RE: Amendment to the Agreement for Professional Services
between the Weld County Department of Human Services
and Devereux Cleo Wallace for Placement on the Consent
Agenda
Enclosed for Board approval is an Agreement for Professional Services between the
Department and Devereux Cleo Wallace. This Agreement was reviewed under the Board's
Pass-Around Memorandum dated February 9, 2012, for placement on the Board's Consent
Agenda.
The major provisions of this Agreement are as follows:

AMENDMENT TO AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
DEVEREUX CLEO WALLACE
This Amendment,made and entered into the day of January 2012,by and between the Board
of County Commissioners, on behalf of the Weld County Department of Human Services,hereinafter
referred to as"Department,"and Devereux Cleo Wallace,whose address is 8405 Church Rand Boulevard,
Westminster,Colorado, 80021,hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Day Treatment Services in
order to extend the term of the agreement and increase the maximum reimbursement under this agreement;
and
WHEREAS,Human Services has budgeied, appropriated, and otherwise nmde available; and
WHEREAS,required approval,clearance, and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as
follow:
I. Agreement for Professional Services, Item 2, is hereby revised to include the following:
TERM. The term of this Agreement shall be from September 19,2011,through and
until May 31,2012.
2. Exhibit A, Item 2, is hereby revised to include the following:
County agrees to purchase and Contractor agrees to furnish service at the cost of S95.00
per day for a maximum amount of$11,250.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day,
month,and year first above written.
