may not
be obligated to pay for goods and/or services provided.
